Minimum Qualifications:

Position Summary

KIND seeks a Senior Social Services Coordinator to work with KIND's child clients and their families to identify needs and gaps in services, assist families in accessing services related to education, health, mental health, housing, mediation, and childcare and provide leadership in establishing and sustaining relationships with local and state social services partners.

This position is contingent upon continued funding.

Essential Functions
    • Develops and maintains a knowledge base of social services available to clients in assigned work areas.
    • Leads outreach efforts to establish strategic working relationships and partnerships with local service providers in each service area to enhance the referral process.
    • Completes case management activities for referred clients inclusive of assessments, comprehensive service plan, referrals to services, monitoring and follow-up, and client/caregiver engagement ensuring appropriate tracking of appropriate documentation. regarding attempts to access services and service delivery.
    • Contributes to completion of data/metrics reports for internal and external program requirements inclusive of client data, outreaches, and other program outcomes.
    • Leads and guides in client therapeutic support and crisis management as assigned or necessary.
    • Supervises MSW placement interns as required by program agreement.
    • Lead designs, planning and monitoring of in-house therapeutic events, resource fairs, and preventive programming.
    • Supports hiring, onboarding, training, and mentoring of new staff inclusive of foundational training areas and in concert with hiring manager/supervisor.
    • Supports with termed special projects related to the delivery of services to children as assigned.
    • Conducts assessments of referred clients, including the larger context in which the child is living, identifying needs, barriers, and safety concerns.
    • Supervises general interns and volunteers.
    • Other duties as assigned.

Qualifications and Requirements
    • Graduate degree in Social Work or related field, preferably in counseling, or related specialty or 6 years of work experience. MSW (Master of Social Degree) preferred.
    • Must be fluent in English and Spanish (required for direct communication with the children we serve and their caregivers, the majority of whom are Spanish-speaking).
    • Minimum of 2 years of experience working with children, preferably unaccompanied minors, immigrant and refugee children.
    • Minimum of 3 years of experience in social services or non-profit organizations; community organizing, or grassroots and partnership development experience preferred.
    • Direct experience or strong familiarity with case management frameworks.
    • Ability to work collaboratively, and preferably, prior experience working with attorneys, law firms, or other legal service providers.
    • Experience working with teams of social service providers and advocating on behalf of clients.
    • Ability to verbalize and implement therapeutic-focused strategies for children/families.
    • Ability to effectively conduct trainings in person and virtually, including attorney training and presentations for child clients.
    • Familiarity or ability to work with adult caretakers, or training in trauma-informed parenting.
    • Ability to manage, create and oversee various events that aim towards bringing resources and services to child clients.
    • Ability to network with local coalitions to establish strong relationships to allow for cross-referrals.
    • Ability to travel as assigned.
    • Ability to work within an office environment, have regular interaction via telephone, teleconference, IM, and email with KIND's team, and have in-person meetings with clients.
    • Working knowledge of Microsoft Office Suite (such as Teams, Excel, etc.).

Minimum Qualifications:

Possession of a license to practice law in New York State and four years of experience in the practice of law, at least one year of which must have been engaged in public sector law at the state or local level. In addition, work experience must have included or been supplemented by one year of law practice, as an attorney, which substantially involved domestic relations, family law, and/or the protection of vulnerable adults or children. Candidate must provide a current certificate of good standing. A license to practice law in the State of New York must be maintained throughout the course of employment in this title.

  • Enter accurate and complete case notes in a timely fashion and contribute to periodic reports to funding agency.

  • Develop service and goal plans with client to ensure housing stability.

  • Establish individual and family goals to increase: money management skills, higher educational level, household income and education on tenant rights and responsibilities.

  • Escort family members to entitlement appointments/certifications as needed and appropriate.

  • Deliver services with sensitivity to the cultural and socioeconomic characteristics of all members of the family.

  • Provide crisis intervention, de-escalation, and referrals to counseling services when necessary.

  • Coordinate with community partners to organize educational and recreational workshops.

  • Communicate with building management staff on tenant issues, including discussion of rent ledgers, rental subsidies, annual recertification, etc.

  • Provide access to mediation services, as needed, in the event of rental arrears or violation of house rules.

  • Educate clients on the available services in the community; make referrals when necessary and appropriate.

  • Additional responsibilities may be added as position evolves.

  • Perform other related duties as directed by management.

Title: Senior Director of Social Services, Family Foster Care Programs

Reports to: Assistant Executive Director

Major Duties & Responsibilities:
  1. Responsible for the administration and oversight of the social services components of the Family Foster Care Program, which include regular Family Foster Care (FFC), Special Medical and Developmental Disabilities (SMDD), and Treatment Family Foster Care (TFFC)
  2. Provides direct supervision to the TFFC Director and the FFC Deputy Directors
  3. Ensures consistent and ongoing compliance with all mandated requirements and outcome measures of the Administration for Children's Services (ACS) related to the Scorecard, and ACS Foster Care Standards, including: timely FASP submission; ECANS, PYA Forms, MHCU Forms, ETP requests, timely submission of OCFS and ACS CAPs; timely outcome achievements: reunifications, re-entries, and adoptions; special/exceptional care authorizations; compliance with PAMS performance and documentation requirements; casework contacts; Family Team Conference (FTC) attendance rates; HIV/AIDS PAMS requirements; and casework attendance at Family Court proceedings;
  4. Develops social service programming directed toward the timely achievement of permanency for children in accordance with principles of good social work practice and utilization of the Solution-Based Casework (SBC) evidence-informed model of case management.
  5. Ensures the early engagement of families and the provision of concurrent planning and aftercare services; and monitors issues associated with service delivery, including: length of stay, replacement rates, re-entry to care following discharge rates, movements; sibling separation, timely access to needed and appropriate quality services, and maintenance of the child within community of origin;
  6. Establishes procedures for the timely identification of children requiring transfer from one level of care to another, including oversight of a high risk case review protocol, and a smooth transitioning process to involve the child's discharge resource;
  7. Actively participates in program staff's SBC Certification. This includes but is not limited to facilitating/co-facilitating SBC Supervisor Seminars, "Coaching" staff, actively participating in all SBC Implementation Meetings.
  8. Oversees the program's establishment and development of linkages with relevant community services, and participates in community advocacy networks;
  9. Ensures programmatic follow-up on children absent from care from FFC social service programs;
  10. Responds to emergencies during the evening and weekends associated with the care of children in the FFC, TFFC and SMDD Programs. FFC Director of Social Services must be available 24 hours/7 days per week (unless out of the office on approved time off) to guide staff through emergencies and communicate in "real time" with the agency executive team members. When instructed by the agency's executive team the Senior Director of Social Services is expected to communicate accurately and effectively with ACS OSR and OCFS when critical incidents occur.
  11. Promotes and monitors staff training and development in accordance with good practice standards and mandatory guidelines set by governmental oversight bodies
  12. Monitor program staff's compliance with the timely submission of timesheets and employee evaluation performance.
  13. Provides regular reports, including a comprehensive annual report, on the performance of the agency's FFC, TFFC and SMDD Programs in the achievement of their responsibilities, mandated requirements, and relationship to communities;
  14. Develops and maintains monthly management plans and reports necessary for administrative oversight of child welfare services offered and coordinated through the program
  15. Attends regularly scheduled meetings throughout all the satellite offices and other sites throughout the community, including ACS and OCFS locations
Qualifications:
  1. Master's degree in Social Work; LMSW required.
  2. Minimum of 10 consecutive years of prior administrative, clinical, and managerial experience overseeing a significant and diverse child welfare social service operation;
  3. Proven command of ACS Scorecard contract performance requirements and NYS OCFS foster care standards and requirements;
  4. Current and thorough working knowledge of CONNECTIONS, LTS, PYA, and e-CANS;
  5. Excellent computer skills, e.g.; capacity to develop tracking systems and excel templates.
  6. Demonstrated knowledge of best practices in the field of child welfare;
  7. Ability to travel to New York City communities;
  8. Ability to work a flexible schedule, including evenings and weekends;
  9. Ability to be "on-call" 24 hours/7 days per week
  10. Ability to work effectively with the agency's executive management team;
  11. Proven management abilities, including working in a high volume work environment, meeting deadlines, ensuring compliance with agency policy and procedures, and overseeing complete and timely maintenance of agency case records, in accordance with contractual requirements;
  12. Demonstrated effective inter-departmental project management and coordination;
  13. Excellent writing and project management skills.

We have an exciting opportunity for a full-time, Social Worker to join Ramapo Ridge Behavioral Health. The Social Worker provides services to psychiatric patients and their families including individual, group and family supportive and educational counseling, care coordination and post-acute care planning. The individual is competent to care for patients ages 18 and over including older adults and those with dementia. Demonstrates an understanding of, and embraces, the mission statement of the Christian Health Care Center.



The opportunity for professional growth working at The Haven at Ramapo Ridge, New Jersey’s first and only inpatient trauma- informed care program and which has received the Clinical Excellence in Behavioral Health award from the New Jersey Hospital Association (NJHA).



Clinical supervision provided on site to those seeking hours toward LCSW licensure.



Completes psychosocial assessments including obtaining information for diagnosis and treatment as required by regulatory and accrediting agency requirements.

Serves as liaison to patients and families and available community partners and resources.

Maintains accurate, timely documentation as a basis for review by regulatory bodies, accrediting agencies and the interdisciplinary treatment team.

Provides support, education and assistance to families and their support system regarding community resources, course of treatment and hospitalization, and discharge planning.

Coordinates and facilitates post-acute care plan for assigned patients.

Assists in developing favorable attitudes in the community toward recognition and treatment of mental illness and the aging process through participation in community activities concerned with mental health and/or aging.

Minimum of one year social work experience preferred.

Master's degree in Social Work required.

Current valid New Jersey Licensed Social Work (LSW) required.

Job Overview:

The Licensed Social Worker provides direct clinical social work services to patients and their families in an interdisciplinary team approach. The Licensed Social Worker collaborates with members of the healthcare team to facilitate the progression of the patient's treatment and discharge plan. This role incorporates assessments, care coordination, and education of the patient/caregiver/healthcare team to reach appropriate goals from hospitalization and post-hospitalization care.

Qualifications:

Required:

  • Master's degree in Social Work (M.S.W.) from an accredited School of Social Work

Preferred:

  • Three or more years of Social Work experience in a healthcare-related setting

  • Case Management experience

Certifications and Licenses Required:

Essential Functions:

  • Completes initial SW assessment on cases referred in electronic medical records
  • Interviews patients/families to obtain psychosocial information relevant to medical problems and for discharge planning
  • Interprets psychosocial situations in relationship to medical condition/hospitalization
  • Counsels patients/families in regard to adjustment to illness and developing short and long-term goals
  • Coordinates individual/family conferences in relationship to developing discharge plans and supportive counseling during hospitalization
  • Communicates and coordinates with the multidisciplinary team for the purpose of developing discharge plans during team meetings
  • Communicates with CM on cases
  • Communicates and educates the team on the bio psychosocial needs of patients to accomplish the goals and objectives of the hospital for effective patient care
  • Provides ongoing written documentation in patient's electronic record of social work activity
  • Demonstrates knowledge of the growth/development needs of patients/families in the geriatric and adult age levels as reflected in daily activities/responses to patient needs
  • Coordinates/schedules transportation
  • Provides linkage to community agencies for patients/families for specific service
  • Provides information/referral for outpatients in ER and other disciplines
  • Provides education, information and referrals to various community agencies for services
  • Completes Level 1 PASRR screen, EARC-PAS
  • Provides information/education on advance directive and POLST
  • Daily Rounding on specific medical units for discharge planning
  • Submits data for CCDA
  • Provides education to patient/family on insurance coverage as it relates to hospitalization and discharge planning
  • Demonstrates knowledge of cultural competency in working with patient/families and team members
  • Advocates for patients when working with insurance companies and other agencies
  • Monitors for abuse/neglect for all patient populations
  • Provides supervision to staff in complex cases requiring a greater degree of professional knowledge
  • Provides supervision to beginning level social workers and students in the social work department

The working Supervisor of Case Management is responsible for providing supervision to the Case Managers, Coordinators, and Discharge Planners. Provides oversight for the quality and resource management of all patients that are admitted to the facility from the point of their admission and across the continuum of the health care management. Works on behalf of the advocate, promoting cost containment and demonstrates leadership to integrate the health care providers to achieve a perceived seamless delivery of care. The methodology is designed to facilitate and insure the achievement of quality, clinical and cost effective outcomes and to perform a holistic and comprehensive admission and concurrent review of the medical record for the medical necessity, intensity of service and severity of illness.


EDUCATION, EXPERIENCE, TRAINING

Required qualifications:

1. Grandfathered prior to April 1, 2015 for LCSW. Valid RN or LCSW state licensure required.

2. CCM obtained within 6 months with a minimum of 2 years’ experience.

3. Experience and knowledge in basic to intermediate computer skills.

Preferred qualifications:

1. Minimum 5 years of acute care experience preferred.

2. At least 2 years’ experience in case management, discharge planning or management, preferred.

3. Current BCLS (AHA) certificate preferred.

4. Knowledge of Milliman Criteria and InterQual Criteria preferred.
