The **Placement Support Coordinator** plays a critical role in the Foster Care team by ensuring that children are placed in appropriate and caring foster homes. This role requires excellent communication skills, empathy, and a strong sense of responsibility to match children with the most suitable foster families. The coordinator works closely with case managers, foster families, and other stakeholders to facilitate placements and provide ongoing support.
**How you will make an impact:**
+ Perform case management telephonically and/or through home visits within the scope of licensure for special programs like Foster Care.
+ Assess and identify appropriate foster home placements considering each child's specific needs and circumstances.
+ Develop comprehensive care plans to address objectives and goals identified during assessments.
+ Facilitate pre-placement visits and introductions to help children and families transition smoothly.
+ Coordinate with internal and external resources to meet the integrated (physical and behavioral) whole person care needs of the member.

+ Requires an active and current license as an LCSW, LCMHC, LPA, LMFT, or RN issued by the state of North Carolina.
+ Requires a MS/MA in social work, counseling, or a related behavioral health field, or a degree in nursing.
+ Requires three (3) years of experience providing care management, case management, or care coordination to individuals served by the child welfare system.
**Preferred:**
+ Previous experience in foster care, social work, or related fields is strongly preferred.
+ Strong understanding of the foster care system and child welfare policies is strongly preferred.
+ Excellent interpersonal and communication skills, with an ability to work collaboratively is strongly preferred.

The Care Management Extender (Peer Support Specialists) is responsible for engaging with community partners, providing education, and strengthening relationships. This position plays a crucial role in supporting individuals and families by navigating the CFSP Foster Care program. You will utilize your professional expertise and lived experiences to guide and connect members and caregivers with those who can provide insight and expertise to assist in overcoming challenges.
**Primary duties may include, but are not limited to** :
+ Engage in general outreach, engagement, and follow-up with members and their caregivers and families to support care management efforts.
+ Support care managers as they advocate for foster parents and other caregivers, addressing their needs or challenges with various agencies and systems.
+ Coordinate services and appointments, such as wellness reminders and arranging transportation.
+ Assist the care manager in assessing and addressing unmet health-related need
**For the State of North Carolina, in accordance with federal/state law, scope of practice regulations or contract, the requirements are** **:**
+ Requires a HS diploma or equivalent and a minimum 2 years of related experience; or any combination of education and experience, which would provide an equivalent background.
**Preferred Qualifications** **:**
+ Lived experience that provides valuable insights into navigating similar challenges is a must!
+ Peer Support Certification is preferred.
+ Excellent skills in maintaining professionalism and effective communication when interacting with individuals, providers, and stakeholders.
+ Skilled in articulating ideas clearly and concisely, both verbally and in writing.
+ Ability to build respectful and appropriate relationships with individuals across a wide range of disabilities and health conditions, as well as with their families.
+ Strong capacity to collaborate effectively within a team environment and follow guidance from the Care Manager and the Care Management department.
