  1. Inform parents/guardians of their child’s rights and suggest appropriate special education services and programs to meet their needs
  2. Facilitate understanding of the child’s educational needs by explaining reports in plain/ native language to families and educators
  3. Help families prepare for IEP meetings, including considering goals, reviewing evidence and materials, and help develop strategies
  4. Review special education and section 504 documents, including student files, evaluation assessment reports, IEPs and Section 504 Plans
  5. Accompany parents to IEP, Section 504, and other school meetings and provide advice and assistance as needed throughout the process
  6. Work with parents to get their concerns heard by the district and help resolve disputes that may arise
  7. Educates oneself on relevant resources, benefits, support services, educational plans, programs, and trainings in the community and utilizes this information to educate parents of children with special needs
  8. Promote community and institutional advancement by representing the rights, desires and interests of individuals with developmental disabilities with the community, educational institutions, and/or other relevant agencies and service providers.
  9. Assists Social Services Administrator with network within the special needs community and community outreach to market program, establish and maintain relationships with community partners, and recruit clients
  10. Maintains accurate case notes on services delivered, develops service plans with parents, and tracks and submits monthly billing
  11. Assures that the program is in compliance with contractual requirements, utilization of program units, funding, Company policies and procedures and all applicable state and federal laws.
  12. Collaborates with Social Services team members from other programs and departments to cross refer clients and ensure clients are benefitting from all relevant BCC programs
  13. Assist clients to access appropriate benefits and basic needs; appropriate medical, mental health and substance use treatment providers
  14. Provide crisis intervention for immediate housing needs, medical, mental health and substance use issues
  15. Assists with the development, implementation, and administration of the Advocacy Program.
  16. Assist with any community inquiries on all agency services, client referrals to community resources, and represent the program at occasional community events and/or presentations
  17. Maintains appropriate records and reporting for the Advocacy Program.
  18. Seeks guidance from the Social Services Administrator when appropriate.
  19. Attend all Social Services Department in-service training and community workshops/seminars when needed.
  20. Flexibility of working hours will be required to meet the child and family’s needs which include evening (2-3 per week) and weekend hours as needed
  21. Other duties may be assigned.

Education:

Other:

Work experience with individuals with developmental, emotional and/or physical impairments preferred. First Aid and CPR certification required. Working knowledge of training techniques, behavior modification and task analysis. Interpersonal skills sufficient to communicate with participants, public and staff. Writing skills sufficient to prepare required reports. Must be flexible to perform job duties in community-based sites.

  1. Implement behavioral and other training programs.

The purpose of this position is to assist the District Nurse in meeting the comprehensive health
needs of students in the educational environment. This position would include working 1:1 with a medically fragile student in the classroom environment.

Essential Duties and Responsibilities:
The following duties are normal for this position. These are not to be construed as exclusive or all inclusive. Other duties may be required and assigned.
  1. Under the supervision of a professional educator, district nurse, and/or building administrator, provide for educational, personal, and physical needs of students with complex health needs.
  2. Responsible for the monitoring and intervention of complex health and medical needs including respiratory status, management of a tracheostomy/ventilator, seizure management, G-tube feedings, medication delivery, positioning in wheelchair, hygiene, students communication etc.
  3. Work collaboratively with school personnel to provide for the health and educational needs of the assigned student in order to enable a supportive learning environment
  4. Additional specific duties: Ride bus to and from school with student.
  5. Maintains current Wisconsin Certification in "Care of the Child on a Ventilator", CPR, First Aid, blood borne pathogens and other trainings as required.
  6. Follows through with directives and instructions consistently from District Nurse, independently makes decisions and solves problems to meet the needs of students according to licensure and knowledge base.
  7. Maintains a safe learning atmosphere for students to enable them to meet or surpass their capabilities.
  8. Supports and facilitates all IEP/504 goals/plans set by IEP/504 Teams and collects related behavioral and academic data.
  9. Facilitates the inclusion of students whenever and wherever possible, be that the classroom or the community; also promotes acceptance of differences with students, staff and community.
  10. Provides student supervision across all educational environments.
  11. Must be able to take and follow through with directives and instructions consistently from special education contact teacher, as well as be able to independently make decisions and solve problems to meet the needs of students.
  12. Follows all relevant district policies and procedures including District Technology Use Policy throughout workday.
Minimum Training and Experience Required to Perform Essential Job Functions
  1. Licensed Practical Nurse, graduate of accredited school for Licensed Practical Nursing or
higher with one to two years nursing experience.
  1. Current certification for Wisconsin "Care of the Child on a Ventilator", or be enthusiastic about obtaining certification.
  2. CPR Certification or ability to obtain within 30 days of start date.
  3. Must possess a valid driver's license.
  4. DPI Special Education Program Aid License certification
Minimum Physical and Mental Abilities Required to Perform Essential Job Functions
Physical Requirements
  • Ability to perform light to moderate physical work, frequently lifting up to 10 to 15 pounds without assistance and when needed, lifting up to 50 to 80 pounds with assistance.
  • Ability to reach in all directions with full range of motion, bend/stoop and use hand/eye coordination to assist students in any required capacity when needed.
  • Must have current physical exam on file with the district certifying employee is able to meet all job qualifications.
Knowledge, Skills, and Abilities
  • Ability to work independently within the guidelines of policy and procedure using discretion and good judgment.
  • Ability to react quickly, calmly, and decisively in a crisis.
  • Demonstrated ability to prioritize tasks requiring timelines and accuracy.
  • Demonstrated ability to be a collaborative and confidential team member.
  • Ability to use various health and first aid equipment and supplies.
Mathematical Ability
Ability to add, subtract, multiply, divide and calculate decimals and percentages

Language Ability and Interpersonal Communication
  • Ability to comprehend, interpret and/or prepare a variety of documents, including immunization and health history records, diagnostic and lab reports, Health Services handbook, First Aid manuals, communicable disease charts, etc. according to licensure and knowledge base.
  • Ability to record and deliver information, explain procedures and follow instructions
  • Ability to use and interpret medical terminology
  • Ability to communicate effectively with teachers, students, parents, administrators, nurses, Social workers, guidance counselors, speech and language psychologists, delivery persons, Police officers, School Board members, the general public, and others verbally and in writing
Environmental Adaptability
  • Ability to work effectively in an office environment
  • Exposure to bodily fluids and communicable diseases
