JOB SUMMARY

  • Responsible to integrate, coordinate, and monitor the Active Treatment Programs for your assigned case load of individuals with developmental and intellectual disabilities in accordance with current federal, state, and local standards, guidelines and regulations and facility policies.
QUALIFICATIONS
  • An individual who holds at least a bachelor's degree in Human Services field, i.e., Psychology, Sociology, Speech Communication, Gerontology, Education Counseling, Human Development, Anthropology, Rehabilitation Counseling, etc.
  • At least one (1) year of experience working directly with persons with intellectual disabilities or other developmental disabilities.
  • Must possess current CPR certification or become certified within ninety (90) days of employment.
  • Must be able to read, write and speak the English language in an understandable manner.
  • Must possess the ability to make independent decisions when circumstances warrant such actions.
  • Must possess the ability to deal tactfully with residents, family members, visitors, personnel, government agencies/personnel, and the general public.
  • Must possess the ability and willingness to work harmoniously with other personnel.
  • Must have patience, tact, a cheerful disposition, and enthusiasm.
Review the department's policy and procedure manual, Job Descriptions, etc., at least annually for revisions and make recommendations to the Administrator and/or designee to assure client needs are met.
  • Assure that all personnel follow established policies and procedures, using the disciplinary process as appropriate, and informing the Administrator and/or designee.
  • Assist in developing a written plan of care for each resident that identifies the Active Treatment needs of the resident.
  • Attends and actively participates in IDT conferences.
  • Report all concerns, complaints made by the resident, guardian, or their family to the Administrator and/or designee.
  • Meet the Director of Nursing, Assistant Director of Nursing, nursing personnel and review the 24-hour report on a regularly scheduled basis to assist in identifying and correcting problem areas, and/or the improvement of services.
  • Review communication and appointment logs on a daily basis.
  • Make daily rounds to assure that personnel are performing required duties, and to assure that appropriate policies and procedures are being followed.
  • Act as a liaison between facility, resident's families/guardians, and school personnel.
  • Maintain an atmosphere of warmth, personal interest, and positive emphasis, as well as a calm environment throughout the department.
  • Plan, conduct and participate in In-service Education Programs. Demonstrate (hands on) regarding the resident needs as identified by the IDT, including the use of appropriate materials and equipment.
  • Assist in implementing and maintaining an effective orientation program that orients the new employee to the facility, his/her department and its policies and procedures for active treatment.
  • Attend and participate in continuing education programs.
  • Recommend to the Administrator the equipment and supply needs of the department.
  • Assist with planning and monitoring the twenty-four (24) hour schedule for each assigned client.
  • Ensures that appropriate adaptive equipment is present, in good repair, is appropriately used.
  • Provides positive reinforcement to both staff and clients.
  • Informs appropriate staff members of deviations from the daily schedule.
  • Ensures that each resident has a current IPP and active treatment goals monthly.
  • Holds and actively participates in all IDT meetings.
  • Conducts IPP meetings within 30 days of admission, annually, upon significant change, or upon return from hospital or home visit that lasts more than 30 days.
  • Ensures that all assessments are completed in a timely fashion prior to the IPP annually and as needed.
  • Writes monthly notes regarding progress on resident goals as well as family and school contacts and social service needs.
  • Update goals based on review of data as appropriate and in-service staff upon revisions.
  • Notifies appropriate professional staff when goals are met, regression noted, or no progress noted.
  • Ensures that special staffings are held within 3 days following hospital discharges, changes in medical status, or any update occurs which would affect programming.
  • Assure that all clients are well groomed, clean, dressed appropriately in their own clothing, nails clipped, shaved appropriately, hair groomed, etc.
  • Participates in the development, maintenance, and implementation of the facility's Quality Assurance meetings, etc., as directed.
  • Attends community outings as assigned by the Administrator and/or designee.
  • Assist the Activity Department in calendar planning and special events.
  • Assist the Activity Department in recruiting volunteers and serving as a facility representative when community members come into the facility.
  • Performs other related duties as assigned.

Bringing together a range of supportive services under one roof, the Senior Wellness Center operates 365 days a year and offers two floors of integrated care, including health and social services, housing navigation, enrichment activities, lifelong learning, a Cyber Café and linkage to over 25 onsite direct service partners, including a senior dental center. Last fiscal year, we provided over 1.1 million meals and coordinated services to over 10,700 older adults.

Job Purpose and Overview

The Social Services Case Manager works independently and collaboratively with a multi-disciplinary team, to ensure excellent service provision to older adults 60 and over. The Social Services Case Manager provides older adults with individualized social services, physical and mental health services, referrals to community partners, and general support for healthful daily living.

This position will be stationed at Serving Seniors Gary and Mary West Wellness Center from 8am-4:30pm Monday through Friday to provide case management to older adults in need of assistance with social services. This position will be expected to work in person.

Job Duties and Responsibilities

  • Conducts outreach within the Wellness Center to inform and educate older adults on relevant subjects related to social services case management.
  • Connects clients to in-house and community resources.
  • Provides information on community health and welfare resources and acts as a liaison between community service providers and clients, including referrals to Serving Seniors Health Educators and on-site wellness partners.
  • Updates and manages community resources listings.
  • Adheres to established policies and procedures, provides excellent customer service, and demonstrates ability to work in an interdisciplinary setting.
  • Provides advocacy and outreach to the community and fellow stakeholders.
  • Manages a caseload dependent upon both walk-in and appointment hours.

  • Develops and implements a plan of care for clients' unmet needs including supportive referrals to other community-based agencies.
  • Conducts Universal Senior Assessment as applicable.

Required Qualifications
  • Bachelor's degree preferred or equivalent combination of education and experience sufficient to successfully perform the key accountabilities of the job; may require a degree in a specialized field, e.g., social work.
  • Bilingual proficiency is strongly preferred, specifically in Spanish, Mandarin, and/or Cantonese.

Essential Duties:

• Assist the Social Service Director/Consultant in the planning, developing, organizing, implementing, evaluating, and directing of the social service programs of this facility. • Meet with administration, medical and nursing staff, and other related departments in planning social services, as directed • Assist in the development, administering, and coordinating of department policies and procedures. • Review department policies and procedures, at least annually, and participate in making recommended changes. • Assist in developing and implementing policies and procedures for identifying the medically related social and emotional needs of the resident. • Participate in community planning related to the interests of the facility and the services and needs of the resident and family. • Assist residents in achieving the highest practicable level of self-care, independence and well-being. • Assist with providing medically related social services so that the highest practicable physical, mental and psychosocial well-being of each resident is attained or maintained. • Perform administrative requirements, such as completing necessary forms, reports, etc., and submitting such to the Director. • Provide information to residents/families as to Medicare/Medicaid, and other financial assistance programs available to the resident. • Provide consultation to members of our staff, community agencies, etc., in efforts to solve the needs and problems of the resident through the development of social service programs. • Record and maintain regular Social Service progress notes indicating response to the treatment plan and/or adjustment to institutional life. • Coordinate social service activities with other departments as necessary. • Work with emotional problems including assisting resident/family with anxieties and stress caused by illness and admission to the facility, difficulties in coping with residual physical disabilities, fears related to helplessness and death, and the need for institutional and specialized care. • Evaluate social and family information and assist in determining plan for social treatment. • Meet with department personnel, on a regularly scheduled basis, to assist in identifying and correcting problem areas, and/or the improvement of services • Assist in obtaining resources from community social, health and welfare agencies to meet the needs of the resident. • Assist in making outpatient appointments as ordered and schedule on-site ancillary patient services to include optometry, podiatry, dentistry and psychiatric services. • Assists in discharge planning with appropriate agencies, entities or individuals to include agency services, equipment and agency referrals. • Develop and maintain a good working rapport with other departments within the facility, and outside community health, welfare and social agencies, to assure that social service programs can be properly maintained to meet the needs of the residents. • Assist in the development of and participate in regularly scheduled orientation and in-service training programs in relation to the social, emotional and medical needs of the residents. • Assist in the development of the department's budget. • Report suspected or known incidence of fraud relative to false billings, cost reports, kickbacks, etc. • Maintain the confidentiality of all resident care information including protected health information. Report known or suspected incidents of unauthorized disclosure of such information.

Job Summary:
Develops and monitors the social services programs for the CSA. Establishes and maintains, standards, quality levels, cost effective, and customer focused services. Establishes interfaces with community services.
Essential Responsibilities:
+ Plans, develops, implements and maintains social services policies and procedures, delivery of social work programs, and discharge planning services.
+ Oversees social services case management activities.
+ Manages staff activities, ensures their competencies, and develops/presents inservices.

+ Develops and manages departmental budget.
+ Identifies and implements opportunities for cost reduction and improved services.
+ Identifies community resources for our members.
+ Collaborates with community organizations in developing new programs or enhancing current community services which are available to Kaiser members.
+ Develops, implements, and monitors the quality assurance of the social services programs and staff.
+ Identifies issues which interfere with the delivery of services and develops techniques to resolve them.
+ Collaborates with medical center departments, community agencies and health plan members on matters related to discharge planning and ensures that services are provided in an integrated fashion.
+ Ensures compliance with federal, state, and local requirements.

Basic Qualifications:
Experience
+ Minimum two (2) years of leadership or supervisory experience in medical social work.
Education
+ Masters degree in Social Work from an institution accredited by the Council of Social Work Education.
License, Certification, Registration
+ Licensed Clinical Social Worker (California) required at hire
+ Driver's License (California)
+ Basic Life Support
+ National Provider Identifier required at hire
Additional Requirements:
+ Sound knowledge of fiscal management theories and principles.
+ Knowledge of different levels of health care; understanding of human development and psychodynamic theories.
+ Must be able to work in a Labor/Management Partnership environment.
Preferred Qualifications:
+ Minimum five (5) years of supervisory experience in medical social work preferred.
