Job Summary:

Chair or participate on committees such as Breastfeeding Task Force, Regional Controlled Substance Abuse Committee, etc.

**Summary**
Hayward Wellness Center, part of Alameda Health System, is currently recruiting for a Full Time Advanced Practice Provider (Physician Assistant (PA) or Nurse Practitioner (NP)) to join their team. This APP will be an adult primary care provider responsible for a panel of 1000 patients which corresponds with the 1.0 FTE position.
Hayward Wellness Center, part of Alameda Health System, is an outpatient center that serves a diverse patient population in Hayward, CA. As a provider of care to all patients, regardless of their ability to pay for services, we provide quality, accessible care to our community's most vulnerable patients. Hayward Wellness Center offers full scope adult, pediatric, primary care, women's health services, including gynecology, family planning, obstetric, and prenatal care. **SUMMARY** : Under general direction, assesses, evaluates and manages the health care needs of patients. Performs selected diagnostic and therapeutic tasks. Newark Wellness Center, part of Alameda Health System, is an outpatient center that serves a diverse patient population in Newark, CA. **SUMMARY** : Under general direction, assesses, evaluates and manages the health care needs of patients. Performs selected diagnostic and therapeutic tasks.
