- Meet all clinical obligations of the medical practice: Accrediting Agency compliance, CLIA compliance, drug prescription compliance, HIPAA, State and Federal regulations, Safety Regulations, etc.
- Independently provide care for patients with acute and chronic illness encountered in the older adult patient
- Accountable for patient care and outcomes and will appropriately seek consultation from specialists when needed, though will stay involved in, and be responsible for, the detailed care of the patient
- Engage with hospitalists whenever a patient is in the hospital
- Assess, diagnose, treat, manage, educate, promote health, and provide care coordination and documentation for patients with acute and complex chronic health needs using evidence-based medicine and practice guidelines
- Maintain complete and detailed records, electronically in the EMR, of patients health care plans and prognosis, completed within 48 hours of each patient visit
- Provide quality evidence-based medical care in a managed care environment, focused on prevention, and outcomes
- Exhibit behavior that emphasizes compassion, quality, loyalty, and conscientiousness
- Interpret data and diagnostic results for an appropriate course of action including but not limited to blood gases, radiology, EKG, etc. Ability to establish and maintain effective working relationships with patients, their families, and team members
- Oversee and supervise Nurse Practitioners and/or Physician Assistants when requested
- Responsible for ensuring that patients are seen at least every other visit by a Physician
- Provide 5-Star service in accordance with Better Health Group's mission

- Medical Degree from an accredited medical school
- Active medical license in the state of practice
- At least 3 years of experience in outpatient primary care experience
- ACLS/BLS certification is required as mandated by state of practice

- Ability to diagnose and manage chronic diseases such as diabetes, atherosclerotic disease, COPD, CHF, obesity, etc. in the senior population
