The Psychiatric Rehabilitation Service Coordinator (PRSC) is responsible for coordinating the overall delivery of care to each individual within his/her caseload. The PRSC is responsible for helping individuals address their mental health challenges enabling each to function at his/her highest practical level.
Essential Job Functions:
- Provides the resident with a stable therapeutic relationship.
- Welcomes the new resident and orients them with the facility.
- Reviews and assists the resident in understanding the treatment plan arid program schedule.
- Provides and/or coordinates the delivery of the psychiatric rehabilitation services program.
- Accurately and timely completes assigned of the resident's comprehensive assessment.
- Re-evaluates the resident on a quarterly basis and documents this re-evaluation.
- Oversees the interdisciplinary team in the development of the individual treatment plan.
- Documents the resident's progress on a regular basis, including incidental and quarterly notes.
- Contacts community agencies for the purpose of resident referral.
- Assists the resident to participate in individual and group programs and to utilize these programs to full advantage.
- Additional duties as assigned
- Bachelors or Master's degree in a human service progression (i.e., Social Work, Psychology, Sociology, Counseling Psychology)
- One (1) year of supervised experience in mental health or human services
- Demonstrated skills and ability in working with and understanding the needs of the residents, families, staff members and personnel from community agencies

- Responsible to integrate, coordinate, and monitor the Active Treatment Programs for your assigned case load of individuals with developmental and intellectual disabilities in accordance with current federal, state, and local standards, guidelines and regulations and facility policies.

- Update goals based on review of data as appropriate and in-service staff upon revisions.
- Notifies appropriate professional staff when goals are met, regression noted, or no progress noted.
- Ensures that special staffings are held within 3 days following hospital discharges, changes in medical status, or any update occurs which would affect programming.
- Assure that all clients are well groomed, clean, dressed appropriately in their own clothing, nails clipped, shaved appropriately, hair groomed, etc.
- Participates in the development, maintenance, and implementation of the facility's Quality Assurance meetings, etc., as directed.
- Attends community outings as assigned by the Administrator and/or designee.
- Assist the Activity Department in calendar planning and special events.
- Assist the Activity Department in recruiting volunteers and serving as a facility representative when community members come into the facility.

Position Summary
The Senior Services Social Worker Position offers a unique opportunity to help build the first PACE (Program of All-Inclusive Care for the Elderly) program in the State of Illinois within the framework of a supportive, innovative, and expert leadership team.
The PACE Social Worker is responsible for planning, organizing, and implementing social work services to LCHC PACE participants and families. This position reports to the LCHC PACE Clinical Services Director. Compensation will depend on a number of factors, including a candidates qualifications, skills, competencies and years of experience.
Principal Duties and Responsibilities *
- Responsible for initial, 6 month, change of condition, and annual assessments
- Presents assessments at care plan conferences with the goal of providing a foundation for development of the Plan of Care and team discussion of participant issues
- Acts as liaison with participant and caregivers regarding orientation to and ongoing relations with interdisciplinary team, day center, and other LCHC PACE, including volunteers
- Provides ongoing support and education to participants and family regarding a variety of issues, including but not limited to the aging process, dementia, grief and loss, end of life, disease processes, difficult family dynamics, and changing roles
- Works to maintain participant housing through intervention with participant, caregivers, and housing. Will proactively work to partner with participant and/or caregivers to maintain appropriate housing and assist participant to function at most independent community level possible
- Coordinates admissions/discharges to contracted facilities for temporary respite and permanent placement
- Initiates nursing home co-pay status upon charge from team and supervisor when skilled facility placements become permanent
- Performs home visits as needed to assess living environment and support system
- Provides referrals and assessments with contracted personal care boarding homes and assisted living residences
- Facilitate participant move between residences or assisted care facilities if no other family or support systems are present
- Coordinates as needed hospital discharges in communication with attending medical team members
- Initiates referrals to external resources with community agencies such as Adult Protective Services, Housing Authority, or public utility companies
- Assists participants and caregivers to complete POA, Proxy, and DNR directives as needed
- Assists participants and caregivers to set up and maintain personal needs accounts
- Maintains current documentation in participants medical records including initial assessments (narrative, problem lists, assessment forms); reassessments (updated problem lists and chart note); change of status; temporary or permanent placements; hospital discharges; participant staffing's; home visits; and other significant events
- Assists participants with SSI and SSDI application process as needed.
- Refers potential voluntary disenrollment's due to dissatisfaction to PACE leadership team. Completes internal form for disenrollment's and deaths
- Assists participants and family in coordination with LCHC PACE Outreach Department to keep resources within guidelines for Medicaid eligibility
- Serves as a required member of the Interdisciplinary Team and attends IDT meetings daily.

- Licensure is not required for this position: however, candidates holding an LSW or LCSW will be eligible for higher compensation
