Purpose:
The Community Support Facilitator provides support and leadership for consumers to participate in activities and receive services in community settings. Activities and services are defined by the consumer and his or her Person Centered Plan and Individualized Personal Plan.
Principle Responsibilities:
1. Provide group and/or individual support in the community as identified in the consumers Individualized Personal Plan.
2. Develop opportunities for clients to participate as valued and contributing members of the community. Facilitate and model interactive relationships with community members.
3. Facilitate and develop activities to build relationships, skills, and experience in the community. Travel with and/or transport client throughout the local community.
4. Develop and expand consumer participation in typical and generic community activities chosen by the consumer.
5. Act as advocate on behalf of consumers served.
6. Complete required forms and accountability documentation, incident reports, case recordings, IPPs and other required documentation.
7. Maintain a professional relationship with consumers, family members and appropriate others. Represent HOPE in the community in a professional manner.
8. Implement behavioral intervention plan when needed to assure consumer safety and promote learning.
9. Coordinates consumer programming with Centers activities, services, and staff.
10. Acts as a mandated abuse reporter.
11. Perform other duties as assigned, to assure efficiency of program services.
Minimum Qualifications :
The Community Support Facilitator should possess a High school diploma, GED, or equivalent, plus two years of related experience or an approved combination of experience and education.
Possession of a valid California drivers license and clear driving record.
Knowledge, Skills and Abilities of the Community Support Facilitator role:
1. A belief that all people are valued members of the community.
2. An understanding of the principles of normalization and commitment to community integration.
3. Ability to work independently and cooperatively as a member of the team.
4. Excellent communication and documentation skills including use of computerized word program and data base.
5. Knowledge and understanding of alternative communication systems and assistive technology.
6. CPR and First Aid certification (Hope can provide necessary training, if needed).
7. Positive and cooperative attitude toward a consumer driven service system working with developmentally disabled adults.
8. Ability to be flexible with scheduling and work in a consumer driven environment
9. Strong interpersonal skills. Ability to work with a diverse population.
Environmental Conditions:
Extensive travel around town and in the consumer community; use of public transportation where possible; exposure to climatic conditions, heat, cold, humidity, dampness, sun, pollens; lifting/carrying supplies for consumer supports to and from locations, walking, standing, visual/auditory acuity while supporting consumer in the community; position is community based and in local and consumer home community, climbing stairs to reach the consumer is occasionally necessary; work alone as well as closely with others, sitting for periods of time in meetings and use of office environment; produce work on computers/office equipment.

The Colorado State University ( CSU ) System, along with its partners, has developed a unique and exciting public facility called CSU Spur at the National Western Center in Denver, Colorado. The CSU Spur campus houses three buildings focused on water, food and agriculture, human and animal health, and sustainability. Within the Spur buildings, CSU conducts research and convenes experts from around the globe; students of all ages engage with science and have opportunities for exploratory learning, transforming their thinking about food, water, and health, and the careers available in these fields; and partners join in programming the space and bringing evolving content to the year-round campus. CSU Spur is part of alarger 250-acre redevelopment project, which started in 2013 and reimagines what the National Western Center can be, creates a site that will support the National Western Stock Show long-term, and makes the location a year-round destination. Position Summary
CSU Spur is seeking two motivated and enthusiastic individuals to join the Education Team as temporary hourly Education Support Facilitators. These individuals will be part of a team delivering programs at the CSU Spur campus in Denver that inspire life-long learners to tackle global problems around food, water, and health, and will play a key role in the future of educational offerings at a brand-new state-of-the art campus.
Education Support Facilitators will support the CSU Spur vision and Education Team by executing the daily programmatic logistics and delivery necessary for successful educational experiences at Spur for pre-K-12 audiences, families, community partners, and general campus visitors. This position is responsible for delivering educational content both in classroom program facilitation and exhibit interpretation, engaging with on site visitors, and collaborating with staff and volunteers to support the programmatic goals of the CSU Spur campus.
This role will have a variable schedule, which may include weekends, and could include some evenings. The individuals in this role are requested to work up to 30 hours a week.
This position is supervised by the CSU Spur School Programs Coordinator.
Program Areas of Interpretation:
+ Food: In the Terra building, education staff will be working with partners on controlled agriculture, food systems education, urban gardening, nutrition education, and food development processes.
+ Water: In the Hydro building, educators will partner with Denver Water experts to provide experiences around water conservation, quality, and water systems. Programming will include usage of an outdoor backyard garden space that connects to the South Platte River for riparian ecosystem programming.
+ Health: In the Vida building, the education team will interpret live veterinary procedures, equine-assisted services, and equine rehabilitation therapy in partnership with the Dumb Friends League, the Temple Grandin Equine Center, and CSU Equine Sports Medicine, providing opportunities for visitors and school groups to learn about animal and human health through active observation and engagement.
These individuals will learn new and innovative ways to provide educational programming through virtual reality, in person hands on interactive experiences, exhibit interpretation, indoor and outdoor learning opportunities, and live interpretation of on show surgical procedures, animal therapy, equine assisted services, as well as cooking, planting, and water demonstrations.
Flexibility and an openness to learn and try new ideas and experiences is highly encouraged.
Position Minimum Qualifications
+ 2 years' experience working with children, families, and multigenerational audiences, which may include working in schools, informal education, or customer service.
+ Demonstrated experience in public speaking or public demonstrations.
+ Available to serve a diverse weekly schedule, including evenings, weekdays, and weekends.
+ Proficient in Microsoft Office (PowerPoint, Outlook, Word, Excel)
Preferred Qualifications
+ Ability to communicate with program constituents in both Spanish and English is preferred.
+ Familiar with scientific concepts surrounding food, water, and environmental health.
+ Demonstrated experience delivering exceptional customer service.
+ Demonstrated knowledge of, and relevant ability with communities in urban and rural settings.
+ Primarily availability is from 9 am to 2 pm to support school group engagement.

Our support group program is a vital one for the many caregivers in Orange County that are caring for a loved one with dementia. This informal discussion group offers resources for problem solving, support and friendship and to learn from others. It is a place for caregivers to share experiences and feelings and hear from others in similar situations.
Become a support group facilitator: We are looking for dedicated, compassionate individuals willing and able to volunteer monthly to moderate our groups in Orange County. Previous or professional experience is not required, reflective listening skills are.
You will attend our facilitator training, approx. three hours, observe support groups, then co-facilitate until you are comfortable being on your own.

An Alzheimer’s Association support group is an open gathering of people with common problems, needs and interests who come together to share their feelings, thoughts and experiences in a combined effort to better cope with and manage the shared problems of dementia in a safe environment. The goal of the Alzheimer’s Association support groups is to provide emotional, social and/or educational support to group members.
Qualifications of a Support Group Facilitator: Experience with dementia as a family member or health care professional preferred. Ability to be empathic and separate personal needs from group needs. Strong listening skills and effective communication skills. Ability to redirect and facilitate discussion as well as fulfill the duties and responsibilities as described in the Facilitator Agreement. Ability to create and maintain a safe environment where participants’ thoughts and feelings are valued and group members are treated with fairness, equity and respect. Commitment to promote the mission and services of the Association and willingness to act as an extension of the Association and to put other responsibilities and obligations aside when acting in this role.
Responsibilities include but are not limited to: Follow expectations and recommendations of the Caregiver Support Group Facilitator Manual, including participation in initial training, record keeping, and regular check ins with Association staff. Arrange for speakers/presenters for the group as needed. Encourage participants to provide assistance and emotional support to one another. Present pertinent Association materials and information to group members. Inform the Alzheimer’s Association in advance of any changes in group facilitators, time or location of group meetings.
Time commitment: 1.5 hours once per month
Training: Three hours of online training (videos), and two hours in person to shadow
