Bayonne Rheumatology has an immediate opening for a friendly, patient focused and detailed oriented Patient Care Coordinator  to join our team. The Patient Care Coordinator  is responsible for the completion of set processes and protocols. Works cooperatively with all members of the care team to support the vision and mission of the organization, deliver excellent customer service and adhere to Lean processes. Supports the teams in meeting financial, clinical and service goals.

Schedule:  A 40-hour work week, Monday through Thursday from 8:00 am to 5:00 pm, Friday 8 am to noon.  The schedule will be determined by the supervisor upon hire.

Location : 191 Lefante Way, Suite 1-C, Bayonne, NJ 07002

Primary Responsibilities:

  • Obtain accurate and updated patient information, such as name, address and insurance information
  • Perform insurance verification on the date of service
  • Obtain patient signatures for required documents
  • Upload a valid Government issued state ID of patient and patient insurance cards
  • Accurate and complete occupational medicine registration workflow
  • File and maintain medical records
  • Consistently correct registration work queue errors
  • Confirms and schedule appointments
  • Answer telephone calls promptly and courteously
  • Perform referral documentation promptly
  • Working daily in the claims edit work queue to correct registration errors for submission to insurance companies
  • Performs certain follow-up services for patients in a prompt and courteous manner, such as scheduling specialist appointments, scheduling follow-up appointments and obtaining copies of lab results or specialist reports
  • Adhere to the standards identified via Sparq regarding Optum Employee Policies
  • Work cohesively with fellow employees to achieve specific team goals
  • Comfortable working in high pace environment 
  • Participate in planning, coordinating, and implementing care in conjunction with the multidisciplinary team in accordance with the Plan of Care
  • Assure the continuity of care through scheduling and tracking systems
  • Provide effective communication to patient / family team members and other health care professionals as evidenced by documentation, case conferences, communication notes, and evaluations
  • Notify clinical staff of schedule changes as they occur
  • Comply with administrative policies to ensure quality of care
  • Demonstrate precision and efficiency in scanning documents and monitoring the fax server, retrieving and / or scanning documents and assigning to the appropriate electronic chart
  • Participate in orientation (scheduling segment) of new field employees and distribute updates and changes as needed
  • Perform other related duties and assignments as required
  • Organize and maintain medical records
  • All employees are expected to keep abreast of current medical requirements relevant to their position, which includes maintaining patient confidentiality and abiding by all HIPPA and OSHA requirements  
  • Complete new clinical competencies rolled out by the Educational Committee

Required Qualifications:

  • High School Diploma/GED (or higher)
  • 1+ years of experience working in medical office position performing duties such as scheduling appointments, checking patients in/out, insurance verification, collecting co-pays, and maintaining medical records
  • Intermediate level of computer proficiency including MS Word, Excel and Outlook be able to use multiple web applications
  • Ability to travel 10% of the time to cover other offices as needed and have reliable transportation

Preferred Qualifications:

  • Knowledge of Epic EMR software
  • Bilingual in Spanish and English

Soft Skills:  

  • Ability to work independently and maintain good judgment and accountability

**PRINCIPAL** **DUTIES** **AND** **RESPONSIBILITIES:**
**Patient Related**
Education:
+ Assist other health care members in providing patient specific detailed education regarding adequacy measures where appropriate - Online Clearance Monitoring (OLC), Adequacy Monitoring Program (AMP), Urea Kinetic Modeling (UKM), and regarding disease process/access.
Treatment:
+ Welcome assigned patients and inquire as to their wellbeing since their last treatment.
+ Evaluate vascular access for patency, perform vascular access cannulation, and perform administration of heparin as delegated or as allowed by state law.
+ Obtain necessary pre and post treatment vital signs and weight and perform vascular access evaluation pre- treatment
+ Initiate dialysis treatment according to prescribed orders including blood flow (QB) and dialysate flow (QD).
+ Evaluate intradialytic problems and provide intervention as prescribed by physician order or as directed.
+ Monitor patients' response to dialysis therapy.
+ Obtain vital signs prior to reinfusion; perform all relevant functions necessary for the discontinuation of treatment - document.
+ Discontinue dialysis treatment according to established procedures and evaluate patient prior to termination of venous access - standing & sitting blood pressure
+ Obtain Hemostasis and apply appropriate dressings.
+ Evaluate the patient prior to discharge.
+ Perform and record Pre and Post dialysis evaluation, weight, and vital signs with initial identification
+ Responsible for calculating and entry of individual patients' dialysis machine programming for Ultrafiltration (UF) goal; treatment time; and UF modeling as prescribed.
+ Responsible for ensuring appropriate safety alarms are enabled, including Narrow Venous Limits, (NVL) 160.
+ Record accurate and timely information regarding vital signs and treatment parameters onto the Hemodialysis Treatment Sheet with initial identification
+ Responsible for accurate documentation of information related to patient treatment including completion of Hemodialysis Treatment Sheet and, if applicable, progress notes.
+ Report any significant information and/or change in patient condition directly to the registered nurse or supervisor.
+ Observe patient and conduct machine safety checks according to facility policy. Report any change or unusual findings to the registered nurse or supervisor.
+ Perform and document any intervention for unusual patient status and document patients' response to intervention.
+ Evaluate patients' vascular access during treatment including arterial and venous monitoring pressures, provide appropriate intervention as needed, document and report any unusual findings to the registered nurse or supervisor.
+ Obtain all prescribed laboratory testing and prepare specimens for collection.
+ Ensure that all blood spills are immediately cleaned with appropriate disinfectant according to facility policy.
+ Prepare, organize, and efficiently utilize supplies and equipment to prevent wastage.
+ Monitors patients performing self-care under the supervision of RN.
**Staff Related**
+ Assists in training of applicable direct patient care staff on appropriate programs under the supervision of a nurse.
**Education/Communication:**
+ Follows all organization business policies, procedures and systems incorporated into training, including compliance with ethical business practices
+ Maintain environmental integrity and aesthetics - ensure all areas are safe and clean.
+ Clean and disinfect dialysis machine surface, chair, equipment, and surrounding area between treatments according to facility policy.
+ Conduct all tasks necessary for preparation for dialysis treatment and document where appropriate and perform all required pretreatment dialysis machine alarm testing including Pressure Holding (PHT).
+ Initiate Solution Delivery System (SDS) system.
**RECORD KEEPING:**
+ Complete and document ongoing participation in Continuous Quality Improvement (CQI) activities.
+ Enters all treatment data into the designated clinical application in an accurate and timely manner.
+ Review treatment sheets for completeness, ensure nursing signatures are documented, and ensure
omitted entries are completed or corrected by appropriate staff.
+ Prepare initial patient chart for admission and compile paperwork for appropriate placement in chart.
+ Prepare/print lab requisitions for prescribed or stat Lab specimens according to laboratory destination.
+ Collect, label, appropriately prepare and store lab samples according required laboratory specifications.
+ Ensure collection of lab specimens by appropriate lab courier.
**INFECTION CONTROL:**
+ Assists in collecting information for infection control audits.
+ Supports staff and patient Adherence to infection control practices.
+ Follows infection control Policies and Procedures
+ Participates in and reinforces infection control education of patients and families under the direction of the CM or designee as allowed by state law.
**TRANSITIONAL CARE UNIT:**
+ Follows all newly admitted patients through their first 4 weeks of dialysis and coordinating their transitional dialysis care
+ Reinforces all education and care related matters as it relates to the new patient as allowed by state law
+ Liaisons with appropriate FKC staff members to provide the best patient experience including making sure all disciplines are scheduled to see patient according to plan.
+ Assists in assuring patient understanding or home dialysis products, benefits of home and how home dialysis can work for them.
+ Sets and preforms a treatment on all machines used in the transitional care unit.
+ Assist in assuring patient understanding of treatment options including demonstration of the peritoneal dialysis set-up.
+ Assists with coordination of transition to patient's modality choice.
+ Assists with supply ordering and inventory for TCU

Assists in the direction, coordination and development of performance improvement, patient safety and patient experience outcomes. Assures the unit is always in a state of survey readiness and meets compliance with all environmental/equipment requirements. Insures the prompt placement of purchase orders and executes follow up to assure prompt delivery of materials and/or effective performance of services. Makes daily rounds to insure smooth functioning and throughput of patients on the unit and intervenes with all ancillary and support services to provide quality patient care.

The compensation offered to the candidate selected for the position will depend on several factors, including the candidate's educational background, skills and professional experience.

Job Overview: 

Ensures all scheduled and add-on surgical procedures and pain management procedures have a completed electronic record in the OR Information Management System. Reconciles these records, auditing them for content accuracy and closing these records. Inputs new implant and supply information into the system. Allocates C-codes and system number and sends to the charge master so accurate billing can be accomplished. Assists OR Manager with creation of staff schedules as needed. Organizes and administers individualized age specific professional care to patients in the operating room. Participates in staff orientation in relation to appropriate documentation. Participates in unit performance improvement evaluation initiatives. Takes charge as needed. Insure that (unit/department/division) is in compliance with all applicable policies, laws and regulations. 

Qualifications: 

  • Must be a graduate from an accredited school of nursing; RN BSN preferred; current NJ RN licensure required,
  • BLS certification required; knowledge of BS in life science with a minor in information system preferred,
  • 2-3 years experience in the Operating Room
  • Knowledge of PICIS, data entry and PC environment,
  • Successful completion of all Orientation Programs,

Certifications and Licenses Required:

  • Current Registered Nurse License in the state of New Jersey
  • Basic Life Support (BLS) from the American Heart Association upon hire
  • Certifications as required by law, stature and/or regulation for area or specialization

Scheduling Requirements:

  • Evening Shift
  • Full-Time
  • 37.5 Hrs weekly
  • 2pm - 10pm 
  • Schedule during the week will vary depending on department needs

Essential Functions:
The Assistant Director of Patient Care is responsible for maintaining clinical/management skills; keeps abreast of current issues/trends, based on current healthcare environment. Serves as a resource for staff and other colleagues. Provides nursing administrative coverage as required. Ensures staff participation in Nursing Quality improvement activities and identifies quality issue trends with respect to quality patient occurrences, Incident Reports and patient care outcomes; submits reports and statistics as required, and devises, supervises and/or participates in implementing planned changes to improve patient care. Participates in the development, implementation and evaluation of hospital policies and procedures related to nursing practice and personnel management. Reviews, analyzes, updates and implements action plans to improve patient care and satisfaction.
