Work Shifts

The Patient Care Associate, under the direction of a licensed nurse or other medical staff, in the health care setting assists the professional staff by performing, veni-puncture, finger-sticks, HIV Pre and Post Test Counseling, Rapid HIV Testing, responding and assisting with emergencies, facilitating telehealth encounters, various clerical and health-related duties associated with patient care (ADLs as indicated) and contributing to the safety, care and comfort of patients. The duties are assigned by and are performed under the supervision of a Registered Nurse. The Patient Care Associate may be required to perform the duties listed below and other duties involving same type, area of responsibility. Patient care associates also observe their patient's mental, emotional and physical conditions and report any changes to the nursing staff.
Responsibilities:
1. Facilitate Telehealth encounters as needed
2. Administer total care (bed bath, partial or complete)
3. Assist with patient's personal hygiene - including skin care, mouth care,
shaving, etc.
4. Transfers patients - in and out of bed, stretcher and wheel chair. Including
physical and mechanical lifting
5. Positions patients and carry out range of motion exercise
6. Assists with ambulating and all ADL as directed
7. Prepares patients for meals and assist with feeding as needed
8. Assists with unit admission and transfer of patient
9. Makes beds and tidies patient area for bed ridden patients
10. Takes and document in the electronic medical records all patient's vital signs -
B/P, TPR, weights, intake and output and performs EKGs as required and follow
outlined procedures for submitting abnormal EKGs
11. Administers hot and cold applications to patients, under supervision
12. Maintains intake and output for oral and body excretions
13. Notify MD/Nurse Staff of any changes in patient's status
14. Performs urine testing by dipstick and fractionalization
15. Chaperon staff in all clinical areas when requested
16. Maintains competency in nursing skills as detailed in the clinical
Performance checklist for Nursing Assistant
17. Assist with Nursing Follow-ups and transmit necessary information to
laboratory electronically.
18. Anticipates and acts to maintain a safe environment
19. Performs job functions in a manner that promotes efficient use of resources
20. Must wait for relief at change of tour by staff with same title (relieving PCA)
21. Document all encounters in Electronic Medical Records
22. Complete Encounter forms every shift
23. Performs dry sterile dressing changes and assists with other patient

3. Maintain cleanliness of surface area of treatment and exam rooms
4. Maintain security precautions with keys, locked areas, and equipment
Human Relations Skills:
1. Assists other employees where needed.
2. Is responsible and cooperative with supervisors, fellow associates, patients,
visitors, and Department of Correction personnel.

Working Conditions:
Because patents require round the clock care, working hours include days, nights
weekends and holidays. PCAS are required to stand for long periods of time.
Employees are encouraged to safely move patents and use alternative methods
whenever possible; alternative methods include requesting assistance to roll/lift/
transfer patients. PCAs may face hazards from exposure to chemical and infectious
diseases. In addition, they have to treat patients that are confused, irrational, agitated
and/or uncooperative
Physical Demands
1. Ability to lift fifty (50) pounds. Moving, lifting or transferring of patients may
involve lifting of up to 100 pounds
2. Ability to stand for extended periods
3. Fine motor skills
4. Visual acuity
Minimum Qualifications
Qualification Requirements:
1. High school diploma or its educational equivalent, approved by a State's Department of Education or a recognized accrediting organization; and
2. Successful completion of training courses including the performance of phlebotomy and
electrocardiography (EKG), or completion of a training program or programs that include the performance of phlebotomy and EKG, such as Phlebotomy Technician and EKG Technician training programs, a Patient Care training program, or a Medical Assistant training program; and
3. Holds, or obtains through facility orientation, a valid and current certification in Basic Life Support (BLS) through the American Heart Association (AHA).
Special Note:
In addition to meeting the qualifications above, assignment to a long-term care facility requires New York State Nurse's Aide certification.

Since 2016, Correctional Health Services (CHS) has been the direct provider of health care in the New York City jails. Deeply committed to human dignity and patient rights, CHS is part of the NYC Health + Hospitals system and is a key partner in the City's efforts to reform the criminal-legal system. Our in-jail clinical services include medical, nursing, and mental health care; pharmacy services; substance-use treatment; social work; dental and vision care; discharge planning; and reentry support.
Given the high visibility of this initiative, we are seeking the highest caliber health care professionals in key clinical services to staff our Outposted Therapeutic Housing Units (OTxHU). To be located in three NYC Health + Hospital acute care facilities, the OTxHU is a pioneering approach to safely increasing access to high quality clinical care for patients in custody who have complicated health conditions. OTxHUs will bridge the gap in the continuum between care provided in the jails and inpatient hospitalization, with admission to and discharge from the OTxHU in accordance with a patient's clinical needs. CHS will be the primary health care providers on these units and the NYC Department of Correction will provide security and custody management.
The OTxHU at NYC Health + Hospitals/Bellevue in Manhattan will be the first of this unique, groundbreaking project to open with a planned completion date as early as the end of 2024. This is an incredible opportunity to be part of a passionate and motivated team providing care to some of the City's most marginalized, vulnerable people.
*To help support continuity of operations and care, stff selected to work in the OTxHU may also be required to work in CHS locations within the jails. Position Overview

- Oversee two or more nurses including interviewing, hiring, and scheduling while ensuring compliance with regulatory requirements
- Monitor the health and medical condition of individuals served
- Manage the administration of medications, performance of physicals, provision of treatments and scheduling of medical appointments
- Evaluate nursing care, observe nursing activities, and inspect patient facilities for cleanliness and comfort
- Plan and organize orientation and in-service training for staff members
Qualifications:
- Graduate of an accredited nursing school or nursing program, with three years of related experience (when supervising RNs), and with a current RN license
- Associate’s degree from a two-year college or technical school, with two years of related experience (when supervising LPNs/LVNs), and with a current LPN or LVN license
- Strong, leadership qualities, attention to detail, and organizational skills
- Ability to multi-task and meet deadlines
- Effective communication skills to manage relationships with vendors and employees
- A reliable, responsible attitude and a compassionate approach
