Blue Sky MD utilizes a team consisting of Medical Doctors, Physician Assistants, Family Nurse Practitioners, and other Wellness Coaches and Medical Assistants with varying graduate and post-graduate degrees. The offering of services at Blue Sky MD embodies a preventive approach to assist patients with chronic medical issues due to weight or inadequate hormones.
Wellness Coach Job Summary:
The Blue Sky MD Wellness Coach is a core role within the organization and is integral to the support and care of all or our patients. The Wellness Coach position requires communication skills and a personality conducive to a fast-paced environment, as well as the willingness to make a difference in the health and appearance of patients.
Wellness Coach Responsibilities:
The Blue Sky MD Wellness Coach position will have the following job responsibilities:
· Conduct initial new patient evaluations and motivational follow-up visits
· Regularly communicate the medical, physiological, and nutritional aspects of the Blue Sky MD program
· Facilitate workflow for patient and practice efficiency, including but not limited to scheduling patients for follow-up visits
· Triage patients on occasion with biometric scale and communicate medical triage needs to Medical Assistant.
· Maintain awareness of new trend information in terms of nutrition, exercise, and healthy lifestyles.
· Communicate and elaborate with on sight provider supervisor for medical needs for patient care.
· Maintain Monthly Inventory with supplements, program materials etc.
· Manage Weekly Patient Outcome Letters for local PCP’s on patient Progress.
· Maintain Schedule Preparation two days out, along with Provider/ Medical Assistant Daily Huddle Schedules.
· Conduct inventory of applicable products and reorder and stock as needed
· Other duties may be assigned according to business needs
Education Required:
Minimum: Bachelor’s Degree in health and Wellness, Exercise Physiology, or related field
Preferred: Master’s Degree in Health and Wellness, Exercise Physiology, or related field
Experience Required :
· 3 years in a Primary Care, Dermatology, Urgent Care and/or Emergency Care setting.
· Experience with typing and computer applications, including Electronic Medical Records.
· Excellent grammatical skills, attention to detail, and ability to multitask also important.

The Wellness Coach will assist consumers with accessing needed community services (medical, psychological, substance use, vocational, food, clothing, etc.) through the development of a referral network, referrals, and follow up.
Essential Duties & Responsibilities:
- Provide case management to assigned caseload
- Develop and monitor individual service plans in conjunction with consumers
- Assist consumers with problem solving and crisis intervention
- Accompany consumers on medical, psychiatric, and public benefit appointments, as necessary
- Act as advocate for consumers as appropriate; encourage and support self-advocacy

- Assist with moves with consumers and staff; may include lifting, packing, and cleaning
- Work as a member of the social service team to develop off-site recreation, socialization, educational, and skill-building activities to promote community inclusion and integration
- Serve as liaison to community-based services and groups, including civic, social service, government, and business organizations
- Participate in ongoing individual and group supervision, team meetings, case conferences, division meetings, and trainings
- Adhere to on-call system to facilitate crisis intervention, emergencies, and other situations that require immediate action
- Perform other duties as assigned
Qualifications
Required Education and Experience
- Bachelor’s degree in Social Work or a related human services field with two years of relevant experience, or equivalent background
- Minimum 2 years’ experience working with individuals with mental illness, substance use issues, and/or a history of homelessness, or 2 years in the human services field
- Experience in group work, tenant organization, case management, referral, and/or community outreach strongly preferred
- Experience or training in peer counseling or advocacy preferred
- Proficiency in common computer programs, including email, Microsoft Word, Excel, PowerPoint, and Windows
Preferred Qualifications and Skills

GENERAL PURPOSE OF JOB
Under the direction of the HCM, the Wellness Coach will coordinate and deliver health promotion activities. This position will provide outstanding service to program participants to maximize participation, while facilitating behavior change, maximizing engagement and outcomes, and customer satisfaction.
KEY RESPONSIBILITIES
+ Provide appropriate health outreach and wellness coaching via face to face, telephone, or telehealth technology to reduce or eliminate high-risk behaviors; encourages active adoption of habits that are conducive to a higher quality of life.
+ Utilizes coaching methodologies and communication skills to solicit change and/or increase a patient's desire to change behavior and adopt a healthier lifestyle; documents client activities and coaching session notes in established clinical format using EMR technology.
+ May develop and implement assessment plans to determine the population's health and lifestyle needs; assemble data to assist with designing an Annual Wellness Work Plan.
+ Deliver total population health risk management programs/services, while using corporate culture change strategies defined through employer Health Promotion standards/best practice. May assist with developing these programs.
+ Provide services and programs specific to QuadMed's strategic goals, employee health risks and interests, and collaborate with internal and external resources to provide a comprehensive plan of care with appropriately defined interventions.
+ Serve as the key resource for engaging employee awareness and participation in Wellness and Health Improvement programs.
+ Proactively contribute to and/or lead wellness committee meetings while acquiring program buy-in and support through all levels of the organization.
+ Coordinates and delivers fitness and wellness programs and services, including but not limited to health awareness, educational campaigns, behavior change programs, group fitness classes (combination of cardio, weight training, mind/body), and physical fitness services (personal training, program design, fitness assessments and orientations).
JOB REQUIREMENTS
Education:
+ Bachelor's Degree from an accredited college or university in health education, health promotion, nursing, nutrition, exercise science or another related field
Experience:
+ Minimum one (1) year of experience in the wellness industry, to include experience in the delivery and coordination of corporate based health promotions programs, one-on-one health coaching and/or personal training
Certificates, Licenses, Registrations:
+ Current CPR (BLS) certification or must be obtained within 60 days of hire
+ CHES (Certified Health Education Specialist) certification preferred.
+ Health coaching certification from National Board for Health and Wellness Coaching (NBHWC) Approved Program or must obtain within one year of hire date.
+ Fitness and physical activity focused positions require discipline specific certification, as applicable. Examples may include
+ Personal Trainer certification or must be willing to acquire within 3-6 months of employment.
+ Group Fitness certification or must be willing to acquire within 6 months of employment.
Knowledge, Skills & Abilities:
+ Skills in program development, implementation, marketing, and promotion.
+ Strong interpersonal communication and customer service skills with the ability to motivate others.
+ Ability to make recommendations to effectively solve problems, using judgment consistent with standards, practices, policies, and procedures.
+ The ability to work both independently and as a part of a team with enthusiasm, initiative, and creativity.
+ Ability to apply critical thinking skills and keen attention to detail
