Job Description
The Clinic Education Specialist is responsible for meeting the needs of internal and external Clinic patients, customers, and supporting the clinics by providing a variety of indirect and direct patient care services. Functions as an expert while working with market managers, providers and other clinic staff to manage and resolve complex situations. The Clinic Education Specialist is responsible for development and implementation of orientation, training, education and competency development of a variety of clinic roles in Primary Care and Specialty Clinics. The Clinic Education Specialist will utilize the HLC function to input and track competencies. Assists with protocol and policy development, and implementation of Joint Commission Standards. Serves as a Super User for LSS and acts as a liaison between MIS and staff to instruct as to the proper use of EMR for documentation. Collaborates in development of standardized clinic process and flow with clinic teams and Process Improvement Engineer. Remains responsive to new developments, and performs related duties as assigned. Serve as a role model in Clinical Support functions. Will function as Clinical Support as needed.
Additionally, you will:
- Responsible for ongoing training, new user set up and password resets.
- Provides support for the processes of registration, clinic scheduling to ensure work flow, efficiency and knowledge of the Clinical Assistant are maintained at the highest level possible.
- Provides orientation for scheduling, registration, EMR applications to new hires and ongoing.
- Collaborates with clinic team members to enhance the workflow utilization of scheduling and registration applications.
- Participates as needed in the interview process for all new Clinical Assistant and Clinical Support Candidates. Develops and monitors goals and objectives for continuous process improvements.
- Participates in the clinic department meetings on a regular basis.
- Leads and/or participates in special projects as requested.
- Responsible for training all WRMC clinic staff who Schedule, Pre-Register, and Register patients throughout the clinics.

- Current licensure as a Licensed Practical Nurse(LPN) or as a Certified Medical Assistant by the Wisconsin State Board of Nursing.
- Two (2) years experience in healthcare related field.
- Minimum three-years experience working as an LPN or CNA in a hospital or clinic setting. Registration and reception experience preferred.
- Three years related clinic experience, including time in a leadership role, preferred.
- CPR (AHA Healthcare Provider) certification required

In collaboration with multidisciplinary diabetes management team , provides diabetes education to patients and families living with diabetes mellitus by performing the following duties.

- Demonstrates advanced knowledge and skills in screening/assessing nutritional status of patients/clients seen for Diabetes Education and collaborate with patient to develop and implement a plan and document all pertinent information into the patient's electronic medical record using program related documentation requiring computer systems knowledge.
- Identified when a program participant's needs are outside the scope of the educator's practice/expertise and arranges for additional services to meet their needs. Works collaboratively with the multi-disciplinary education and treatment teams.
- Develops and implements care plans, coordinating services with diabetes management team and ancillary services.
- Train/educate patient's on blood glucose monitoring including/not limited to include continuous glucose sensor (CGMS) , insulin pumps and prescribed medication use including injectables.
- Represents the system and diabetes program as a diabetes education care resource person.
- Participates in ongoing clinic/program development to include policy/procedures, curriculum and quality initiatives as they relate to diabetes care, clinical development and efficiency.
- Enters appropriate charges into EPIC with strong attention to detail and accuracy.
- Provide information/education to staff members including patient diabetes education/medical nutrition therapy/program/materials development and on patient issues or needs identified.
- Maintains current knowledge and skills in specialty area of diabetes and chronic disease care. CEU/CME required for licensure/registration/certification. Attends mandatory in-service and completes online education as requested.
- Participate with program needs as requested- continuous quality improvement process to measure DSME program and identify and address opportunities for improvement. as assigned, peer review and be familiar with ADA program requirements.
- Knowledge of clinical practice guidelines, behavior change theory, motivational interviewing and Medicare/Medicaid guidelines.
- Understanding of community resources and needs of diverse populations.

Education and Experience
Graduate of an accredited nursing program.
BSN or upon hire BSN completion agreement for enrollment in an accredited BSN Completion Program May require specific years of applicable experience to qualify (per department guidelines). If hired prior to 1-1-14 in Winnebago County and prior to 9-1-14 in Rock, Walworth and McHenry Counties an ADN only is required.
Certification/LicensureLicensed as RN in states of WI and IL
Approved CPR certification required in 90 days (other certification per unit requirements: ACLS, PALS, PEARS, NRP)
CDCES certification required. If not CDCES certification eligible or not CDCES certified at hire, 15 CME credit per year required.
CDCES certification required within 2-years of becoming eligible or being hired.

+ Under direct supervision of a licensed pharmacist, counsels patients and answers their questions regarding usage of medicine, side effects, interactions, contraindications, patient information privacy, generics, less expensive medicines, over-the-counter products, and refers to medical provider as needed to ensure medication is taken correctly, health needs addressed, and satisfaction with service.
+ Performs pharmacist tasks including compounding, drug therapy reviews, verification, and medication management.
+ Reviews, interprets and accurately dispenses prescribed medications when necessitated by workload.
+ Ensures the pharmacy operates in accordance to regulations, company policies and standards. Assists pharmacy manager with establishing procedures that promotes the efficient workflow of the pharmacy including assigning roles, coordinating activities and soliciting employee suggestions. Responsible for the opening and closing of the pharmacy and shift change duties.
+ Ensures the use of all elements of the Good Faith Dispensing policy in conjunction with state and federal controlled substance laws when filling prescriptions. The Product Review/Retail Fill Process Pharmacist has the ultimate responsibility for ensuring that the elements of Good Faith are present.
+ Maintains current knowledge of information technology associated with pharmacy systems such as Intercom Plus, registers, automation, StoreNET to provide support to customers as well as pharmacy staff. Proposes and implements enhancements to pharmacy systems to further promote productivity.
+ Ensures the accurate processing of insurance claims to resolve customer issues and prevent payment rejections. Follows- up with insurance companies as well as medical providers and participates in 3rd party audit.
+ Follows-up with medical providers' offices to clarify prescribed medications, dosages, refills, interactions and allergies to suggest alternative medications, and answer medical provider questions.
+ Performs retail, clinical, and wellness services such as immunizations, flu shots, diabetes awareness and other preventive healthcare services.
+ Assists and supports the pharmacy manager in analyzing performance data including pharmacy financial, customer service, and implementation of procedures for pharmacy asset protection and inventory management. Reviews KPI's with Pharmacy Manager, Store Manager, and Healthcare Supervisor.
+ Develops and maintains good relationships with local medical community including physicians, nurses, and other health care providers. Participates in community outreach activities to promote the pharmacy business and further enhance growth opportunities.
+ Partners with Central Pharmacy Operations for customer registration, 3rd party reviews, and assists with resolving customer issues to support the Well Experience locations. In virtual environments, conducts virtual product review by following specific company procedures and guidelines (e.g. using specific photographs taken by the technicians sent electronically during the F4 process and reduce transactional tasks).
