Job Title: Rehabilitation Assistant

Location: Nashville TN

Duration: Long Term for 12 Months

Rate: $19/hr. on 1099(Max Bill)

Schedule

  • Three (3) – 12.5-hour shift per week
  • Rotating schedule with Weekends, Nights, and Holidays required
  • Overtime

Responsibilities:

  • Follows the individual support plan to ensure that the needs and the active treatment requirements of persons served are met
  • Communicates with families, conservators, and employers of persons served and the public to share information and meet active treatment requirements. –
  • Analyzes medical treatment and training data to detect changes in needs, behavior, and abilities.
  • Analyzes environmental data to ensure the safety and appropriate environment for persons served.
  • Promotes teamwork and working together.
  • Trains other staff on active treatment interventions to meet the requirements of individual support plans of persons served.

Minimum Qualifications

  • Education and Experience: Education equivalent to graduation from high school and experience equivalent to one year of full-time work in teaching, business sales, and marketing, or providing services for disabled persons.
  • Substitution of Education for Experience: Additional college or technical coursework with at least nine quarter hours in education, a social or behavioral science, industrial arts, nursing, or mental health or human services field from an accredited college, university, or technical school may be substituted for the required experience.
  • Necessary Special Qualifications: A valid vehicle operator's license will be required for employment in this position.
  • All necessary training is provided on-site.
  • Pre-employment Drug Testing and a Criminal Background Check is Required.

-Completed three years of personal life experience with mental illness within the previous 10 years; or
-Completed three years of personal life experience as the primary caregiver to an adult with a mental illness, traumatic brain injury, substance use disorder, or developmental disability; or
-Holds an Associate of Arts degree; or
-Completed 15 semester or 23 quarter hours post-secondary education in behavioral science or related field; or
-Holds an unencumbered registered nurse license.
Previous experience in providing mental health services to adults is preferred. Good written and verbal communication and organizational skills, and the ability to work independently in an effective and efficient manner. Computer skills must be sufficient enough to be able to document information within an electronic medical record.
A valid driver's license may be required, depending on department/facility.

Shift: FT Days

Schedule: 40 hours weekly, rotating weekends and Holidays, 8:00am - 4:30pm

More about our team: Mosaic Life Care provides Inpatient Rehabilitation/Post-Acute Care in St. Joseph, MO. Therapy plays a very important role in recovery from a life-altering injury or illness. We offer a hospital-based physical rehabilitation program for the treatment of those who have experienced a debilitating injury or illness. Our Unit is a very fast-paced yet fun environment with endless opportunities to learn and grow!

What you will do in this role:

· Communicate patient progress or problems to supervisor and other team members; instruct patient's family or nursing staff in follow-through programs

· Assist with patient scheduling and post charges daily to patient records

· Document patient care in accordance with regulatory, licensing, payer and accrediting requirements

· Maintain equipment and work area in a safe and clean condition

· Handle job responsibilities in accordance with the standards of the Company's Code of Business Conduct, the Corporate Compliance Agreement, appropriate professional standards and applicable state/federal laws

· Other duties as assigned

Qualifications :

· Candidates must have certification from the American Occupational Therapy Association (AOTA)

· Graduate of an accredited American Occupational Therapy Assistant program culminating in an associate's degree

· Current and valid state AOTA license or certificate of practice or equivalent in the states where services are rendered

· Inpatient experience preferred

· CPR certification (when required)
