Brief Job Description:

The Process Improvement Unit (PIU) Strategic Coordination Project Manager (PM) works under the direction of the Director of the Process Improvement Unit in the Office of Compliance and System Coordination (OCSC). This position is responsible for planning, implementing, and evaluating the agency's cross-program and cross-agency project initiatives. Establishes reporting tools and mechanisms to regularly report cross division coordination activities and other agency projects to executives and other key staff. Manages DSHS special projects, utilizing project management tools in coordination with agency business and program units. The position facilitates work groups and compiles and analyzes information to develop project recommendations and reports. May plan and direct organizational assessments and monitor and report on successes and challenges related to organizational change activities. Promotes and implements approaches to continuously assess and improve health outcomes and/or business processes. The work requires regular interaction and coordination with executive, program, and legal staff and others within DSHS and HHSC.

Essential Job Functions (EJFs):

EJF 1: The PM serves as project manager and lead for the development of the agency's strategic and operational plans. This work involves: providing technical assistance to all functional divisions and programmatic areas within DSHS for the development of their area's strategic and operational initiatives; leading the agency in planning for the agency's submission of it's formal strategic plan; developing all necessary documents as dictated by the State Legislative Budget Board (LBB), legislature, and Office of the Governor; working with agency staff across all areas of DSHS and HHSC to develop strategic initiatives; facilitating project status reporting; leading work groups and project teams; ensuring that any discussion or decisions made regarding project status are accurately captured in the tracking system; assisting program area managers when it is time to close out a project; and working with program area managers to evaluate performance to see that the project had the intended impact (50%).

EJF 2: The PIU Project Manager serves as project manager for initiatives that impact key functions within DSHS. This work involves: providing technical assistance to areas in the development of project charters and work plans at the start of a new project; ensuring that critical project documentation is completed timely; facilitating project status reporting; leading work groups and project teams; ensuring that any discussion or decisions made regarding project status are accurately captured in the tracking system; assisting program area managers when it is time to close out a project; and working with program area managers to evaluate performance to see that the project had the intended impact (20%).

EJF 3: The PM leads management studies conducted by the PIU team, including, but not limited to analyzing any data sources and identifying possible trends; contributing to the design and implementation of survey tools; identifying key findings; and, proposing recommendations for change that represent practical solutions to problems identified during the study. The PM will also work with the area under review to ensure that recommendations are addressed in an action plan that will subsequently become part of DSHS business processes. The PM will also be available to oversee, review, and evaluate corrective actions to determine if the actions have led to increased efficiency and effectiveness. (15%).

EJF 4: The PM is responsible, alone or in conjunction with others, to produce a variety of communication items related to departmental planning exercises and department-wide initiatives being undertaken at DSHS that may include: DSHS strategic and operational plans, priority initiatives, response to legislative request or reports, Survey of Employee Engagement (SEE), etc. (10%).

EJF 5: Performs other duties as assigned. Other duties as assigned include but are not limited to actively participating and/or serving in a supporting role to meet the agency's obligations for disaster response and/or recovery or continuity of operations activation. Such participation may require an alternate shift pattern assignment and/or location (5%).

As a Systems Engineer in the Coordination group you will manage/coordinate the review, analysis, and response to system level requirements and requirement changes to the Weapons System for the Fleet Ballistic Missile (FBM) Program, specifically Missile interfaces. In this roll you will be directly responsible for coordinating with subject matter experts (SMEs), Management, Customers, DoD, and active Duty Military/Navy. The ability to effectively communicate technical information in written or verbal form to the customer and other members of the program is required, as is a strong attention to detail, being organized, and a commitment to excellence and ownership in the creation of your work products. A strong understanding of system level requirements and proper requirement verbiage is preferred, as you will be working with Engineers in all disciplines to ensure all parts of the Weapons System integrate to perform to the customer's expectations.

For over 60 years, the Fleet Ballistic Missile (FBM) team has supported the Navy's mission to provide affordable and credible strategic defense. We are seeking a candidate to support the Fleet Ballistic Missile (FBM) Program by joining the Systems Engineering Integration Team and Test (SEIT) as a Systems Engineer. The SEIT Requirements and team handles all System level requirements for the Trident II ballistic missile (commonly known as the D5LE missile) and now the development of the D5LE2 missile, oversees interface definitions, plans and executes processes, and has fingers in every development project under the FBM Program.

We have a Weapon System level view of the FBM Program with activities related to integrating the D5LE and D5LE2 missile into the submarine weapon system, as well as supporting efforts to integrate missile subsystems into the flight vehicle . You will play an integral role on this team by taking on the following key responsibilities:

As a Systems Engineer in the Coordination group you will manage/coordinate the review, analysis, and response to system level requirements and requirement changes to the Weapons System for the Fleet Ballistic Missile (FBM) Program, specifically Missile interfaces. In this roll you will be directly responsible for coordinating with subject matter experts (SMEs), Management, Customers, DoD, and active Duty Military/Navy. The ability to effectively communicate technical information in written or verbal form to the customer and other members of the program is required, as is a strong attention to detail, being organized, and a commitment to excellence and ownership in the creation of your work products. A strong understanding of system level requirements and proper requirement verbiage is preferred, as you will be working with Engineers in all disciplines to ensure all parts of the Weapons System integrate to perform to the customer's expectations.

**Basic Qualifications:**

- CAD drafting (Creo/AutoCAD/IDEAS/etc.)

- Knowledge of Windchill/EPDM

- Experience with Model Based Systems Engineering practices and implementation

- Familiarity with Engineering and System Interface Requirements

- US citizenship with Valid Secret Clearance (cannot be Inactive more than 2 years); ability to obtain and maintain a Top Secret Security Clearance.

**Desired Skills:**

-Government/DoD Experience

- Basic programming

- Performs technical planning, system integration, verification and validation, cost and risk, and supportability and effectiveness analyses for total systems.

- Perform analyses at all levels of total system product to include: concept, design, fabrication, test, installation, operation, maintenance and disposal.

- Ensures the logical and systematic conversion of customer or product requirements into total systems solutions that acknowledge technical, schedule, and cost constraints.

- Performs functional analysis, timeline analysis, detail trade studies, requirements allocation and interface definition studies to translate customer requirements into hardware and software specifications.

- Proficient with Correspondence - grammar formatting/etc.

- Communication Skills

- Microsoft Office (Excel/Word) Proficient

- Comfortable with Public Speaking

Care Coordination Asst

The Care Coordination Assistant role is responsible for supporting critical and time sensitive functions of the Clinical Care Coordination team, including but not limited to managing post acute care referrals, direct communication with patients/families, post acute care agencies, and other key stakeholders. The Care Coordination Assistant performs these duties with a high degree of accuracy utilizing critical thinking skills and in compliance with hospital policies, standards of practice, and Federal and State regulations. Essential Key Job Responsibilities

Collaborates with and supports the Care Coordination team, including, but not limited to the RNCC and Social Worker.

Manages timely post-acute care referrals, to assist with length of stay management and mitigation of denials.

Communicates and collaborates with interdisciplinary teams, patients, and family to facilitate care progression.

Ensures post-acute follow-up appointments for identified patients to assist with readmission prevention and ensuring continuity of care.

Delivers required patient notifications and letters, and obtains patient or family signatures on required documentation as needed to meet regulatory compliance.

Manages transportation referrals as requested.

Complies with confidentiality policies, HIPAA regulations, and organizational standards when transmitting protected patient information to external agencies and vendors as needed for placement, payment or referral.

Ensures compliance with hospital policies, standards of practice, applicable Federal or State regulations pertaining to their role, and contributes to the identification of opportunities for process improvement.

Prioritizes, manages, and documents communications relating to insurance requests, authorizations, denials and Physician Advisor status recommendations to the appropriate licensed staff member as applicable.

Qualifications

Minimum Qualifications

Required Education and Experience:

  • Associate (two-year) Degree in Business/Health-Related field or High School Diploma or GED with a current license/certification as a healthcare professional (Medical Assistant/Certified Nursing Assistant/Emergency Medical Technician).
  • Medical terminology certificate or ability to demonstrate knowledge of medical terminology is required.
  • Intermediate to advanced computer skills required to include Microsoft Office and/or Google Workspace applications including Word, Docs, Excel, Sheets, typing, and data entry skills.

Preferred Education and Experience:

  • At least 3 years previous experience in health care, care coordination, utilization review, discharge planning or similar healthcare setting.
  • Bachelor's degree or BSW.
  • Experience supporting clinicians in demanding and fast paced patient care environments.

Required Licensure and Certifications:

  • None

Preferred Licensure and Certifications:

  • LVN/LPN, Licensed BSW, Medical Assistant, Certified Nursing Assistant, or Emergency Medical Technician certificate preferred.
