- Conduct individual and/or group coaching sessions following the health coaching model via in-person outpatient, telehealth, and EHR messaging modalities.
- Facilitate the exploration of personalized mission reflection, personal health inventory creation, and action planning processes with patients, clients, and groups.
- Provide individual and group education and evidence-based coaching interventions related to lifestyle behaviors, health management, and health promotion for purposes of improving physical and mental health.
- Administer health behavior assessments and co-develop plans for health behavioral changes that increase quality of life and reduce risk of chronic disease diagnosis or progression.
- Perform coaching sessions with compassion, empathy, and cultural competence to meet patients where they are, help them develop relevant health action planning steps, and track progress on goals.
