Our client, a leading provider of comprehensive community and social services, is seeking a compassionate and experienced Senior Clinical Social Worker to join their team in Jacksonville, Florida, US . This role is dedicated to providing high-quality therapeutic interventions and support services to individuals and families facing various social and emotional challenges. You will conduct comprehensive psychosocial assessments, develop individualized treatment plans, and deliver evidence-based counseling and case management services. This position involves working collaboratively with other healthcare professionals, community resources, and families to ensure holistic client care and promote well-being. A portion of this role will involve remote work, allowing for flexible service delivery where appropriate, while also requiring in-person client interactions and team meetings.

Key Responsibilities:
  • Conduct comprehensive psychosocial assessments to identify client needs, strengths, and challenges.
  • Develop and implement individualized, evidence-based treatment plans in collaboration with clients and families.
  • Provide direct clinical services, including individual, group, and family therapy, utilizing various therapeutic modalities.
  • Offer crisis intervention and support services to clients experiencing acute distress.
  • Coordinate care with other healthcare providers, social service agencies, and community resources.
  • Maintain accurate and timely client records, documenting progress and interventions in accordance with agency standards and confidentiality regulations.
  • Advocate for clients' needs and connect them with appropriate community resources and support systems.
  • Participate in interdisciplinary team meetings to discuss client cases and treatment strategies.
  • Provide supervision and mentorship to junior social work staff and interns.
  • Stay current with professional development, research, and best practices in social work and mental health.

Qualifications:
  • Master's degree in Social Work (MSW) from an accredited institution.
  • Active LCSW (Licensed Clinical Social Worker) or equivalent state licensure is required.
  • Minimum of 5 years of post-MSW clinical experience in a social services or mental health setting.
  • Demonstrated expertise in diagnostic assessment, treatment planning, and therapeutic interventions.
  • Strong knowledge of community resources and social service systems.
  • Excellent interpersonal, communication, and active listening skills.
  • Ability to manage a caseload effectively and prioritize tasks in a dynamic environment.
  • Experience with electronic health records (EHR) systems.
  • Proficiency in utilizing telehealth platforms for remote service delivery where applicable.
