JOB SUMMARY:

To promote the mission of PATH to help individuals achieve a more independent and fulfilling life by being an effective team member. The Behavioral Health Rehabilitation Specialist will utilize a variety of approaches and interventions including the Boston University Approach to Psychiatric Rehabilitation and Motivational Interviewing to promote recovery within a site based program as well as provide successful community integration experiences. The Behavioral Health Rehabilitation Specialist will also provide individualized support and skill teaching in the community to promote new role development. Additionally, they will assist individuals in gaining access to needed psychiatric, medical, social, residential, financial and other services appropriate for the individual.

QUALIFICATIONS:

  • Bachelor’s Degree in Human Services or Rehabilitative Services.
  • Experience working with individuals who have a mental illness or a co-occurring disorder.
  • Active Driver’s license required.
  • Ability to use electronic health record required.

SPECIFIC DUTIES:

  • Maintain individual caseload under supervision of Program Manager
  • Actively engage individuals in services including outreach visits at the individual’s home, crisis center or outpatient facility
  • Engage and support individuals in all phases of the psychiatric rehabilitation process
  • Assist individuals in developing and practicing the skills defined on their psychiatric rehabilitation plans which may include accompanying individuals to appointments, leisure activities or other activities of interest such as religious, educational or vocational activities

  1. Maintain a supportive, therapeutic relationship with each service participant

  4. Oversee, teach skills and actively assist in the completion of household chores ensuring the home is neat and clean at all times
  5. Assist service participants in developing and practicing skills defined on their individual service plan. This may include accompanying residents to appointments, social activities and/or job opportunities, and other activities of daily living.
  6. Maintain ongoing communication with each service participant’s family/significant others utilizing diplomacy and tact and creating a welcoming environment

**Qualifications:**
+ Currently licensed as a registered professional nurse, or other licensed professional in specialty, in the state(s) of practice and/or has an active compact license, in accordance with law and regulation.

+ Basic Life Support (BLS) certification

**Clinical Rehabilitation Specialist**
We are currently looking for an ambitious Clinical Rehabilitation Specialist to join our team on a full-time/part-time/PRN/per diem/as-needed basis. A Clinical Rehab Specialist acts as a coordinator between internal/external referral sources and acute inpatient rehab operations. If you are dedicated to caring for the well-being of others, this could be your next opportunity.
Your duties and responsibilities may include:
+ ·Establishing relationships with case managers, physicians, payers and other referral sources
+ Ensure appropriate patient placement through clinical screenings and assessments in collaboration with both the hospital case management team and the Rehab Medical Director
+ Work closely with the hospital case management team to ensure a responsive and smooth-flowing referral and admission process for the inpatient acute rehab program
Requirements:
+ Graduate of an accredited school in clinical specialty
+ Minimum 2 yrs. recent clinical care experience
+ Licensed Professional in a clinical health care specialty (i.e., RN, Physical Therapist, Occupational Therapist, or Speech Therapist)
+ Pass Functional Independence Measurement (FIM) within 90 days upon hire, then maintain current certification
+ If RN, CRRN preferred and/or achieved within 2 years of hire
+ BCLS certification (AHA)
+ Clinical experience in the care and management of Inpatient Rehabilitation patients
+ Experience as a clinical liaison coordinating admissions for Inpatient Rehabilitation patients

**Job Summary and Qualifications**
The Registered Nurse demonstrates critical thinking skills, scientific judgment, and leadership in the anticipation and planning for the care of the patient.
