**Job Summary and Qualifications**
**The Case Management Assistant (CMA) will work under the direction of the RN case manager or social worker to assist with facilitating patient discharges requiring post-acute placement or services, and will identify and report barriers to discharges. The CMA will provide administrative support in Case Management functions.**
**What will you do in this role:**
+ Coordinates with RN case manager and social worker for support needed. Communicates updates on progress throughout the day. Refers patient questions back to the case manager/social worker.
+ Provides clerical support in multidisciplinary rounds, including updating NATE Tempo at direction of team.
+ Uses web-based referral system to manage post-acute care (PAC) referrals. Includes sending initial referrals with appropriate clinical information from the medical record required for the level of care.
+ Monitors patients with PAC referrals and intervenes to expedite discharge.
+ Prioritizes workload to ensure a timely contact and intervention with PAC providers.
+ Sends additional information to PAC provider when requested.
+ Communicates with RN case managers/social workers regarding the status of PAC referrals, bed availability, and other barriers that cannot be resolved.
+ Communicates results of PAC referrals with the patient, including having the Patient Choice letter signed.
+ Monitors Medicare patient's planned discharge dates and delivers the Important Medicare Message (IMM) to the patient per facility policy.
+ Obtains physician signature on required Case Management documents and forms.
+ Based on facility practice; sets up transportation/sends information to Transfer Center and generates discharge packets for PAC transfers.
+ Utilizes Case Management systems, standards, and processes. - Maintains knowledge of and actively develops relationships with PAC providers.

Job Description
Under the supervision of the Team Manager, this position will set up and maintain product information for all business divisions of C&S Family of Companies, across all product types, and in support of all C&S facilities nationwide. This position requires knowledge and understanding of details related to the set up and maintenance of item/product information and technical knowledge of how systems handle data. This position is also responsible for processing high volume update requests, while aware of specific nuances for each request type, in support of business units along with maintenance of daily reports and audits to maintain the best data possible. This role requires users to learn how to query relational databases within tools such as PLSQL and OBI. This role is accountable for upholding strict SLAs, good communication with all business entities, and ad-hoc reporting requirements as directed by the Manager or Analyst. This role may be assigned long-term data cleansing tasks at an introductory level. Project work will be introduced on a case-by-case basis, and individuals will be expected to execute any project tasks with a quick turnaround and high level of data integrity. This role requires individuals to apply critical thinking and be analytical with each activity to ensure correct action is taken.

+ Perform New Item setup company-wide within established time parameters based on email requests or assigned lists distributed by the MDM Manager, Associate Analyst, or Analyst.
+ Process email requests for data updates in support of business (ordering) and warehouse operations (shipping/receiving) with minimal delays and based on SOP rules
+ Complete item audits and assigned reports at assigned intervals to review and correct identified data errors in support of best data possible.
+ Track and report daily KPIs, including number of emails resolved, how many items created, and how many changes made.
+ Communicate both internally and externally to requests, validate and/or acquire missing/inaccurate information related to item data as needed
+ Act as an information resource for internal and external customers as relates to product and master data.
+ Contribute to special tasks as assigned and directed by Manager.
