Responsible and accountable for the independent planning, organization, implementation and evaluation of Ambulatory Specialty Practice patients. * PATIENT CARE - Perform all RN Nursing duties. Assist in specialty clinic; triage patients as necessary in daily walk-in clinic
* PATIENT CARE - Coordinate all activities of the specialty related to patient care
* PATIENT CARE - Serve as liaison between patient/family and various members of the health care team
* PATIENT CARE - Counsel and teach families regarding medical conditions, diagnostic procedures, medication control, and other interventions

Nurse Case Managers

* Serves as the team leader of the interdisciplinary healthcare team promoting the coordination, collaboration, and continuity of care from preadmission to post-discharge from a variety of health care providers (doctors, nurses, nurse practitioners, specialty and contract providers, nurse midwives, physical therapists, dieticians, pharmacists, medical social workers, and mental health practitioners) Incumbent ensures patients receive quality patient centered care that is consistent with the patient-centered medical home principles, and that care is timely, appropriate, cost effective, sensitive and coordinated to achieve desired clinical outcomes.

Nurse Clinical Care Coordinator

* Prioritizes the information collected based on the patient's condition and preferences, the situation, and/or anticipated needs. Develops a plan of care with the care team according to clinical practice guidelines with the patient, family, caregiver(s) and significant other(s). Leads the team in panel management activities to ensure all patients receive appropriate preventative and chronic disease management

Nurse Emergency Room

* Determines the severity of illness or injury and initiates and provides rapid intervention in emergent, life-threatening conditions, even when a physician is not present or immediately available. Nurse Post Anesthesia Care Unit

* Delivers comprehensive nursing care specific to the age of the patient (adult, geriatric, pediatric, neonate) undergoing a wide variety of surgical procedures, ranging from simple to complex in nature, under both controlled and stressful situations-including on-call hours. Administers therapeutic measures as prescribed by the physician to include, but not limited to, medication administration, intravenous fluids, and blood transfusions. Independently manages patients receiving sedation/analgesia/anesthesia to include anatomy, physiology, pharmacological modalities, cardiac arrhythmia recognition, and complications related to adverse effects of procedural sedation and general anesthesia.

Nurse Medical/Surgical

* Utilizes the nursing process to identify individual patient needs and to develop, implement and evaluate plans of patient care for acutely ill patients. Provides professional skilled, competent nursing care specific to age of the patient (adult, geriatric, obstetric, pediatric, neonate) to meet physical, emotional, spiritual and social-cultural needs of the patient, by means of direct care and teaching. Observes and assesses patient's clinical condition: recognizes, identifies and interprets serious situations and immediately implements proper action.

Nurse Community Health

* Formulates and implements overall community health nursing programs and plans to satisfy varying patient load requirements, priorities for service and other healthcare objectives. Tracks community health status and evaluate health trends and risk factors of population groups. Collects data in a systemic organized manner and analyzes the collected assessment of data.

Nurse Outpatient

BASIC REQUIREMENT(S):

Education: A graduate or higher level degree, bachelor's degree, associate degree, or diploma from an accredited professional nursing educational program is required. This education must have been accredited by the Commission on Collegiate Nursing Education, Council on Accreditation of Nurse Anesthesia Educational Programs, Accreditation Commission for Midwifery Education, or an accrediting body recognized by the U.S. Department of Education at the time the degree was obtained.

Degree from Foreign Nursing School: Official certification from the Commission on Graduates of Foreign Nursing Schools is required for individuals who graduated from foreign nursing schools.

Licensure: For all grade levels and positions, applicants must have passed the National Council Licensure Examination.
