Works collaboratively with the Medication Renewal Center CMA and clerical team members to maximize productivity and efficiency Completes medication refill encounters, care due messages, and order entry based on established protocols. Provides clear EMR documentation in accordance with department standards regarding medication renewal requests, care due messages, patient care needs and/or follow up. Reviews patient compliance with medications by thorough review of the patient's electronic medical record. Communication of medication orders to designated pharmacies via verbal or electronic means. Provides general health information using approved references including, but not limited to, providing individual and group health and wellness education. Ensures documentation of care is complete and accurate. Supports maintenance and completeness of patient's electronic medical record, including medication reconciliation. Maintains and adheres to medication management and refill guidelines and protocols Coordinates with Primary Care Physicians and Nursing staff in order to meet the needs of the patient. Review medication renewal requests received from pharmacies and patients under a variety of methods including but not limited to electronic communications, fax, or phone calls. Identifies care gaps and promotes health standards and activities to prevent non-compliance with chronic medication management. Reviews current, past, and future appointments and lab work for compliance with safe medication administration. Communicates with pharmacies regarding discrepancies and/or clarification for medications Disseminate education to patient as written by provider Reports medical information obtained from patients accurately, completely, and in a timely manner ensuring accuracy of the information available within the EMR. Communicates with patients, providers, care teams, and pharmacies of any changes to medications to reduce pharmacological errors. The Supervisor will report to the director of pharmacy shared services and will be responsible for the supervision of the medication renewal center team. This individual will be responsible for the day to day operations and oversight of a team of pharmacist and pharmacy technicians. This individual will also work closely with the director of pharmacy shared services on various projects, implementation and management of shared pharmacy services.
**Job Description:**
1. Oversees the performance of the various pharmacist and pharmacy technicians and works closely with leadership to meet departmental goals and performance. (essential)
2. Monitor and provide reporting on key performance indicators and meet with staff on a routine basis to have performance discussions (essential)
3. Plans, reviews and optimizes workflows within the medication renewal center ensuring maximization of efficiencies. Ensures the proper work flow and intakes distribution through departments, and schedules and delegates work load amongst team members. (essential)
4. Ensures employee training and quality of work through development of training materials, administration of training and continuous quality control. (essential)
5. Ensures consistent communication with staff, patients and providers to ensure proper and timely renewal of prescriptions. (essential)
6. Manages direct reports' work load to effectively manage cost and audits and verifies team members work to ensure information accuracy. (essential)
7. Monitors situations for fraud, waste, and abuse of Medicare/Medicaid benefits and reinforces HIPAA and PHI protocols. (essential)

**Required Qualifications:**
1. Bachelor's degree in Pharmacy required.
2. 3-5 years related work experience required in pharmacy.
3. Strong understanding of data & analytics.
4. Clinical/ambulatory care pharmacist experience.
5. Understanding of insurance and adjudication processes. Knowledge of 3rd Party Insurance Plans.
6. Experience with computer systems required, including web based applications and some Microsoft Office applications which may include Outlook, Word, Excel, PowerPoint or Access.
**Preferred Qualifications:**
1. Specialty Pharmacy, 340B and/or contract pharmacy experience.
2. Management/supervisor experience in pharmacy.

Required Qualifications:

The MRC RN supports Carle Health Primary Care patient's medication management, adherence, and refill needs by providing advanced level patient care and support. MRC RN's utilize Physician led guidelines and protocols to assist in efficient decision making and enhances patient compliance with health maintenance and chronic medication management. Responsibilities: Works collaboratively with the Medication Renewal Center CMA and clerical team members to maximize productivity and efficiency Completes medication refill encounters, care due messages, and order entry based on established protocols. Provides clear EMR documentation in accordance with department standards regarding medication renewal requests, care due messages, patient care needs and/or follow up. Reviews patient compliance with medications by thorough review of the patient's electronic medical record. Communication of medication orders to designated pharmacies via verbal or electronic means. The Medication Renewal Center Pharmacist provides comprehensive clinical pharmacy support in collaboration with the clinics of BILH in managing the prescription refill process with the goal to improve operational efficiencies and deliver enhanced refill and medication management services. The pharmacist will practice under a scope of agreement to provide appropriate and safe refill authorization. This position will also support pharmacy operations under the supervision of the Supervisor, Medication Renewal Center.
The pharmacist is expected to provide continuity of care to patients who require prescription renewals and enhance patient care through education, monitoring and follow-up. The pharmacist will engage with patients and educate about the clinical benefits of utilizing our in house pharmacies.
**Job Description:**
Essential Responsibilities:
Practices pharmacy in accordance with laws, rules and regulation of the Massachusetts Board of Registration in Pharmacy, Department of Public Health, U.S. Food and Drug Administration, and Drug Enforcement Administration and in accordance with established departmental policies and procedures. Maintains credentials in accordance with Massachusetts Board of Registration in Pharmacy.
Monitors patients in a specific specialty service/ pharmacy, including collecting patient- specific data, interpreting drug-related laboratory results, performing relevant physical assessments, and identifying patient goals of therapy.
Under the scope of practice agreements, reviews drug therapy based on information obtained through assessment of medical records, objective monitoring parameters, and review of medication profile to determine appropriateness of renewal request.
Provides patient monitoring for the purpose of evaluating and optimizing patient outcomes, drug utilization, patient compliance, detecting and/or minimizing ADR's, and eliminating unnecessary drug use and duplication.
Supports pharmacy services by contacting patient for clarification regarding any medication related matters.
Required Qualifications:
Bachelor's degree in Pharmacy required.
License Massachusetts Pharmacist required.
Minimum of 2 years related work experience required.
Understanding of insurance and adjudication processes.
