Responsibilities:

  • Relevant professional registration or licensure (e.g., LSW, LCSW, LICSW).
  • In-depth knowledge of social care legislation, policies, and procedures.
  • Proven experience in case management, crisis intervention, and person-centered care planning.
  • Strong leadership, team-building, and motivational skills.
