BAYADA Home Health Care is seeking a Physical Therapist (PT) for a full time position with our new Central NJ Senior Living team - a Medicare Certified home health office.
This office provides best-in-class care to adult and geriatric clients living in communities that include assisted living, independent living, senior congregate living, 55+ communities, and intermediate care facilities for clients with intellectual disabilities throughout Monmouth County .
*This is a hybrid position that may also include seeing clients in the traditional home health community setting.
Responsibilities:
- Make home visits to clients in designated geographic territories.
- Perform diagnostic tests and measurements, such as the mobility/range of joints, transfer status, stability, patterns and appearance of ambulation, strength and endurance of muscles, balance testing, and safety assessments.
- Implement integumentary protective interventions to prevent wound formation and promote wound repair utilizing evidence-based dressings and topical agents.
- Develop and implement appropriate individualized care plans, including manual therapeutic exercises, gait training, balance, and other interventions.
- Continually assess and revise the Physical Therapy - PT care plan, and participate with nursing in the multidisciplinary care plan, as appropriate.
- Educate and instruct clients, family members, or other client representatives, in rehabilitative care and activities necessary to promote the client's health, safety, and independent living.
- Accurately document observations, interventions, and evaluations pertaining to client care management and services provided, utilizing a state-of-the-art touch pad tablet.
Qualifications:
- A current NJ Physical Therapist - PT license.
- A minimum of one year of recent work experience as a Physical Therapist - PT.
- Graduation from a program approved by the American Physical Therapy Association or the Committee on Allied Health Education and Accreditation of the American Medical Association, as indicated by school transcript or diploma.
- Ability to work independently and manage time effectively.
- Strong interpersonal, organizational, and problem solving skills.
- Solid computer skills; prior experience with electronic medical records (EMR) preferred.

Occupational Therapist, OT Responsibilities:
- Perform outpatient therapy evaluations in order to develop a plan of care for patients in designated senior living communities or in a group home setting for adults with intellectual disabilities
- Professionally operate at top of licensure utilizing current research-based approaches to care in order to maximize functional independence for your clients

Occupational Therapist, OT Qualifications:
- A current NJ Occupational Therapist license.
- Prior home care experience preferred, but not required

Your impactful journey involves:

- Tracking and documenting patient performance, progress, and response to treatment.

- Perform 5-6 visits per day in a small geographical territory, focusing on 1:1 care with each patient
- Deliver 1:1 skilled nursing, intervention, and supportive care in accordance with individual care plan, including administration of medication, wound care, treatments, and comfort care
- Perform client assessments, provide education to the patient and/or their family, and monitor patients' condition
Qualifications for a BAYADA nurse:
- A current NJ nursing license in good standing
- One year of clinical experience strongly preferred.
- Previous home healthcare experience welcomed, but not required.
