Under direct supervision the incumbent functions independently in performing the nursing process through assessment, planning, implementation and evaluation of safe therapeutic care for patient overt and covert needs and/or complications of treatment in the patient's home. Incumbent is also responsible for providing assistance to physicians and other members of the team in coordination of services and resources for high risk and chronic care patients. To provide education and supportive intervention aimed at increasing patient knowledge and confidence in managing their health as well as to provide coordination of services across the continuum.
