Job Description
Develops and monitors the social services programs for the CSA. Establishes and maintains, standards, quality levels, cost effective, and customer focused services. Establishes interfaces with community services.
Essential Responsibilities:
+ Plans, develops, implements and maintains social services policies and procedures, delivery of social work programs, and discharge planning services.
+ Oversees social services case management activities.
+ Manages staff activities, ensures their competencies, and develops/presents inservices.
+ Recommends, develops, and implements new programs enhancing services to specialized patient populations and reducing hospital and office visit utilization.

+ Collaborates with medical center departments, community agencies and health plan members on matters related to discharge planning and ensures that services are provided in an integrated fashion.

Basic Qualifications:
Experience

Proactively identifies and plans for capital needs related to current equipment and future department projects. Ensures staffing plans and schedules meet department needs that reflect understanding of the importance of cost-effectiveness.
+ Creates department strategies to achieve financial target and staffing needs, through optimizing productivity, supply/resource efficiency, minimizing incidental overtime and overtime percentage, and other areas according to department specifications.
+ Ensures that outcomes management activities are integrated and maintained in a clinically appropriate manner. Identifies and plans strategies to reduce length of stay and resource consumption while providing excellent customer/patient service.
+ Plays a leadership role in utilization review committee which includes identification of opportunities for appropriate utilization of resources to avoid denials and resource management as it pertains to delivery of care.

Summary:
The Social Services Worker provides psychosocial support to residents and their families.
Qualifications:
- Education: BSSW, MSSW preferred
- Licenses/Certification: Licensing as required by the state of employment.
- Experience: One year of experience in a long-term care environment.
Job Functions:
- Provides direct psychosocial intervention.
- Performs resident assessments at admission, upon condition change and/or annually.
- Creates, reviews and updates care plan and progress notes.
- Provides direct psychosocial intervention.
- Coordinates resident visits with outside services, dental, optical, etc.
- Attends and documents resident council meetings.
- Assists resident’s families in coping with skilled nursing placement, physical illness and disabilities of the resident, and the grieving process.
- Works with the patient, family, and other team members to plan discharge.
- Conducts in-service programs to educate staff regarding psychosocial issues and patient rights.
- Supervises and guides Social Services Assistants.
- Performs other tasks as assigned.
Knowledge/Skills/Abilities:
- Ability to work cooperatively as a member of a team.
- Ability to communicate effectively with residents and their family members, and at all levels of the organization.
- Ability to maintain confidentiality.
- Knowledge of psychosocial practices applicable to a long-term care environment.
- Skilled in directing and motivating the workforce.

The Community and Social Services team is dedicated to helping patients and their families cope more effectively with the life changes and challenges associated with a hospitalization. These experienced professionals are vital components of a multidisciplinary health care team that recognizes how important it is to support hospitalization and health care treatment in a variety of ways. When a patient is hospitalized, it can impact everyone in the family, and our Community and Social Services department strives to provide comprehensive support in a caring and compassionate manner.
If you are compassionate and empathetic, you could be our next Social Worker and provide invaluable counseling and guidance services to patients and their families undergoing traumatic or challenging situations. Your responsibilities include providing assessment, counseling and referral services to patients and their families. We are looking for someone sensitive and perceptive, with a strong sense of community care and social service, who shares our commitment to delivering the highest quality health care to Hawai'i's people.
