The Intensive Home-Based Therapeutic Care program provide services for youth with emotional challenges and/or multistressed families. The scope of services includes flexible individualized treatment, rehabilitation, supervision, and support that vary in intensity, depending on youth and family needs. The services are delivered by a core team of staff using a Wraparound Philosophy support which helps family, support members, and service providers work together to support the family or child. In addition, all services are delivered using a Trauma Informed Care approach.
The Outreach Counselor will provide direct support weekly, and ongoing, to each assigned family and youth. The services will be tailored to meet the unique needs of each youth and family served, providing opportunity to maximize their ability to spend time together at home and in the community. They will also collaborate with the team to promote a culture in which each individual served, and their family member’s point of view, experiences, and preferences are recognized, understood, and respected, and in which self-determination and decision-making in treatment planning are maximized and supported.

Therapeutic Foster Care is a trauma informed program that provides foster care placements with skilled professional parents to children and youth who are not currently able to live with their family of origin. We provide wrap around supports to children placed in our homes, which includes significant support for professional parents as well as training and generous tax free reimbursement. The Home Finder Therapeutic Foster Care recruits, trains, and certifies high-quality therapeutic foster/kinship homes. This includes providing support to foster/kinship parents for successful placements. This position collaborates with state and county partners and leads home finding tasks and functions.
Essential Job Functions

Recruit, train, and certify high-quality therapeutic foster/kinship homes
-
Organize, coordinate, and facilitate foster/kinship parent recruitment events and training schedules.

Identify the relevant curriculum and provide training, 1:1 or in groups, for potential foster parents/kinship caregivers to assist them in meeting certification requirements.

Identify additional resources and/or training needed to ensure the foster/kinship parents are capable and confident in addressing the needs of youth in foster or kinship care.

Perform end to end home certifications.

Complete all aspects of the home study process which includes conducting a thorough home assessment and writing the home study reports with a clear rationale for the recommendation.

Obtain and complete all paperwork and documentation required by regulatory bodies including all background and State Central Registry checks to ensure the certification process for foster/kinship homes is on file and meets all necessary requirements.

Lead and facilitate in home meetings with foster/kinship families. Works to understand successful aspects of the placement and gaps where support is needed.

Create and implement plans of corrective action or retraining for foster/kinship homes based on any skill gaps or regulatory deficiencies.

Ensure foster/kinship homes maintain their certification; assist foster/kinship parents with the completion of annual recertification.

Assist in referral assessment and foster/kinship home matching.

The TFC Family Consultant is a key member of the TFC Clinical Team and plays a vital role in supporting youth placed in therapeutic foster care. Core Responsibilities
- Youth & Family Support
- Build supportive, therapeutic relationships with youth and foster families.
- Ensure placements are safe and conducive to each youth’s growth and healing.
- Advocate for the needs of youth and help coordinate services across providers.
- Case Management
- Serve as primary coordinator of each youth’s Individual Service Plan (ISP).
- Attend and participate in admission, ISP, and placement meetings.
- Ensure timely documentation of case notes, logs, ISPs, and incident reports.

- Conduct weekly check-ins with youth and foster families; meet jointly at least monthly.

Key Responsibilities:
- Develop, implement, and evaluate therapeutic programs for adolescents with various mental health needs.
- Supervise, train, and mentor a team of licensed therapists, counselors, and support staff.
- Conduct comprehensive psychosocial assessments and diagnostic evaluations.
- Oversee the creation and execution of individualized treatment plans.
- Ensure adherence to ethical standards, professional guidelines, and regulatory requirements.
- Facilitate group therapy sessions and individual counseling as needed.
- Collaborate with families, guardians, and external agencies to coordinate care.
- Monitor client progress and adjust treatment strategies to optimize outcomes.
- Manage program budgets and resources effectively.
- Contribute to the continuous improvement of services through data analysis and feedback mechanisms.
- Participate in on-call rotations as required.
- Master's degree or Doctoral degree in Psychology, Social Work, Counseling, or a related field.
- Current licensure as a LCSW, LMFT, LPC, or Psychologist in the state of Tennessee.
- Minimum of 5 years of clinical experience working with adolescents, with at least 2 years in a supervisory or management capacity.
- Extensive knowledge of adolescent development, mental health disorders, and therapeutic modalities (e.g., CBT, DBT, Trauma-Informed Care).
- Proven ability to lead and motivate a clinical team.
- Excellent communication, interpersonal, and problem-solving skills.
- Experience with electronic health records (EHR) systems.
