Description

Position Summary & Essential Duties :

This position is a member of the University Hospitals leadership team with responsibility for leading the Highly Reliable Medicine and Bundle Payments for Care Improvements Initiatives.

HRM & BPCI-Advanced

1. Ensures Appropriate Hospital Admissions while promoting ambulatory volume growth by:

a. Working with Emergency Medicine Team and PCI for diversion of low acuity cases to ambulatory care

b. Aligning with Palliative Care team and Joint Venture Hospice of Western Reserve to build palliative care triggers and

infrastructure to support volume growth

c. Engaging Social Work and Postacute care team for diversion of patients with nonmedical, socioeconomic needs to

reduce high LOS due to placement failure and inpatient denials

2. Ensures Clinical Effectiveness for Revenue Optimization through:

a. Working with Operational Engineers and service line leaders to process engineer Clinical Practice Guidelines

for all High Reliability Medicine Initiatives

b. Engaging Utilization Management to ensure Inpatient criteria for admissions is embedded in Clinical Practice

Guidelines for Emergency Medicine and Hospitalist Physicians

c. Engaging Lab, Pharmacy, Radiology, ICU to ensure appropriate use criteria is met to decrease unnecessary utilization

of services

d. Reducing LOS and variable cost per case through reduction in care variation and waste

e. Working with Utilization Management and System Physician Advisors to optimize revenue through enhanced clinical

decision support and documentation

3. Optimizes Next Site of Care decisions by:

a. Creating standardized process for next site of care decisions which include medicine team, home health, social work

and therapy

b. Implementing evidence based guidelines for determining next site of care according to national standard of care

improvement during the discharge process

c. Ensuring hospitalist are trained on new core competency requirements from the American Hospitalist Association as it

pertains to care transitions

d. Engaging postacute site leadership (IRF, home health, SNF) in providing high quality care to patients discharged to

their sites

4. Develops reliable process for readmission prevention by:

a. Developing Ambulatory Dose for Readmission prevention and scaling as a system wide process

b. Working with hospital leaders to develop Transitional Care Clinics for system wide readmission reduction and ED

diversion

c. Aligning all forms of patient education during the continuum of care and developing distribution channels for delivery

d.Conducting system wide Root Cause Analysis on readmissions to implement process to improve care delivery

Customers
  • Initiates and maintains positive relationships with co-workers.
  • Creates strong relationships with System Physician Leaders, Institute Leads and Chairs
  • Consults with fellow members of leadership to conduct overall needs assessments to ensure project needs are met.
  • Directly oversees staff members, and is responsible for ensuring their work output is satisfactory and their concerns are resolved.
  • Proactively seek input from UHs management and staff to promote mutual understanding/respect and gain insight into their operations and issues.
  • Ability to partner with Senior Executives to plan strategic initiatives, monitor project risk, and resolve significant issues.
  • Skilled with managing customers and building relationships at the Physician, Director, Vice President, Senior Vice President, and Senior Executive level.
Process
  • Educate the organization on the HRM and Bundle Payments for Care Improvement methodology.
  • Establish project timelines and milestones/deliverables.
  • Conduct project post mortems and create a recommendations report in order to identify successful and unsuccessful project elements.
  • Plans, develops and manages a significant operating plan and associated the HRM budget (>$1M).
  • Effectively oversee project teams time and allocate resources to ensure deliverables are completed.
Knowledge
  • Leads the development of executive board presentations on HRM and BPCI initiatives (including change management principles).
  • Define how the scope, goals, and deliverables of projects should support clinical and operational goals, in collaboration with senior management.
  • Define project success criteria and disseminate them to involved parties throughout the project lifecycle.
  • Develop best practices, techniques, and tools for project planning, execution, project management, ongoing milestone/deliverable tracking, communication, and key performance metrics.

Performance Management

  • Builds a high-performance team by developing, challenging, supporting individuals to achieve their highest potential.
  • Responsible for talent management on the team including talent selection (hiring), conducting performance reviews, delivering team and individual recognition, and maintaining team dynamics (firing).
  • Coach, mentor, motivate and supervise new physician team leads, team members. Influence staff to take positive action and accountability for their assigned work.
  • Identify and resolve issues and conflicts within and between various project teams.
  • Develops associates in their growth toward greater understanding of information systems and the healthcare environment.
  • Provides clear points of contact; creates and maintains environment that fosters open, honest and transparent dialogue.
  • Commitment to work collaboratively prior to escalation. Is available and responsive.
  • Provides leadership and direction for staff.
  • Develop internal team promoting engagement and growth in their positions and within UH.
  • Strong interpersonal and teamwork skills.

The Food Service Worker will assist the manager with food/meal preparation; maintain cash receipts and meal records. Assist manager in completing daily reports. Maintain high standards of quality in food production, sanitation, and kitchen safety practices.
**Job Responsibilities**
+ Prepare quality food and baked goods according to a planned menu
+ Prepare a daily report that verifies transactions
+ Understand what is inclusive of a meal
+ Ensure storage of food in an accurate and sanitary manner
+ Serve food according to meal schedules, department policies and procedures
+ Use and care of kitchen equipment, especially knives
+ Timely preparation of a variety of food items, beverages, and
+ Add garnishments to ensure customer happiness and eye appeal
+ Coordinate and assist in major cleaning of refrigerators, freezers, and cooking and serving equipment
+ Adhere to all food safety regulations for sanitation, food handling, and storage
+ Adhere to the uniform policy
+ Connect with the Manager daily to understand and accurately prepare menu for the day
+ Supervise the food temperature requirements
+ Maintain a clean and organized work and storage area
+ Scrub and polish counters, clean and sanitize steam tables, and other equipment
+ Follow established procedures and standards for cleanliness, to ensure a balanced and safe environment; duties include sweeping, moping, ware washing
+ Maintain garbage collection site and kitchen floor areas in a neat and sanitary fashion
+ Perform other duties as assigned including other areas in the kitchen
+ This role may have physical demands including, but not limited to, lifting, bending, pushing, pulling and/or extended walking and standing. This role may also require uniforms and/or usage of Personal Protective Equipment (PPE).

WHAT WILL YOU DO:

* Our Talented Advanced Practice Providers plan, evaluate, and implement high quality medical care with appropriate resource management to patients, both adults and children in cooperation with center physicians
* Adheres to the service and courtesy standard of Prisma Health Urgent Care by delivering the highest standard of patient satisfaction at all times
* Provides patient care within the respective clinical area which complies with the Professional Staff Bylaws, rules and regulations and University Hospitals Urgent Care policies and procedures
* Provides and/or serves as a resource for patient/family and staff
* Provides an authenticated report of all services rendered and maintains accurate and complete records in an acceptable manner
* Completes necessary documentation and ensures communication to facilitate patient care and appropriate reimbursement
* Participates in meetings/activities as required to support operations of the urgent care
* Participates in activities that enhance professional development, complete and maintain certification in BLS, ACLS, ATLS, PALS, or NALS and maintains any additional specialty training/certifications as required by the area assigned
* This position will report to the Regional Medical Director (RMD) and/or Area Clinical Operations Director (ACOD)

MINIMUM QUALIFICATIONS:

* Board Certified (or Board Eligible) and Licensed to practice medicine in the state of practice
* Advanced degree from an accredited Nurse Practitioner or Physician Assistant program of study
* Licensed Nurse Practitioner/Physician Assistant in accordance with the state of Ohio
* Current DEA registration
* Must be DOT certified
* Experience with procedures including suturing and drainage of abscesses
* Experience with reading diagnostic studies including x-rays and EKGs
* BLS certification (we will provide this if needed)
* Meets eligibility requirements for credentialing set forth by WellStreet Urgent Care
* Experience in Urgent Care, Family Medicine or Emergency Medicine preferred
* Demonstrated ability to communicate effectively with patients, families and other employees
