Schedule: Monday-Friday 8a-5p

**Key Job Responsibilities:**
+ Conducts a thorough individualized assessment of the person with or at risk for diabetes. Works with the person with or at risk for diabetes to identify mutally acceptable diabetes self management training (DSMT) outcomes and behavioral objectives.
+ Provides diabetes education according to the individualized plan of care and desired outcomes. Develops the self-management training plan to attain the mutually defined outcomes. The plan integrates current diabetes care practices and established principles of teaching and learning. The plan is coordinated among the healthcare team members, the participant, referring provider, and other relevant support systems.
+ Coordinates/provides individual/group education sessions for diabetic outpatients.
+ Provides leadership in maintaining and developing diabetic education program standards, policies, and procedures.
+ Maintains on-going evaluation of the diabetes education program.
+ Assists in Quality Improvement activities related to patients with diabetes.
**Qualifications**
**Minimum Qualifications:**
+ Current Registered Nurse (RN) license that allows you to practice in our state
+ Graduate of an accredited nursing program
+ Basic Life Support (BLS) from the American Heart Association required
+ Be certified as a Diabetes Educator or Board Certified in Advanced Diabetes Management with 39 months of hire

This position contributes to the fulfillment of CommonSpirit Health's mission and vision by planning, organizing, implementing and directing resources in assigned areas of centralized nursing education. This position contributes to the centralized education, training and development goals and objectives based on evaluation of organization and individual needs. Facilitates resources to deliver education, training and development programs. Under the leadership of the Market Director, works to ensure consistent and effective delivery of education programs that meet the evolving needs of the organization.
1. Authority to interview, hire, orient, terminate, promote, train and conduct performance evaluations.
2. Manage performance including: setting goals, clarifying job expectations, monitoring performance progress, providing feedback, recognizing, developing skills and addressing performance issues related to work and our commitments using corrective action.
3. Assigns day to day work activities; directs the functional and technical job performance of team members; professionally communicates with team members.
4. Provides support and leadership for projects and initiatives to support onboarding and clinical education and training across multiple markets. Supports nursing education throughout the region, providing direction in evidence based practice, research, and overall strategies.
5. Participates in interdisciplinary meetings and projects (e.g. with pharmacy, therapy, quality, etc.) to ensure best practice, exceptional care of patients across the Market(s).
6. Monitors and assesses current operations/identifies opportunities and provides recommendations to stakeholders for performance/process improvements initiatives; implements approved changes and ensures that staff receive the necessary training to perform in concert with performance improvement initiatives.
7. Interprets, and monitors compliance with, applicable internal/external legal and regulatory agreements, standards, requirements; keeps abreast of changing regulatory requirements/regulations and competitive industry practices impacting assigned functions, as well as organizational initiatives/changes with potential to impact current operations; monitors impact on existing activities and identifies/implements effective response/solution while maintaining acceptable service levels and work quality.
8. Supports academic mission through oversight of clinical student placements, partnerships, programs and collaboration throughout the markets,

10. Monitors operations of department productivity and budget in collaboration with the director.
11. Ensures optimal utilization of department resources within guidelines set forth through capital and operational budget planning.

**Qualifications**
Required Bachelor's degree in Nursing. Must also have Master's Degree in Nursing or related healthcare field.
Current license as RN in Nebraska, Iowa or multi-state compact license.
Basic Life Support is required upon hire. Acceptable credentialing bodies and certifications include the following: American Heart Association: Basic Life Support.
