**Job Description :**
**GENERAL OVERVIEW:**
This job, under the direct supervision of a pharmacist performs a wide variety of technical and clinical duties for the hospital wide unit-dose drug distribution system and intravenous admixture program. Maintains confidential reports, entering, and retrieving data utilizing the computer system, prepares intravenous medication, fill routine orders for unit dose and prepackaged pharmaceuticals, and provides accurate assistance to the pharmacist in performing daily duties. All orders are checked by a pharmacist prior to dispensing.
**ESSENTIAL RESPONSIBILITIES:**
+ Uses clinical skills to assess medication orders for accuracy and appropriateness.
+ Procures and distributes medication orders per physician or pharmacist request, or prearranged work assignment according to established policies, procedures, and protocols. Under direct supervision of a pharmacist, fills routine patient orders for unit dose carts and prepackaged pharmaceuticals, preparing, compounding and labeling IV admixture, antibiotics, cardiac drips, solutions, irrigations, parenteral nutrition, and chemotherapy.
+ Continuously maintains pharmacy inventory and supplies according to established policies and procedures through assigned inspections, use of automated dispensing units and medication supplies depending on facility. Assists in ordering, receiving, unpacking, and storing pharmaceuticals and supplies in appropriate locations, restocks medications and IV dispensing area, prepackages bulk medications in unit dose packages.
+ Compounds, stocks, repackages and batches from bulk medications. Prepares routine patient orders for IV pharmaceuticals including compounding, preparation, and labeling of any IV admixture including antibiotics, solutions, irrigations, narcotics, parenteral nutrition, and chemotherapy.
+ Maintains competence required for current position and has ability to perform all technician duties to assist in training of new employees and students.
+ Contributes to the effective operation of the department through all activities including computer functions, process cart fill lists, pick lists, refill medications, generation of labels, and answers phones appropriately. Maintains a clean and orderly work environment.
+ Participates in continuous performance improvement and regulatory compliance efforts. Takes responsibility for quality improvement efforts and then monitors processes and results.
+ Maintains an attitude that promotes and fosters an environment of safety.
+ Performs other duties as assigned or required.
**QUALIFICATIONS:**
Minimum
+ Enrolled in a School of Pharmacy orForeign pharmacy graduate and has applied for a Pharmacy Intern license within the state of PA within 120 days of hire

GENERAL OVERVIEW:
The hospital-based Wound Ostomy Nurse serves as a clinical resource and consultant for the acute and rehabilitative care of patients with selected disorders of the gastrointestinal, genitourinary, and integumentary systems such as acute and chronic wounds (e.g., complex draining wounds, fistulae, pressure ulcers, vascular ulcers), ostomies, percutaneous tubes, and continence disorders. Professional responsibilities are carried out through direct patient care, clinical consultation, education, research, and administration.
ESSENTIAL RESPONSIBILITIES:
* Utilizes the nursing process when directing or delivering care to patients with WOC needs, including assessment, diagnosis, outcomes identification, planning, and implementation (e.g., coordination of care delivery, health teaching and promotion, consultation), and evaluation
* Utilizes and maintains current physical, psychosocial, clinical assessment, and therapeutic communication skills to determine individual patient needs and expected outcomes
* Participates in committees or task forces related to wound, ostomy, and continence issues
* Provides consultation for individuals with altered skin integrity (e.g., pressure ulcers, draining wounds, fistulae) to develop individualized plans of care and attain expected outcomes
* Provides guidance to staff in implementing protocols to identify, control, or eliminate etiologic factors for skin breakdown, including selection of appropriate support surfaces
* As applicable, with a physician's order, provides appropriate debridement of devitalized tissue (e.g., conservative sharp debridement, silver nitrate (AgNO3) cauterization of nonproliferative wound edges, hypergranulation tissue, or to control minor bleeding)
* Provides appropriate education to patients, caregivers, and staff regarding skin care, wound management, care of percutaneous tubes, and draining wound/fistulae management
* Validates pressure ulcer data collection for nursing quality indicators (e.g., pressure ulcers and restraint use)
* Provides pre- and postoperative education to patients who are undergoing ostomy surgery (and their families), with consideration of the need for physical and psychological adaptation
* As applicable, marks the stoma site preoperatively by determining the appropriate site for stoma placement with consideration of the patient's anatomical markings, physical capabilities, and lifestyle
* Assesses the stoma and stoma functioning and initiates appropriate procedures for stoma care (e.g., removes rod/sutures, measures/fits a pouching system, teaches care of stoma/peristomal skin and pouch)
* Provides appropriate educational information to patients, including postoperative and discharge instructions about care, referrals, and supply needs.
* Serves as a resource for patients after discharge through telephone consultation or outpatient services
* Identifies risk factors for urinary or fecal continence disorders
* Assesses patients with urinary and/or fecal continence disorders
* Establishes an appropriate management program to include dietary and fluid management; bowel training or stimulated defecation program; bladder retraining, prompted voiding, or a scheduled voiding program; pelvic muscle reeducation without biofeedback; indwelling catheter management; recommendations regarding containment/absorptive products and devices and skin care; and education and counseling for patients/caregivers.
* Identifies patients requiring referral for assessment/management of complex urinary or fecal continence disorders.
* Participates in developing and implementing procedures and protocols, based on current national guidelines, to deliver care to patients with the following: Urinary or intestinal disorders resulting in the need for an ostomy; Conditions requiring wound management; Impaired or altered skin integrity; Urinary and/or fecal continence issues
* Assists the nursing and medical staff in maintaining current knowledge and competence in WOC care by providing the following: Formal or informal continuing education programs; Clinical rounds; Lunch and learn sessions; Orientation sessions; Collaborative practice committees; Nursing councils
* Attends continuing education programs related to WOC nursing to maintain current knowledge and skills
* Collaborates with leadership and multidisciplinary clinical practice teams to establish protocols which support best practices to achieve optimal clinical benefits for patients needing WOC care.
* Maintains and submits required activity and statistical reports to the appropriate department supervisor
* Contributes to the selection of cost-effective supplies and equipment related to WOC care, which might need to be individualized for different units/departments.
* Participates in policy review and development as it relates to wound, skin care and ostomies.
* Collaborates with frontline managers and staff on data collection including prevalence studies and the review of reports related to hospital acquired pressure injuries for purposes of determining accuracy, recommending interventions and educational opportunities.

GENERAL OVERVIEW:
The hospital-based Wound Ostomy Nurse serves as a clinical resource and consultant for the acute and rehabilitative care of patients with selected disorders of the gastrointestinal, genitourinary, and integumentary systems such as acute and chronic wounds (e.g., complex draining wounds, fistulae, pressure ulcers, vascular ulcers), ostomies, percutaneous tubes, and continence disorders. Professional responsibilities are carried out through direct patient care, clinical consultation, education, research, and administration.
ESSENTIAL RESPONSIBILITIES:
+ Utilizes the nursing process when directing or delivering care to patients with WOC needs, including assessment, diagnosis, outcomes identification, planning, and implementation (e.g., coordination of care delivery, health teaching and promotion, consultation), and evaluation
+ Utilizes and maintains current physical, psychosocial, clinical assessment, and therapeutic communication skills to determine individual patient needs and expected outcomes
+ Participates in committees or task forces related to wound, ostomy, and continence issues
+ Provides consultation for individuals with altered skin integrity (e.g., pressure ulcers, draining wounds, fistulae) to develop individualized plans of care and attain expected outcomes
+ Provides guidance to staff in implementing protocols to identify, control, or eliminate etiologic factors for skin breakdown, including selection of appropriate support surfaces
+ As applicable, with a physician's order, provides appropriate debridement of devitalized tissue (e.g., conservative sharp debridement, silver nitrate (AgNO3) cauterization of nonproliferative wound edges, hypergranulation tissue, or to control minor bleeding)
+ Provides appropriate education to patients, caregivers, and staff regarding skin care, wound management, care of percutaneous tubes, and draining wound/fistulae management
+ Validates pressure ulcer data collection for nursing quality indicators (e.g., pressure ulcers and restraint use)
+ Provides pre- and postoperative education to patients who are undergoing ostomy surgery (and their families), with consideration of the need for physical and psychological adaptation
+ As applicable, marks the stoma site preoperatively by determining the appropriate site for stoma placement with consideration of the patient's anatomical markings, physical capabilities, and lifestyle
+ Assesses the stoma and stoma functioning and initiates appropriate procedures for stoma care (e.g., removes rod/sutures, measures/fits a pouching system, teaches care of stoma/peristomal skin and pouch)
+ Provides appropriate educational information to patients, including postoperative and discharge instructions about care, referrals, and supply needs.
+ Serves as a resource for patients after discharge through telephone consultation or outpatient services
+ Identifies risk factors for urinary or fecal continence disorders
+ Assesses patients with urinary and/or fecal continence disorders
+ Establishes an appropriate management program to include dietary and fluid management; bowel training or stimulated defecation program; bladder retraining, prompted voiding, or a scheduled voiding program; pelvic muscle reeducation without biofeedback; indwelling catheter management; recommendations regarding containment/absorptive products and devices and skin care; and education and counseling for patients/caregivers.
+ Identifies patients requiring referral for assessment/management of complex urinary or fecal continence disorders.
+ Participates in developing and implementing procedures and protocols, based on current national guidelines, to deliver care to patients with the following: Urinary or intestinal disorders resulting in the need for an ostomy; Conditions requiring wound management; Impaired or altered skin integrity; Urinary and/or fecal continence issues
+ Assists the nursing and medical staff in maintaining current knowledge and competence in WOC care by providing the following: Formal or informal continuing education programs; Clinical rounds; Lunch and learn sessions; Orientation sessions; Collaborative practice committees; Nursing councils
+ Attends continuing education programs related to WOC nursing to maintain current knowledge and skills
+ Collaborates with leadership and multidisciplinary clinical practice teams to establish protocols which support best practices to achieve optimal clinical benefits for patients needing WOC care.
+ Maintains and submits required activity and statistical reports to the appropriate department supervisor
+ Contributes to the selection of cost-effective supplies and equipment related to WOC care, which might need to be individualized for different units/departments.
+ Participates in policy review and development as it relates to wound, skin care and ostomies.
+ Collaborates with frontline managers and staff on data collection including prevalence studies and the review of reports related to hospital acquired pressure injuries for purposes of determining accuracy, recommending interventions and educational opportunities.

Provides distributive pharmacy services, assistance and support for the registered pharmacist.
10 hours evening shift position 4 times per week
Essential Responsibilities:
- Processes medication orders including computer order entry, medication preparation and assembles all items prior to pharmacist final check. (25%)
- Maintains pharmacy inventory and supplies according to established policies and procedures, including inspection of assigned areas and use of pharmacy computer systems. (25%)
- Provides customer service by taking patient's prescriptions and documenting all required information, handles all customer interactions in a courteous and efficient manner, and releases medications required by policy/procedure. (25%)
- For Outpatient Techs: Validates Medicaid, Medicare, or commercial insurance drug coverage and ensures adjudication is complete addressing any error messages, plus handles cash, prepares deposits and accounting reports. (25%)
- For Inpatient Techs: Compounds, stocks, repackages and batches from bulk medications. Prepares routine patient orders for IV pharmaceuticals including compounding, preparation and labeling of any IV admixture including antibiotics, solutions, irrigations, narcotics, parenteral nutrition and chemotherapy (25%)
- Other duties as assigned.
Qualifications:
Minimum:
- Pharmacy Technician Certification (CPhT) or ExCPT Pharmacy Technician - National Healthcareer Association (NHA). Incumbents in role as of July 1, 2022 must achieve CPhT by 1/1/2024.
- Completion of a pharmacy technician training program or 1 year equivalent experience in lieu of formal training program.
Preferred:
- 1 year of pharmacy experience in respective area.
