The Engagement Specialist II will serve an integral role in patient engagement. The Engagement Specialist II will be responsible for telephonic outreach to Somatus members to enroll them in the program and facilitating their initial interaction with the care team. This role will heavily support researching new member contact information for hard to reach members and engage and re-engage attributed members and assisting care teams with scheduling assessments. This role is a fully remote
position.
Responsibilities
- Utilizing problem solving, advanced tools, and databases to research new member contact
information to engage and re-engage hard to reach members.
- Tools may include contacting outside sources to obtain member information, outreach will include but not limited to providers, pharmacies, home health agencies, EMR's, HIE's, hospitals and community agencies.
- Provide mentoring and training support for Engagement Specialist I via Teams Chat, Shadowing,
or other methods as needed.
- Support Engagement Specialist Supervisor as needed via special projects.
- Outreach to members to introduce the Somatus program and encourage enrollment. During
the enrollment process complete the scheduling of their initial visit with the care team.
- Manage a daily queue of members to be outreached. Performance will be measured by number of calls completed and members enrolled in the program.
- Quality documentation of interaction with members will be expected. Quality will be measured by correctly following the process and the level of detail in the notes.
- Ability to perform in a fast-paced environment while still completing tasks with a high level of accuracy.
- Consistently presents a professional, empathetic and energetic approach during all interactions with members. This position must be effective at quickly assessing situations and determining the most appropriate path is taken to address the members' needs.
- Establish relationships with field and central care teams allowing clinicians to work at the top of their license.

The Jr Outreach & Engagement Specialist serves as a technical advisor to support the Government and focuses on engaging with external stakeholders, promoting overhead systems knowledge, and ensuring effective communication of customer messaging and narratives. Collaborate with customers, users, and partners to understand their needs, gather feedback, and address concerns. Create and deliver presentations, organize workshops, and participate in conferences to share information about Govt systems and services. Provide and execute the procedural implementation guidance to conduct Joint Exercises through a common virtual environment and framework. This is a position of trust advising the US Government and supporting critical missions.

Specific responsibilities may include:
+ Perform strategic integration across multiple aspects of the program as well as multiple offices throughout the IC and DoD communities to assist with outreach and awareness
+ Guide Developers in Integration: Guide software applications developer and users in the integration and use of applications associated with program-specific hardware
+ Coordinate with the customer Front Office and other organizations throughout the enterprise to develop content for strategic executive-level presentations and documents, including technical, programmatic or graphics materials such as conference presentations, program overview and capabilities briefings

Basic Qualifications:
+ Bachelor's degree in a technical or business discipline is required and a minimum of 10 years of experience with components of the IC Community or 8 years with a Masters.

+ Excellent writing, presentation, and communication skills with experience presenting to senior executives and customers
+ Understanding of value drivers in recurring revenue business models and channels to market

Responsibilities
- Participates in care planning in collaboration with the patient and healthcare team to identify effective interventions that will help the patient meet rehabilitation, treatment goals, and improve quality of life.
- Assesses patient knowledge of kidney disease for barriers that may affect adherence to treatment.
- Assess social determinants of health that may impact treatment and assist with identifying community resources to address needs.
- Assist patient and/or family/caregiver in completing applications for services and/or resources.
- Provides ongoing education to patients/family regarding psychosocial issues related to chronic kidney disease (CKD) and End Stage Renal Disease (ESRD) and all support services that are available.

- Adjusts treatment plans in compliance with changes in the status of the patient.
- Provides information and assists the team and patient with referral to community resources (home health services, vocational rehabilitation, etc.) to facilitate optimal treatment outcomes.
- Provides supportive and goal directed counseling to patients who are seeking transplant.
- Assesses patient awareness of advance directives; assists with accessing advance directive forms/information and facilitates discussion of advance directive wishes, if necessary, with the healthcare team and the patient's family/support persons.
- In collaboration with the physician and nurse, participates in the discussion of patient DNR status in the facility to ensure patient understanding and informed decision making.
- Utilize motivational interview techniques to address non-adherence and quality of life concerns for all patients.
- Educate patients on their insurance options including Group Health Plans (GHPs), Medicare, Medicaid, and managed care plans. Serve as a liaison between patients, insurance companies, and the facility staff
- Assists patients in understanding their financial responsibilities, payment arrangements, and potential financial assistance programs
