Rivers Health is seeking a Patient Care Supervisor. This is a position for supervisory coverage of the Nursing Department, other patient care areas, and the hospital in general. Responsibilities require knowledge of hospital and nursing policies and procedures, allowing independent judgment in their interpretation. The Nursing Supervisor duties include provision of managerial and clinical links between the Nurse Director and in their absence, and the administrator on-call. Additionally, this position will include knowledge of Utilization Review rules and regulations as they assist physicians and staff in assigning admission status.

Essential Job Functions:

The following is a summary of the major essential functions of this job. The incumbent may perform other duties, both major and minor, that are not mentioned below; and specific functions may change from time to time.

• Promote the mission, vision, and values of the hospital.
• Assess patient care requirements on each unit, by making rounds in the Emergency Department (ED), and all other areas of the hospital (according to departmental need/activity).
• Implement staffing plan for shift based on patient acuity, patient census, and available resources.
• Serve as a resource to staff for solving clinical problems.
• Assist staff to efficiently utilize staffing resources to meet patient care requirements and other professional needs.
• Provide immediate follow-up, documentation, and communication of incidents to appropriate personnel.
• Respond to emergency situations (RRT, Code Blue) and assist as needed.
• Act as a resource in identifying and/or solving administrative/operational problems.
• Anticipate actual/potential problems that may occur on their shift and set shift priorities.
• Work collaboratively with nursing and ancillary staff to solve problems in a fair, consistent manner.
• Contact representatives of departments not presently in the hospital for assistance with problem solving, timely and effectively.
• Act as a resource, interpret and ensure adherence to Hospital and nursing philosophy, policies, and procedures.
• Report to the administrator-on-call significant incidents and problems such as: unusual patient or family problems or concerns, physician concerns, borrowing or lending of major equipment, major damage or utility outages, security problems, public relations or media events, fire, disaster, bomb threats, matters of legal importance, calls received from the Health Department or any other regulatory agency, safety issues.
• Obtain materials/supplies from Materials Processing and/or the Storeroom for patient care when these departments are not available.
• Provide support, guidance, and feedback to staff:
• Give ongoing timely feedback, verbal, and written anecdotes, to staff on Written anecdotes are passed on to the nurse director to be used as part of performance evaluation.
• Contribute to staff performance improvement.
• Facilitate staff attendance at staff development sessions offered during their shift.
• Identify and communicate staff needs for continuing education to the Nurse Director.
• Assist staff to interpret change in a positive manner.
• Assist staff to provide a safe work environment for patient care.
• Assist in the preparation and implementation of the annual goals for the department.
• Keep professionally current by attending continuing education programs, seminars, workshops, and/or independent research reading.

• Manages and documents phone calls from physicians, clinicians, patients, referral sources, and communicates patient updates/new orders to clinicians. Uses coordination notes to document, as needed and appropriate.
• Reviews assessments and plans of care daily, per assigned workflow, and consults clinicians with recommendations, as appropriate.
• Coordinates all aspects of care with all disciplines, physicians, durable medical equipment providers, caregivers/family members, transferring facilities, and any other applicable healthcare providers.
• Follows-up on lab and other clinical diagnostic test, physician contact, and significant changes in the patient condition to ensure adequate physician notification, follow-up, and needed plan of care modifications and communicates such to clinicians.
• Schedules, prepares for, facilitates, and documents case conference/SOC reports and facilitates effective exchange of information across disciplines especially with adverse findings, changes in patient condition, daily and urgent updates, as necessary.
• Assists clinicians in coordinating the transfer and discharge of patients from agency services as indicated by the physician.
• Receives report from field clinicians prior to scheduled days off on patient status and ongoing needs.
• Processes new orders and updates the visit frequency, as appropriate, when the oncall RN takes supplemental verbal orders which alter frequency going forward.
• Writes and processes orders when taking verbal orders directly from the physician and communicates such to field clinicians.
• Assures payer change documentation is completed properly and timely, as required.
• Reviews clinician visit notes weekly to ensure timely, complete, appropriate, and accurate submission of all documentation by field staff. Takes necessary action to correct adverse findings and communicates trending to clinical director.
• Reviews, evaluates, and supervises service delivery to ensure appropriateness of care and utilization of services, equipment, and supplies through activities such as random patient visits, medical record reviews and case conferences.
• Enters infections and incidents/occurrences into the online Risk Management Incident Reporting System, as specified by policy.
• Assists in the orientation of new agency personnel.
• Provides direction and leadership to clinical team members in collaboration with the clinical director.
• Provides direct patient care, as necessary, in accordance to scope of practice and physician orders.
• Participates in QAPI program.
• Assures compliance with and ensures timely follow up on daily clinical and coding edits.
• Directs clinicians in utilizing best practice interventions when finalizing Plan of Care for all patients.
• Participates in on-call rotation.
• Follows-up with On-Call events daily.
• Receives report from weekend and after-hours clinicians admitting new patients.
• Completes LHC required learning courses, additional assignments per Executive Director request, as well as any state specific required training per state regulation/practice act requirements.
• Directs team in adherence to and participates in the Episode Management process.
• All other duties as assigned.
