SCOPE OF ROLE

Reporting to the Supervisor, Academic Placement, the Student, Clinical Placement will be successful during their time by engaging in a variety of patient care experiences both in collaboration with clinical providers and under direct supervision. In this role and in alignment with the affiliation agreement from the Student's Academic Institution, the Student will build professional clinical skills and knowledge to prepare for a profession in a Counseling, Social Work, or Marriage and Family Therapy professional role. The Student, will be successful in developing competent and professional skills in both individual and group facilitation as a licensed trainee.

KNOWLEDGE & EXPERIENCE

  • Current active enrollment in a graduate level program & course requiring onsite direct patient interaction hours with a minimum of 1 clinical placement experience completed.
  • Must be in good standing with their academic institution and comply with all Signature Health policy and procedure.
  • Registered with the Ohio CSWMFT Board as a Counselor Trainee (CT), Social Work Trainee (SWT) or Marriage & Family Therapy Trainee (MFTT). Applicants not currently registered must be eligible for Trainee status on their start date.

Located within the Office of Student Support and Engagement (OSSE), the Student Outreach and Re-engagement (SOAR) Center is a cross-agency collaborative focused on the re-engagement and re-enrollment of chronically truant and out-of-school youth. The SOAR program is intended to ensure the successful transition and graduation of students who have dropped out of school and desire to re-enroll in a program that will assist them in developing their path to graduation. The SOAR teams work closely with District and Options Schools, which offer additional supports and services for students who need a new pathway toward high school graduation, who have been out of school and seek to return, or who may need opportunities to earn credits in an accelerated program.

The Student Engagement Specialist will be responsible for facilitating the reconnection of disengaged students to school via the appropriate academic and social-emotional interventions. Student Engagement Specialists play multiple roles in locating and reconnecting students who are chronically truant, including working with community partners to locate and engage out-of-school students, and completing comprehensive intake interviews and assessments of each individual student's needs. This position will build and maintain strong relationships with the student, family, and school community to support successful placement in the learning environment and ensure the interventions are effective. This position will also monitor students' progress while receiving services, and after re-enrolling into school. The Student Engagement Specialist will report to the SOAR Center Re-Engagement Program Coordinator.

The Student Engagement Specialist will be held accountable for the following responsibilities:
  • Develop a supportive and trusting relationship with at-risk students
  • Conduct a comprehensive intake interview to identify the student's interests, strengths, and specific barriers or challenges impeding successful engagement in school and academic performance
  • Review student transcripts, records, and historical school data to guide student conversations, school placement recommendations, and transition/support plans
  • Utilize active listening, motivational interviewing, and empathetic, non-judgmental conversation to deeply engage with and learn about students
  • Work with students to establish academic and career interests and goals, and to utilize those in the determination of the best school fit
  • Work with the school counselor, social service workers, and community partners to develop individualized intervention and graduation plans to address social/emotional issues impacting performance
  • Monitor student academic progress, attendance, and behavior and provide personalized interventions as needed to keep the student enrolled in the school and on-track for graduation
  • Facilitate events and workshops for character building and career exploration
  • Maintain accurate records of student progress and submit monthly student activity logs and required documentation
  • Conduct home visits and provide support to families in acquiring needed services to ensure student success
  • Work with students to explore career interests and opportunities and establish personal goals.
  • Assist students with exploring multiple educational pathways that meet their academic goals.
  • Work with receiving schools to facilitate enrollment and monitor student persistence.
  • Work with students, parents, and schools to develop individual success plans for each student.
  • Maintain all confidential student records, student plans, and performance tracking reports.
  • Maintain database for tracking student participant information, activity delivery, and program participation data.

In order to be successful and achieve the above responsibilities, the Student Engagement Specialist must possess the following qualifications:

Education Required:
  • Highschool diploma or GED equivalent is required
  • Bachelor's degree from an accredited college or university preferred
Experience Required:
  • Minimum of five (5) years of work experience in social services, health services, education, or related outreach work is required.
  • Two (2) years of this relevant experience may be considered in combination with a preferred degree. Experience with high school students and CPS schools, systems, and policies preferred
  • Experience with Restorative Justice practices and Motivational Interviewing preferred
  • Language proficiency in both English and Spanish preferred
Knowledge, Skills & Abilities

Summary Statement

Job Description

PURPOSE AND SCOPE:

The Nursing student supports FMCNA's mission, vision, core values and customer service philosophy. Adheres to the FMCNA Compliance Program, including following all regulatory and FMS policy requirements.

The Nursing student functions as part of the hemodialysis health care team in providing safe and effective dialysis therapy for patients under the direct supervision of a licensed nurse in accordance with FMCNA policies, procedures, and training and in compliance with regulations set forth by the corporation, state, and federal agencies. Responsible for the setup and operation of hemodialysis machines. Assist in the maintenance of a safe and clean working environment. Supports the FMCNA commitment to the Quality Enhancement Program (QEP) and CQI Activities, including those related to patient satisfaction and actively participates in process improvement activities that enhance the likelihood that patients will achieve the FMC Quality Enhancement Goals (QEP).

DUTIES / ACTIVITIES:

Nursing students must adhere to all facility and FMCNA policies and procedures.

Based on the length of the clinical placement, the student nurses may perform all or some of the duties as time allows after they receive training in the specific functional area. Under the direction and visual supervision of an RN-P, the Student may:

During Clinical placements for less than 3 months duration:

Required Education:

* eLearning: Infection Control; Intro to Basic Dialysis; Intro to Water self-study, Blood Glucose Monitoring
*
* PREP Classes: Infection Control, Evaluating and Monitoring the Patient, Foot checks, water class

Allowable Duties:

*
* Escort patients to and from the scale
* Assist patient with washing access
* Vital signs, including weight
* Attend daily huddles
* Surface disinfection stations/machines
* Hand crank in emergency
* Lab preparation
* Internal machine disinfection
* Check for residual bleach
* Mix bleach water
* Check blood sugars
* Administer ONSP per order
* Pre initiation safety check
* RN Assessment/POC in tandem with preceptor RN
* Attend Care plan meeting
* Crash cart, AED, refrigerator daily checks
* Medication review with preceptor
* Attend QAI Meeting
* Attend POC Meeting
* Foot checks with RN preceptor
* Access assessment with RN preceptor
* Obtain pH and conductivity of machine
* Observe in Home Therapies
* Attend a clinic day in HT
* Attend a day at the access center

During Clinical placement for 3-6 months duration:

Required Education:

* PREP Class: Basic Access, Emergency Preparedness, Safety, Complications, Machine Day

Allowable Duties:

* Machine Set up
* Machine tear down
* Post treatment emptying
* Water checks
* Draw blood cultures and labs
* Lead a daily huddle
* Initiate or terminate a treatment on a patient with a mature fistula or graft only (preceptor to cannulate)
* Medication administration: heparin, lidocaine, and NS
* Assist on QAI team project with preceptor RN
* Preparation of dialysate

PHYSICAL DEMANDS AND WORKING CONDITIONS:

Day to day work includes desk and personal computer work and interaction with patients, facility staff and physicians. The position requires travel between assigned facilities and various locations within the community. Travel to regional, Business Unit and Corporate meetings may be required.

The work environment is characteristic of a health care facility with air temperature control and moderate noise levels. May be exposed to infectious and contagious diseases/materials.

The position provides direct patient care that regularly involves heavy lifting and moving of patients and assisting with ambulation. Equipment aids and/or coworkers may provide assistance. This position requires frequent, prolonged periods of standing and the employee must be able to bend over. The employee may occasionally be required to push and/or pull equipment, exerting up to 15 pounds of force. The employee may be required to lift and to lower solutions on a frequent basis of up to 30 lbs., and on an occasional lift basis up to 40 lbs., as high as 5 feet. There is a two-person assist program and "material assist" devices for the heavier items.

EDUCATION:

An individual who has successfully completed the following as provided by the academic program:

* Three quarters of the ADN program, or
* Junior year of a BSN program, or
* Two years of a nursing school program
* Other programs require approval from Clinical Services and Education

Note: If a student nurse has completed an approved PCT certification program, please refer to PCT Student Clinical placement Policy that outlines allowable duties for students enrolled in a PCT certification program.

EXPERIENCE AND REQUIRED SKILLS

RELATIONSHIPS:

The clinical placement student cannot work under the supervision of a family member.

The student cannot care for a patient who is a family member. If the student's family member is a patient at the facility in which they work, the student will be scheduled to work on days the patient is not in the facility or will be assigned to another patient.

SUPERVISION:

Will be supervised by a Clinical Manager and Preceptor.

Preceptor: An individual who directly supervises the work of the Nursing student in the dialysis facility.

OTHER:

Performs additional duties as assigned.
