Job Description:

As an LPN, your scope of responsibilities will include but is not limited to:

* Providing in-home nursing care to villagers via scheduled appointments, wellness checks, and emergency responses.
* Scheduling and following up with routine lab appointments for villagers.
* Participating in weekly care team meetings to help with care coordination.
* Collaborating with physician and/or RN as needed.
* Assisting in the scheduling and administering yearly flu vaccines to villagers/staff.

Education and Experience:

* Must be licensed as a practical nurse in the State of Pennsylvania.
* Post licensure experience preferred but not required.
* Must demonstrate CPR competencies.
* Evidence of continuing education in line with community, state and federal regulations.
* Must possess quality mindedness, critical thinking, initiative in leadership responsibilities, interpersonal sensitivity and communication skills.

+ Obtains and documents reason for visit, patient vital signs, including temperature, blood pressure, weight, respirations, pulse and BMI.
+ Assists providers with examinations, procedures and other processes related to direct patient care.
+ Collects specimens as directed by provider. Uses appropriate equipment and protocols for collecting, handling, testing, and preparing for transportation to lab including proper identification, storage procedures and techniques.
+ Ensures all patient care areas are adequately stocked with unexpired supplies and medications.

+ Efficiently manages daily patient flow to manage fluctuating patient volumes and demands. Prioritizes daily patient care tasks and activities that are delegated by the care team, including but not limited to in basket management.
+ Under the direction of your supervisor, takes ownership in an office function/process improvement.
+ Maintains strict confidentiality of patient's health information. Completes annual WSH Corporate Compliance, and other mandatory education.
+ Supports the provider during the visit and patient exam by collecting and entering information in the patient record, carrying out orders and completing post visit patient care education and instructions.
+ Maintains an 80% or higher on all yearly audits performed.

This position is responsible for answering the telephone and explaining the operation of PerformCare to our Members when they call; Completing intake information regarding the member; Explaining procedures for accessing services to Members; Coordinating/Interfacing with the Medical Assistance Transportation Program (MATP), as necessary, to assist Members in securing transportation to scheduled appointments; Completing bed searches, as required, for IP and SA bed requests; Addressing Member complaints through the initialization of the PerformCare Grievance and Complaint process; Explaining a Member's rights as requested; Providing additional resources to the member such as a Member Handbook and list of in-plan providers from the member's geographic region and completing data entry of requests and authorizations.

Work Arrangements/Schedule: Remote opportunity; Saturday and Sunday 3:00pm-11:00pm

Education/Experience:

  • A bachelor's degree (BS or BA) from an accredited college or university is required.

  • A minimum of one (1) to three ( 3 )years of Behavioral Health experience preferred.

  • Excellent written and verbal communication skills with the ability to respond sensitively to members' requests preferred.

  • Previous customer service and data entry experience desired, preferably within a call center.

  • At least one year of experience in mental health and/or human services required; or equivalent background and experience that would translate well into this position.

  • Ability to communicate well verbally and in writing with Members and all other internal and external customers.

  • Ability to use PC's in a Windows-based environment.
