Position Summary:
The Wraparound Care Coordinator provides home-based services to assist youth and families within the Erie County System of Care and who are receiving services through Erie County Department of Social Services. Coordination is provided through strength-based, individualized service planning. Linkages, coordination, and monitoring of services to improve and promote family stability are the focus of service.
Major Responsibilities/Activities:
- Provides home based services to families in Erie County utilizing the Wraparound Care Coordination model
- Utilizes a strengths based family centered approach to services
- Partners with children, parents, guardians and service providers to create a comprehensive Plan of Care and identify any additional services the family may benefit from
- Conducts assessments throughout duration of the case
- Provides education, care coordination, and community linkages as appropriate
- Conducts face to face contacts with families
- Conducts Child and Family Team meetings each month
- Participates in trainings required by stakeholders and agency
- Maintains timely and accurate documentation in two systems
- Other duties as deemed appropriate
Competencies:
- Solid writing and verbal communication
- Strong engagement skills and ability to engage with diverse populations
- Flexibility related to scheduling
- Familiarity with computer applications (i.e. Word, Excel, Outlook)
- Familiarity with Fidelity EHR and Connections a plus
- Bi-lingual ability a plus
Minimum Requirements:
- Bachelor’s Degree or Master’s Degree in a human services related field plus 1 year experience in a professional or internship human services setting. *Experience must be providing direct care services or linkage services to at risk youth/children and families.
- Valid NYS Driver’s License and adequate auto insurance
- Ability to work effectively with clients, families, staff and community contacts from a variety of cultural and ethnic backgrounds.
Hours: Flexible schedule based on clients' needs.

Job Responsibilities: CARE COORDINATOR WRAPAROUND PROGRAM Wisconsin Community ServicesPosition Summary: Working under the Wraparound Practice Model of care, which includes a unique set of values that emphasizes individualized, strength-based, trauma-informed, community-based and culturally intelligent care, provide intensive and individualized care coordination for children and adolescents.Essential Functions:1. Maintains a caseload of approximately 8 children and families.2. Contact enrolled children and families within 48 hours of enrollment, and set up initial visit to occur within 5 business days of enrollment to introduce self and supports available.3. Schedule and attend weekly visits with assigned children and families, facilitate monthly Child and Family Team Meetings to discuss treatment planning, and maintain regular phone contact with assigned children and families and service providers as needed.4. Assemble a Child and Family Team within two (2) weeks of enrollment by interviewing the family, identifying family members/natural supports/service providers and other significant persons.5. Within the first 30 days, coordinates the meeting of the Child and Family Team and develops the initial individualized Plan of Care based on the identified strengths and needs, including a comprehensive 24-hour Crisis Plan. 6. Monitor the provision and quality of services provided to the family through the Child and Family Team and is the liaison when new services/resources need to be sought or developed. 7. Advocate for Children and Families across a variety of settings, including home, educational, court, and community settings, as needed.8. Attend all scheduled court hearings as a representative of Wraparound Milwaukee to support children and families throughout the court process and explain supports available/in place.9. Seek community resources with the assistance of the Team and modify the SAR whenever services or resources need to be added and/or deleted.10. Provide or arrange for transportation for youth/young adults to appointments, crisis/respite services, including assisting a child and family with identifying natural supports and sustainable transportation plans, etc., if needed. 11. Provide or secure support and crisis/emergency services for youths enrolled and/or their families. This may be done through face-to-face contact, phone contact/ availability or an on-call system. 12. Completes all necessary paperwork in a strength-based manner per Wraparound Milwaukee/Agency requirements,(i.e., court letters, Plans of Care, change of placement forms, SARs, Referrals, Progress Notes, evaluation instruments, consent forms, etc.).13. Maintain accurate information online, ensuring that family demographic information and Child and Family Team Member contact information is up to date.14. Collaborate with other necessary individuals the youth and family may have contact with. 15. Keep Wraparound Milwaukee informed of all emergencies or critical incidents (refer to Wraparound Policy #14 - Critical Incident Reporting).Other Duties and Responsibilities:1. Attend in-services and participate in staffings, weekly/monthly meetings, and consultations, as needed.2. Assist with child and family coverage for co-workers as needed or directed.3. Reassess the child/youth using the CANS every three months. Use the CANS outcomes as part of the Plan of Care. Document client progress to maintain a permanent record of client activity according to established policies and procedures.Assume primary responsibility for developing, writing, implementing, evaluating, and revising overall treatment goals and plans in collaboration with the treatment team.Educate and support clients' families and advocate for clients' rights and preferences. **Responsibilities**

**Minimum Qualifications**
+ **Bachelor's Degree in Social Work or comparable Human Services field from an accredited institution.**
+ **2 years of work experience working in direct service with youth and families strongly preferred.**
+ Must have flexible availability as needed.
+ Must have a valid driver's license, reliable transportation, automotive insurance, and a good driving record.
+ Must have car insurance requirements of 100,000/300,000 bodily injury insurance.
+ A willingness to work flexible and non-traditional hours in the service of families of origin, and persons served.

Head Start - Family Services Associate
Location: South Los Angeles
What We're Looking For:
We are seeking a Family Services Associate who supports Head Start families in making choices and decisions, and taking actions that will improve the conditions and quality of life for all family members; creates opportunities for meaningful family engagement with Head Start for all families, uses a case management approach to share information with families regarding resources, program options, sources of professional and parental support, and ways of accessing resources within the community; recruits Head Start children, and links families with services such as health, disabilities, nutrition, social services/mental health, and family engagement services. The ideal candidate will support and coordinate program services in the following areas:
- Implements the family needs assessment process, consolidates the results, develops appropriate recommendations in response to the expressed needs of families.
- Networks with key community organizations and agencies to provide parent training in developing life skills, including parenting, income generation/entrepreneurship, improvement of communication skills, literacy, mental and physical health care, financial planning, and resolving conflicts or violence.
- Recommends specific procedures for responding to emergency needs of families, works with existing community agencies and resources to design and implement an effective emergency needs response system.
- Identifies and explores cooperative relationships community resources and agencies that provide employment/job skills training, job placement, on-the-job training opportunities, apprenticeships, and job preparation activities.
- Recruits prospective families and assists with enrollment process.
- Conducts home visits to facilitate family plans, assists families in achieving identified goals and follow-up on referrals. Acts as a case manager and works cooperatively with local agencies to assist families to linkup with services in the local community.
- Promotes effective community support for families by coordination and advocacy for services with community agencies.
- Assists families in making the transition into and out of the Early Head Start/Head Start Program and into Kindergarten.
EXPERIENCE, KNOWLEDGE, SKILLS AND ABILITIES YOU SHOULD POSSESS:
- BA/BS degree or AA/AS degree in social work, human services, liberal studies or related field required. Bilingual in English/Spanish is a plus.
- Expert in Microsoft Office computer skills required.
- Successfully completes and maintains Basic First Aid and CPR certification.
- Knowledge of Child Care Licensing Regulations and Head Start Performance Standards and best practices related to education and early childhood development, staffing and program options as well as services to children with disabilities.
- Knowledge of social service delivery, community resources, requirements, and procedures.
- Knowledge of family dynamics, strategies for male involvement and family-centered practice.
- Knowledge of the immediate business communities and private industries
- Experience in community outreach, community health or family-focused intervention.
- Experience working or volunteering in social or community services required.
- Experience in planning and conducting workshops and training programs.
- Experience working in a multi-cultural and multi-ethnic setting.
SPECIAL REQUIREMENTS:
- Must possess a valid California driver's license, have use of a motor vehicle and provide evidence of automobile liability insurance.
- Must meet State health requirements, including TB clearance.
- Fingerprints must be on file with the Justice Department.
