Job Summary
The Rehabilitation Specialist shall educate team members on the availability of meaningful community activities and facilitate member connection/referral/access to employment and rehabilitation services. Rehabilitation Specialist will educate team members on the value of meaningful community activities, including employment, psychiatric rehabilitation, work incentives and benefits planning. The Rehabilitation Specialist assesses, coordinates, links, monitors, and provides direct supportive and skills-based services in a community setting for members. This includes utilization of the Vocational Activity Profile or similar tool to assist members identify interests and connect to supportive services identified. Rehabilitation Specialist participates in the development of the member individual service plan (ISP) and provides/initiates/coordinates services related to the related ISP goal. The Rehabilitation Specialist is familiar with referral process and can complete referrals in a timely manner.
Education and Experience Required
- Bachelor's degree in a field related to behavioral health OR a combination of education, training, and/or experience meeting A.A.C. R and CBI Clinical Policies and Procedure standards for BHT.
- 1 year experience working with the behavioral health population with additional experience in multiple settings and levels of care, including both mental health and substance abuse
- Ability to pass criminal background check for jail visitation/clearance.

Essential Duties
- Provides skill building services to children, young adults, and families using a community-based approach. Services will be provided in-person and/or via telehealth.
- Conducts a thorough assessment and gathers all information pertinent to the case to assist with developing an individual service plan. Develops, implements, and monitors an individual service plan in conjunction with the client and family to achieve the desired outcomes.
- Monitors established measurable goals and routinely assesses the client's status and progress in order to proactively make appropriate recommendations for adjustments in the individual service plan and/or services to promote better outcomes.
- Establishes and maintains effective communication and collaboration with client's internal and/or external (Managed Care Plan) teams regarding client's status, services, and progress.
- Provide linkage and assistance to client in applying for and connecting with public and community benefits and resources, including educating clients on qualifying criteria for various benefits and resources.
- Submits timely documentation in compliance with contract and agency expectations and maximizing billable client hours/contacts according to caseload notes.
- Accurately completes all required documentation for assigned clients as required by contract within the agency's Electronic Health Record system and as measured by QA standards and audit results.
- Complies with productivity standards of the program and maintains an appropriate caseload to reach the budgeted daily expectation of the number of billable client hours/contacts. May be assigned to multiple program cases among more than one program based on needs
- Participates in supervision by coming prepared to discuss current caseload, documentation, and productivity; accepts constructive feedback and direction; and puts feedback into clinical and or therapeutic practice with clients.
- Maintains confidentiality of all information regarding clients, families, and operations of the Agency (including media).
- Maintains a professional relationship with clients, staff, and other outside stakeholders at all times.
- Exercises good judgment in the performance of duties and responsibilities.
Qualifications
- High School Diploma plus two (2) years of mental health experience (paid or unpaid) required. Bachelor's degree in a mental health related field, preferred. Must be 18 years or older.
- Mental health experience includes experience providing services to children and/or adults to improve or restore social, vocational, economic, housing and/or mental functioning
- Availability of a personal car with appropriate insurance which employee is willing to use for all Agency business if needed; adhering to federal motor vehicle safety standards when transporting children.
- Effective oral/written communication skills; bi-lingual skills in Spanish/English preferred
- Computer skills sufficient to meet production demands of the job and skill in maintaining detailed, accurate and complete records.
- Must have a valid California Driver License, proof of automobile insurance and meet the Agency's safe driver guidelines.
- Ability to work varied hours including evenings, nights and weekends, if needed.
Compensation Range: $22.50/hr - $0.98/hr

- To provide supervision, role modeling, skill coaching, and guidance to participants with mental health, developmental disabilities, and/or inappropriate sexual behaviors.
- To ensure interactions with participants actively support recovery-oriented social and emotional development, progress towards identified goals, and the acquisition of effective self-management skills.

Responsibilities:
- Supervises and supports assigned PRP Counselors with addressing client needs and accomplishing client goals.
- Meets with PRP Counselor regularly for an hour supervision to discuss caseload and ensure program standards are being met.
- Monitors PRP Counselor caseloads so that appropriate client interventions align to the client treatment plan and client goals for the program.
- Approves PRP documentation, ensuring that all internal and external standards are being met.
- Possession of a Master's Degree in Social Work, Professional Counseling, Addictions, or a related field.
- Minimum of two years of experience with a diverse and/or underserved population in the field of mental health.
- Holds a license to supervise PRP Counselors in the state of Maryland by meeting at least one of the following criteria:
