The anticipated base compensation range for this position is 250,000 - 300,000, depending upon the applicant's qualifications and experience. In addition to the base compensation, this position may also be eligible for a productivity bonus, sign-on bonus, loan repayment assistance, catalyst training stipend, and relocation bonus, equaling total target cash compensation that may be between 275,000 - 450,000.

Effective weight management involves caring for the whole person. In Weight Management, patients receive care from a dedicated team of experts who are passionate about helping people achieve a healthier lifestyle. Our team includes pediatricians, nurses and nurse practitioners, registered dietitians, exercise physiologists, pediatric psychologists, education coordinators and social workers. We partner with families, children, and teens to create actionable plans to help them achieve and maintain a healthier weight.
The nurses in Weight Management staff both the clinic and phone nursing roles, including patient and family education, medication administration, DME/Prior Authorization management and managing forms and Patient Portal communication.
LPNs provides nursing care under the direction of a Registered Nurse or physician. Participates in the development and review of care management plans and in patient/family education.

- Currently using EPIC for all patient interactions.

Qualifications
- Medical degree from an accredited institution.
- Board certification in Family or Internal Medicine
- Preference will be given to candidates certified in obesity medicine (ABOM)

- Current state medical license and DEA registration.

**JOB SUMMARY**

A Brief Overview
Provides health assessment, nutrition, weight management and behavior modification education for individuals, corporations and health plans. Assists in training, marketing and growing the Bariatric program. Primary role is to serve as weight management and nutrition education Health Educator for Bariatric Program patients.
What you will do

Utilizes established coaching guidelines and protocols including those from specified weight management programs.

Assists in providing related health promotion activities.

Assess, develop, and implement wellness and weight management programs for Bariatric surgery and non-surgical weight loss patients.

Communicate effectively with the client, family, physician, and other team members regarding client status, progress, and needs.

Prepares written documentation as required including checklists, screening forms, client file documentation, EMR as applicable and other outcome/quality reports.

Assist in reviewing of: 1) research, 2) patient educational materials, and 3) department protocols to ensure best practice intervention delivery.

Prepares and facilitates programming seminars including: intake of participants, presentation, and 1:1 coaching.

Responsible for program content, development and training while supporting health coaches in effective program delivery. Maintains clinical and behavioral competencies of Certified Health Coaches.

Evaluate quality and effectiveness of programs and services on a regular basis for improvement, observation, comparing and monitoring data to determine compliance and efficacy of determined interventions.

Facilitate program scheduling and work with health coaches to cover shifts appropriately.
