POSITION SUMMARY:

We are looking for a full time Clinical Therapist to serve older children and teens. Our clients reside in mental health units on campus, as well as in foster care and independent living programs. In this role, you will have a small caseload of no more than five cases at a time as well as the opportunity to be a part of dynamic team where you can work alongside of and learn from your fellow clinicians.

Most curriculums follow evidence-based treatment, including Dialectical Behavioral Therapy (DBT). Additionally, our therapists have opportunities to develop curriculums under the clinical guidance of the program director. This opportunity allows you to learn and practice DBT under some of the best supervision in the state of Ohio. As a Clinical Services Therapist, you will have the ability to hone your clinical skills, grow your career with free training and supervision, and develop as a therapist while providing meaningful, value-driven work to clients in need. There is some flexibility in scheduling.

RESPONSIBILITIES INCLUDE:
  • Provide professional clinical services, including but not limited to: biopsychosocial assessments, treatment plans and reviews, individual counseling, group counseling, family therapy, consultation, aftercare planning, and termination reports
  • Collaborate with respective treatment team members regarding treatment planning, progress, and ongoing needs in a timely and consistent manner

QUALIFICATIONS:
  • Master's Degree and valid Ohio LSW, LPC, or MFT required

  • Record accurate and timely information regarding vital signs and treatment parameters onto the Hemodialysis Treatment Sheet with initial identification   

  • Responsible for accurate documentation of information related to patient treatment including completion of Hemodialysis Treatment Sheet and, if applicable, progress notes.   

  • Report any significant information and/or change in patient condition directly to the registered nurse or supervisor.

  • Observe patient and conduct machine safety checks according to facility policy. Report any change or unusual findings to the registered nurse or supervisor.

  • Perform and document any intervention for unusual patient status and document patients' response to intervention.

  • Evaluate patients’ vascular access during treatment including arterial and venous monitoring pressures, provide appropriate intervention as needed, document and report any unusual findings to the registered nurse or supervisor.

  • Obtain all prescribed laboratory testing and prepare specimens for collection.   

  • Ensure that all blood spills are immediately cleaned with appropriate disinfectant according to facility policy.   

  • Prepare, organize, and efficiently utilize supplies and equipment to prevent wastage.   

  • Monitors patients performing self-care under the supervision of RN.

Staff Related

  • Assists in training of applicable direct patient care staff on appropriate programs under the supervision of a nurse.

Education/Communication:

  • Follows all organization business policies, procedures and systems incorporated into training, including compliance with ethical business practices

  • Maintain environmental integrity and aesthetics - ensure all areas are safe and clean.

  • Clean and disinfect dialysis machine surface, chair, equipment, and surrounding area between treatments according to facility policy.

  • Conduct all tasks necessary for preparation for dialysis treatment and document where appropriate and perform all required pretreatment dialysis machine alarm testing including Pressure Holding (PHT).

  • Initiate Solution Delivery System (SDS) system.

RECORD KEEPING:

  • Complete and document ongoing participation in Continuous Quality Improvement (CQI) activities.  

  • Enters all treatment data into the designated clinical application in an accurate and timely manner.   

  • Review treatment sheets for completeness, ensure nursing signatures are documented, and ensure

omitted entries are completed or corrected by appropriate staff.

  • Prepare initial patient chart for admission and compile paperwork for appropriate placement in chart.

  • Prepare/print lab requisitions for prescribed or stat Lab specimens according to laboratory destination.   

  • Collect, label, appropriately prepare and store lab samples according required laboratory specifications.   

  • Ensure collection of lab specimens by appropriate lab courier.

   

INFECTION CONTROL:

  • Assists in collecting  information for infection control audits.   

  • Supports staff and patient Adherence to infection control practices.   

  • Follows infection control Policies and Procedures   

  • Participates in and reinforces infection control education of patients and families under the direction of the CM or designee as allowed by state law.

   

TRANSITIONAL CARE UNIT:

  • Follows all newly admitted patients through their first 4 weeks of dialysis and coordinating their transitional dialysis care 

  • Reinforces all education and care related matters as it relates to the new patient     as allowed by state law

  • Liaisons with appropriate FKC staff members to provide the best patient experience including making sure all disciplines are scheduled to see patient according to plan.   

  • Assists in assuring patient understanding or home dialysis products, benefits of home and how home dialysis can work for them.

  • Sets and preforms a treatment on all machines used in the transitional care unit.

  • Assist in assuring patient understanding of treatment options including demonstration of the peritoneal dialysis set-up.

  • Assists with coordination of transition to patient’s modality choice.   

  • Assists with supply ordering and inventory for TCU

The employee may occasionally be required to push and/or pull equipment, exerting up to 15 pounds of force. The employee may be required to lift and to lower solutions on a frequent basis of up to 30 lbs., and on an occasional lift basis up to 40 lbs., as high as 5 feet. There is a two-person assist program and "material assist” devices for the heavier items.

    EDUCATION AND LICENSES:

    • High School diploma or G.E.D. required.

    Agency Summary:
    Applewood Centers is a private, nonprofit agency providing mental health services, social services and education to children, adolescents and families living in Cuyahoga and Lorain counties. Dedicated to addressing a variety of behavioral, emotional and learning problems, we provide a continuum of care from outpatient counseling to residential treatment.

    Position Summary:

    Under the administrative and clinical supervision of the GTS Program Coordinator & the Director of Community Services, the GTS Clinical Supervisor is responsible for clinical oversight of the GTS groups including providing clinical and administrative supervision and training to assigned staff members, as well as assisting with monitoring assigned budget.

    Essential Duties:
    1. Provide Clinical & Administrative Supervision: Provide clinical and administrative supervision to GTS Licensed Group Facilitators and TBS duties for Youth Specialists (and other program staff as assigned) during team meetings and individual supervision.
    2. Program Growth & Development: Expand and grow service deliveries to increase referral sources and revenue-generating client base.
    3. Budget Management: Collaborate with the Program Coordinator to maintain and manage monthly and yearly budget requirements for the program.
    4. Case Management & Referrals: Screen and assign appropriate cases to groups, track all referrals made to the program, and manage waitlists.
    5. Collaboration & Communication: Collaborate with parents, case managers, school personnel, and other professionals to facilitate clients' treatment progress and address any obstacles or concerns.
    6. Compliance & Standards: Ensure that standards/regulations set by accrediting and/or licensing bodies are maintained by staff.
    7. Staff Development: Monitor and facilitate staff development of skills in service delivery.
    8. Documentation & Training: Train and develop staff in documentation skills and monitor adherence to agency policies and procedures.
    9. Clinical Documentation & Compliance: Monitor and ensure staff compliance with required timelines on the completion of clinical documents, case summaries, outcomes, or other program requirements in accordance with applicable licensing and accreditation regulations and standards.
    10. Performance Evaluations: Maintain supervision records throughout the year for complete and accurate performance evaluations for each assigned staff member.
    11. Curriculum Review & Approval: Review and approve all proposed group curricula, ensuring they are based on individual client treatment goals and supported by treatment plans.
    12. Program Oversight: Collaborate with the Program Coordinator to provide oversight of day-to-day group programming, including ensuring the safety of clients and staff.
    13. Program Census Management: Monitor daily, weekly, and monthly program census, track individual group census, and develop plans with both group staff and management to maintain or improve census within the program.
    14. Quality Improvement (QI/QA): Complete QI/QA reviews for clinical services, monitor QI/QA results for assigned staff, develop plans of correction as needed, and follow up to ensure corrections are completed.
    15. Collaboration & Project Development: Collaborate with other supervisory and administrative staff on projects and the development and implementation of solutions for effective care delivery.
    16. Community Advocacy: Perform advocacy and liaison work with schools, the juvenile justice system, social services, and other community resources as needed.
    17. Staff Recruitment & Selection: Participate in the interviewing and selection of candidates for clinical positions within the agency.
    18. Program Change & Improvement: Collaborate with agency staff and administration on behalf of program changes.
    19. Professional Development: Apply professional training to revise and update supervisory and clinical practice.
    20. Attendance & Punctuality: Maintain a good on-time attendance record.
    21. Committee Participation: Participate in Wingspan Committees as appropriate.
    22. Other Duties as Assigned: Perform other duties as assigned.

    Other Duties:
    1. Attend Meetings & Trainings: Attend scheduled staff meetings, supervision, and on-going trainings.
    2. Maintain Certifications & Licensure: All required trainings, certifications, and licensure must be kept current in accordance with applicable licensing and accreditation regulations and standards.
    3. Client Confidentiality: Respect the privacy of clients and hold in confidence all information obtained during the client's treatment. All client-related documents should be handled in accordance with Agency guidelines on confidential material.
    4. Ethical Conduct: Assure that staff maintain high standards of ethical and professional conduct and adhere to Agency policies and procedures.
    5. Other Duties as Assigned: Other duties as assigned by management.

    Qualifications:
    1. Education: Minimum Master's Degree in Social Work, Counseling, Marriage and Family Therapy, or related field.
    2. Licensure: Minimum Valid Ohio LISW, LPCC, or LIMFT required.

    4. Experience: Minimum of four (4) years' experience in a mental health setting with a minimum of one (1) year supervisory experience.
    5. Other: Must have and maintain a valid driver's license and driving record that meets the underwriting criteria of the Agency's insurance company. Must be approved by Applewood to transport clients in own vehicle.
