**As a PRN Hospice RN/ RN Case Manager, you will:**
+ Reportdirectly to the Administrator, Executive Director or Patient Care Manager Senior and will maintain primary control and professional management of each patient.
+ Act as primary liaison between physician, patient/family and hospice team.
+ Provide direct patient care services to hospice patients and supportive care to the patient's family with a reasonable case load while providing a trusting and compassionate relationship with your patients and their families.
+ Empower patient's caregivers by educating them on care for their loved one
+ Manage and collaborate with an interdisciplinary group consisting of Licensed Practical/Vocational Nurses (LPN or LVN), Certified Nursing Assistants (CNA) and support staff to provide comfort, peace, and dignity
+ Evaluate patient and family needs and assist in the preparation and execution of the plan of care for each patient.
+ Maintain all assigned patient clinical records.
+ Participate in company quality assessment and performance improvement programs.
**About You**
**Hospice Registered Nurse Requirements:**
+ Hospice heart: compassionate, kind and empathetic to your patients and their families.
+ Comfortable providing care wherever the patient calls "home".
+ Demonstrate flexibility and composure while supporting others in a time of crisis.
+ RN license in the state of requested employment (or eligibility to obtain).
+ 1+ years of nursing experience in hospice, home health, ICU, geriatric, or oncology strongly preferred.
+ Additional, preferred experience to help you be successful in a hospice role: hospital Staff RN, Med Surg, PACU, Float pool, ER or emergency room, critical care, acute care, cardiac, geriatric, admissions, wound care, telemetry or travel nursing.
+ Current driver's license and automobile insurance.
+ Current CPR certification

Supervisory Position:
The Clinical Director/Director of Patient Care Services is responsible for the overall direction of hospice clinical services. The Clinical Director/Director of Patient Care Services establishes, implements and evaluates goals and objectives for hospice services that meet and promote the standards of quality and contribute to the total organization and philosophy.
Requirements
- Minimum of two (2) years' experience in the hospice industry, and is a registered Nurse in the State of CA.
- Demonstrated ability to supervise and direct professional and administrative personnel.
- Ability to market and deal tactfully with customers and the community.
- Must be a licensed driver with an automobile that is insured in accordance with state and/or Organization requirements and is in good working order.
- Has excellent observation, verbal and written communication skills.

- Assists the Executive Director/Administrator in the preparation and administration of the organization's budget.
- Interpret operational indicators to detect census changes and increases or decreases in volume that could impact staffing levels, revenues or expenses.
- Evaluates performance of Clinical Staff
- Conducts annual evaluations on clinicians, or more frequently if indicated.
- Oversee the maintenance of patient clinical records, statistics, reports and records for purposes of evaluation and reporting of organization activities.
- Assures proper maintenance of clinical records in compliance with local, state and federal laws.
- Develops, implements and evaluates the orientation program for new organization personnel. Plans and implement Inservice and continuing education programs
- Assists with the evaluation of organization performance via performance improvement program, productivity, quarterly and annual reviews.
- Assures for the quality and safe delivery of hospice services provided through the Organization.
- Assists in the development of organization goals. Develops, recommends, and administers Organization policies and procedures.

Responsibilities
- Utilizes the nursing process to ensure quality palliative care to all clients and their caregivers/families.
- Assumes screening/admission responsibilities of clients that are referred to hospice and gathers and documents appropriate medical and financial data to assure a timely admission to hospice as appropriate.
- Initiates the plan of care in collaboration with the interdisciplinary team upon admission and continues to provide quality nursing care on an intermittent basis following the plan of care.
- Completes thorough ongoing systematic assessments on all clients based upon client status/needs.
- Implements, modifies and evaluates the plan of care on an ongoing basis.
- Evaluates, formulates and communicates education with the client, caregivers and families based on their changing needs.
- Supervises and directs client care provided by the Hospice C.N.A.'s.
- Ensures quality care management by providing appropriate documentation, communication, collaboration and other follow-up and by using time, supplies and resources productively.
- Ensures quality and safe delivery of care.
- Respects client confidentiality in all communication.
- Communicates to the physician changes in the client's condition or status.
- Obtains, interprets, clarifies and implements physician orders.
- Communication and collaboration with the interdisciplinary team as client needs change.
- Attends and participates in the weekly Interdisciplinary Team Meetings and monthly nurses meetings.
- Participates in departmental task teams as appropriate.
- Reports to the Clinical Coordinator or designee all problems encountered with client care.

- Is responsible for rotating on-call coverage for weekends and holidays as assigned.
- Prioritizes on-call client triage as appropriate.
- Responds positively/willingly to requests to assist in covering extra nursing visits and on-call coverage.
- Demonstrates evidence of professional growth and development by maintaining a current knowledge base regarding nursing and palliative care trends. Shows evidence of quality improvement in the processes of hospice care.
- Assumes responsibility for being informed about developments in health care and specifically Hospice care.
- Identifies personal and professional learning needs by participating in educational opportunities.
- Shares education with staff members, clients and caregivers/families as appropriate.
- Maintains all departmental and MMCI educational requirements including, but not limited to:
- Current Illinois License
- CPR Certification
- IV Therapy
- Competencies
- Other certifications required by MMCI and/or Hospice.
- Is knowledgeable and able to identify payer source requirements including, but not limited to, Medicare, Medicaid and private insurance.

Compensation and Benefits
The compensation range for this position is $33.62per hour - 57.83per hour. This represents a good faith minimum and maximum range for the role at the time of posting by Carle Health. The actual compensation offered a candidate will be dependent on a variety of factors including, but not limited to, the candidate’s experience, qualifications, location, training, licenses, shifts worked and compensation model. The RN Nurse Clinician provides holistic care to terminally ill pediatric or adult patients and their families by assessing the needs of the patient, and then planning, coordinating, implementing, and as needed, revising the Plan of Care. The RN Nurse Clinician works collaboratively with the interdisciplinary hospice group (IDG)with the goals of anticipating changes in the patient's condition, avoiding crises, promoting patient and family choice, ensuring continuity of care, and enhancing the quality of life of dying patients. Shift, schedule and caseload of shifts and caseload may vary according to position assignment.

Required Qualifications:
- Associate's Degree in Nursing or
- Bachelor's Degree in Nursing or equivalent educ/experience
- Washington Registered Nurse License upon hire

- 1 year of Nursing experience; some pediatric nursing experience required for a position in the Pediatric Programs.
