This position supports DCHD's Vaccine for Children (VFC) program within Delaware County school districts. The Liaison worker assists with managing the County's immunization service delivery, as well as assists in training development, administration, and quality control procedures. The liaison worker provides support for the Public Health Emergency Preparedness School Nurse Project. Additionally, the school nurse liaison provides consultation on issues related to reportable diseases and other school health concerns; and participates in conferences and meetings with school nurses, administrators, childcare providers, and other community members.
Essential Duties
* Assures that the health needs of disparate populations in Delaware County are addressed related to at risk and vulnerable populations as appropriate.
* Assists and/or facilitates the creation of school health topics and school nursing policies and procedures to support compliance with state and federal statutes, in addition to accepted standards of nursing practice.
* Works with local schools and childcare centers to identify needs, gaps, resources, and priorities related to school health and early childhood especially as it related to COVID-19 response and recovery.
* Provides consultation and professional development to school health personnel, school nurses, parents/guardians, and others regarding school health and school nursing issues.
* Assures adherence to requirements for mandated vaccines and promotion of infectious disease control and safety.
* Analyzes school health records and reports in order to develop and/or modify health programs to meet healthcare needs of school communities.
* Ensures that state immunization program is managed appropriately in schools as mandated by the PA Department of Health.
* Provides nursing consultation in communicable disease control and serves as liaison between the Delaware County School Districts and the DCHD.
* Maintains knowledge of current laws and regulations pertaining to nursing, health care, and public health as it relates to school health in particular.
* Consults with RN's, school district staff, school district administrative staff, parents, other school districts, and the public regarding health issues affecting students in schools.
* Attends pertinent workshops, conferences or training sessions in order to keep up with the-school nursing "Standard of Practice".
Keeps current with local school districts, national and international disease trends, and outbreaks.
* Assists VFC nursing staff with vaccine administration at the DCHD Wellness Centers and events as needed.

The City of Fishers is seeking a full-time Public Health Nurse (PHN) - School Liaison to join our Health Department. This role is supported by Health First Indiana funding. As the designated school liaison, this role will work closely with Fishers K-12 schools, preschools and daycares, to support Fishers schools in public health response and prevention activities. This role is also a member of the public health nursing team, which includes providing clinical and case management support for other FHD programs.

Job Summary:

Responsibilities and Duties:

The Shasta County Health and Human Services Agency (HHSA) partners with communities to protect and improve the health and well-being of Shasta County residents. Established in 2006, the HHSA is organized into four branches: Behavioral Health & Social Services, Public Health, Economic Mobility, with a centralized Administration Services. The HHSA promotes optimal health for all. The frail, the abused, the mentally ill and those without resources look to the HHSA for services and resources to meet their basic needs, and to protect their health and safety. This transformation improves efficiency, avoids unnecessary duplication of effort, encourages collaboration among programs and organizes the HHSA's services to be more responsive to the needs of the public.
The Public Health Branch focuses on community-wide prevention of communicable disease, chronic disease, injury, substance abuse, family violence, and lead poisoning. Public Health helps the community develop policies and support community environments that encourage healthy behaviors. It promotes nutrition and physical activity, healthy aging, worksite wellness and healthy communities. Public Health is also where the HHSA's emergency response unit is headquartered, and its laboratory provides testing services for Shasta and numerous other Northern California counties.
ABOUT THE POSITION
Under the direction of the Specialty Health Services Program Manager, plans, organizes, supervises and evaluates the activities of Public Health Nursing and other assigned staff providing public health services; acts as liaison with other divisions within the Health Department, with other provider staff and community agencies; works to provide, interpret, and improve public health services in the absence of the Specialty Health Services Manager; and perform related work as required.
DISTINGUISHING CHARACTERISTICS
Positions in this class are distinguished from Public Health Nurse II positions by having the responsibility for directing Public Health services by providing leadership, responsibility, and supervision for the staff assigned to the team. Positions in this class are distinguished from PHN III positions by this position's primary responsibility for communicable disease control and the oversight of Public Health Nursing field services.

- 17 working days a year - 10 through 15 years of service.
- 20 working days a year - 16 or more years of service.
Note - accrued vacation leave may not be used until after six months of employment. For purposes of vacation accrual, a working day is no more than 8 hours.
Employees may elect to receive a payment for Vacation hours (in lieu of use) for up to twenty hours - in five whole hour increments - of accrued vacation. *Subject to criteria as outlined in section 13.8C I n the Shasta County Employees Association MOU.
Sick Leave:
- 96 hours of sick leave are earned each year with no maximum accrual.
- Employees terminating in good standing after completion of 5 years of continuous service will receive a pro-rata payoff of accumulated sick leave.
- Unused sick leave may be converted to PERS service credit upon retirement.
Note - accrued sick leave may not be used until after three months of employment. Accrued paid leave benefits are used in conjunction with FMLA/CFRA as appropriate.
*Vacation and Sick Leave accruals are based on regularly scheduled working hours. Additional time will not be accrued on hours worked over 80 hours within a one pay period.
Bereavement:
- 24 hours for each non-concurrent death in the immediate family.
Employees in the Teamsters bargaining unit are insured through a Teamsters Trust plan.
Depending on the plan selected, the County pays most, or all, of the employee premium, and contributes a portion of the dependent premium.
Contributions are made for a portion of a retiree's insurance premium.
Dental Plan:
The County pays a portion of the employee's and dependents' premium for dental insurance.
Vision Plan:
The County pays the employee premium and employees may enroll dependents at their own expense.
Life Insurance:
The County pays the premium for life insurance for most employees. The amount of coverage varies by bargaining unit. For SUPV employees the current coverage amount is 46,000.
Additional Life insurance is available to purchase through Aflac and The Standard.
Retirement:
The County is in the Public Employees' Retirement System (CalPERS) coordinated with Social Security.
- Members hired prior to May 8, 2011 are covered under the retirement formula of 2% @ 55 benefit.
- Members hired after May 8, 2011 but before January 1, 2013 are covered under the retirement formula of 2% @ 60 benefit.
- Members hired after January 1, 2013 are covered under the California Public Employees Pension Reform Act (PEPRA) with a benefit formula of 2% @ 62.
Employees pay 9.5% of his or her salary, on a pre-tax basis for a period of time as specified in his or her labor contract, or Personnel Rules as appropriate. Members hired prior to 2013 may have a different contribution percentage.

Summary of Position
The Health Department's Public Health (PH) Bureau is seeking to fill two (2) fulltime Supervising Public Health Nurse (SPHN) positions in the Enhanced Care Management (ECM) program.
The Supervising Public Health Nurse (SPHN) coordinates and supervises the operation of health-related services and programs involving comprehensive case management, education, prevention, and collaboration with and coordination of community resources. These services are delivered by a team of professional, para-professional and support staff that collaborate to ensure comprehensive health services are provided throughout Monterey County. The supervisor uses principles of reflective practice and staff development to provide primary support and appropriate oversight to the nurse home visitors to build and model a strengths-based client-centered culture.
The ECM Program serves clients who are experiencing homelessness, have chronic medical conditions, are high utilizers of the emergency department and/or hospitalizations, and/or have severe mental health diagnosis. As part of the CalAIM initiative, the Bureau's ECM program is expanding to serve a larger proportion of individuals in Monterey County eligible for services. The SPHN will lead a multidisciplinary team of Community Service Aide IIs, Behavioral Health Aides, Licensed Vocational Nurses, and Public Health Nurse IIs. In this position, the incumbent will hire and train new team members, assign and review the workload of the team, and coordinate with partners. This will include the evaluation of program effectiveness, and provide technical expertise and recommend and implement policies and procedures to ensure ECM's daily operations comply with both long and short-term program goals and objectives and agency policies and Procedures. Additionally, the incumbent will also assist the PH Program Manager track expenditures and draft reports.

The Shasta County Health and Human Services Agency (HHSA) partners with communities to protect and improve the health and well-being of Shasta County residents. Established in 2006, the HHSA is organized into four branches: Behavioral Health & Social Services, Public Health, Economic Mobility, with a centralized Administration Services. The HHSA promotes optimal health for all. The frail, the abused, the mentally ill and those without resources look to the HHSA for services and resources to meet their basic needs, and to protect their health and safety. The Public Health Branch focuses on community-wide prevention of communicable disease, chronic disease, injury, substance abuse, family violence, and lead poisoning. Public Health helps the community develop policies and support community environments that encourage healthy behaviors. It promotes nutrition and physical activity, healthy aging, worksite wellness and healthy communities. Public Health is also where the HHSA's emergency response unit is headquartered, and its laboratory provides testing services for Shasta and numerous other Northern California counties.
ABOUT THE POSITION
Under the direction of the Specialty Health Services Program Manager, plans, organizes, supervises and evaluates the activities of Public Health Nursing and other assigned staff providing public health services; acts as liaison with other divisions within the Health Department, with other provider staff and community agencies; works to provide, interpret, and improve public health services in the absence of the Specialty Health Services Manager; and perform related work as required.
DISTINGUISHING CHARACTERISTICS
Positions in this class are distinguished from Public Health Nurse II positions by having the responsibility for directing Public Health services by providing leadership, responsibility, and supervision for the staff assigned to the team. Positions in this class are distinguished from PHN III positions by this position's primary responsibility for communicable disease control and the oversight of Public Health Nursing field services.

Summary of Position
The Health Department's Public Health (PH) Bureau is seeking to fill one (1) full-time Supervising Public Health Nurse (SPHN) position in the Enhanced Care Management (ECM) program.
The Supervising Public Health Nurse (SPHN) coordinates and supervises the operation of health-related services and programs involving comprehensive case management, education, prevention, and collaboration with and coordination of community resources. These services are delivered by a team of professional, para-professional and support staff that collaborate to ensure comprehensive health services are provided throughout Monterey County. The supervisor uses principles of reflective practice and staff development to provide primary support and appropriate oversight to the nurse home visitors to build and model a strengths-based client-centered culture.
The ECM Program serves clients who are experiencing homelessness, have chronic medical conditions, are high utilizers of the emergency department and/or hospitalizations, and/or have severe mental health diagnosis. As part of the CalAIM initiative, the Bureau's ECM program is expanding to serve a larger proportion of individuals in Monterey County eligible for services. The SPHN will lead a multidisciplinary team of Community Service Aide IIs, Behavioral Health Aides, Licensed Vocational Nurses, and Public Health Nurse IIs. In this position, the incumbent will hire and train new team members, assign and review the workload of the team, and coordinate with partners. This will include the evaluation of program effectiveness, and provide technical expertise and recommend and implement policies and procedures to ensure ECM's daily operations comply with both long and short-term program goals and objectives and agency policies and Procedures. Additionally, the incumbent will also assist the PH Program Manager track expenditures and draft reports.

The Eligible List established by this recruitment process may be used to fill current and future vacancies on a regular full-time, part-time, or temporary basis County-wide.
* Plans, assigns, and reviews the work of staff. Assists in hiring staff and recommends disciplinary action when appropriate; approves staff leave requests. Evaluates staff training needs and develops strategies for addressing training needs; promotes team building.
* Coordinates program operations with other county departments as well as other agencies.
* Coordinates, supervises, and directly participates in carrying out a variety of complex and varied health services programs.
* Assesses and anticipates program needs and recommends and implements policies and procedures designed to assure that daily operations comply with both long and short-term program goals and objects and agency policies and procedures.
* Provides technical expertise concerning the medical, nursing, technical, professional and or administrative elements of programs; evaluates and makes decisions on difficult programs or admirative problems.
* Prepares annual program planning documents; analyzes and provides cost and revenue input for budgets, grants, etc. Administers grant funds and budgets; evaluates financial aspects of programs relative to approved budgets and service plans. Participates in developing division objectives and priorities.
* Establishes methods for evaluating program effectiveness.
* Serves on relevant committees and task forces, as assigned; serves as liaison to state and other governmental agencies.
* Develops and submits a variety of management reports and program evaluation.

Any combination of training, education, and/or experience which provides for the knowledge, skills, and abilities required to perform the duties listed above is qualifying. An example of a way these requirements might be acquired is:
Education: Completion of coursework leading to a Bachelor's Degree in Nursing with a certificate in Public Health Nursing which included courses in the principles of supervision and administration and public health.
AND
Experience: Two years of progressively responsible professional public health program experience which include lead-worker level responsibilities, and which provide a broad knowledge of public health programs.
CONDITIONS OF EMPLOYMENT
The required conditions of employment include, but are not limited to the following:
* Pursuant to Title 17 of the California Administrative Code, possession of a license in active status, to practice as a registered nurse in California and possession of a Public Health Nursing Certificate.
