Job Description
We are seeking a compassionate, experienced, and motivated Registered Dietitian (RD) to join our team as a Diabetes Care and Education Specialist (DCES). As a Patient Educator, you will assist with the overall coordinator of the diabetes education program, provide education and resources to physicians, caregivers and patients, facilitate the organizational change process by promoting and implementing services that achieve individual and organizational improvement, and provide quality service that results in increased effectiveness, efficiency and adaptability to meet and exceed expectations.
**A caregiver in this position works 8:00am - 4:30pm.**
A caregiver who excels in this role will:
+ Provide direct patient care using medical nutrition therapy or diabetes self-management education for assigned patient populations requiring nutrition and/or diabetes support in which medications and monitoring are an integral component in the design of therapy.
+ Collaborate with Endocrinology providers to design and administer nutrition and diabetes care and education that supports patient safety in the setting of high-risk medication use.
+ Complete comprehensive assessments for each patient including emotional and behavioral health, knowledge level, disease history, desired outcomes, and electronic medical record data.
+ Develop an individualized care plan with safe and effective interventions based on the patient' assessed needs and goals and promote successful self-management through education and support.
+ Utilize the most recent national standards and guidelines for diabetes and obesity care in practice, including the ADCES7 Self-Care BehaviorsTM.
+ Use diabetes and weight management technology to assess diabetes status, provide patient care, and track outcomes.
+ Deliver specialized interventions and comprehensive counseling and education services to patients with diabetes and obesity who require nutritional management and diabetes care and education.
+ Implement care plans, communicate plans, establish a follow-up system (outpatient), and maintain documentation.
+ Provide complete, concise, and timely documentation of each patient encounter in the electronic medical record.
+ Develop and contribute to Cleveland Clinic sponsored education programs for physicians, nurses, allied health professionals, students, peers and the public.
+ Use evidence-based guidelines to create and revise education materials related to diabetes and weight management.
+ Participate in program development designed to market and expand departmental services and revenue generation.
+ Market services to potential and current referral sources.
+ Develop and implement services in response to needs and wants of patients, referring providers, and administration.
+ Complete reports for ongoing productivity and outcomes tracking.
+ Adhere to department policies, procedures, quality standards and safety standards and comply with governmental and accreditation regulations.
Minimum qualifications for the ideal future caregiver include:
+ Bachelor's degree or higher in Nursing, Dietetics, Pharmacy or a closely related field
+ Current licensed to practice dietetics or nursing in the State of Ohio
+ Dietitians must have completed a dietetic internship through a credentialed program and have active certification
+ Certified Diabetes and Obesity Care and Education Specialist (CDCES) upon hire or within two years of employment.
+ One year experience as a Registered Dietitian, Registered Nurse or a Pharmacist
Preferred qualifications for the ideal future caregiver include:
+ Connection/experience with diabetes program
+ Skilled in diabetes technology (insulin pumps, CGM, etc.).
+ Understanding of oral and injectable medication for diabetes.
+ Comfortable with virtual and in-person visits, group education and focused behavior change coaching.

- Tracking and documenting student progress, maintaining accurate student files, and generating comprehensive reports detailing student achievements and activities.
- Delivering differentiated and targeted instruction individually or collaboratively in a co-teaching environment to effectively meet IEP-specified educational goals.
- Adapting and modifying curricular content and instructional approaches to accommodate the diverse learning styles and capabilities of students, providing guidance to instructional staff on differentiation methods.
- Utilizing varied and engaging instructional methods, resources, and educational software to effectively respond to and support diverse student needs and interests.
- Managing student caseload responsibilities, including developing, updating, and implementing individualized education plans (IEPs), goals, and interventions.
- Preparing detailed daily lesson plans and organizing instructional materials that align with curriculum standards and individual student objectives.
- Maintaining an organized, safe, and welcoming classroom environment, managing supplies, furniture, and resources effectively according to established procedures.

- Responding openly and constructively to feedback from formal evaluations and informal observations, actively pursuing ongoing professional growth.

‖ Qualifications Required:
- Bachelor's degree or higher in education, special education or a closely related field of study.
- Meet all OH state teaching license and/or certification requirements.
- Licensed currently or in the process of obtaining an OH state special education instruction credential.
- Prior experience working with curriculum development, differentiation and instruction in a classroom setting.
- Prior experience and/or knowledge in special education services and compliance, particularly for students with severe learning, emotional behavioral disorders, and/or other related disabilities.
- Prior experience and highly knowledgeable in working with students with individualized education plans (IEP's).
- Proven success in developing and implementing effective education plans for students with diverse needs.
- Proficiency in providing motivation and having critical conversations with students that help move them toward realistic goals.
- Advanced oral and written communication, organization, conflict resolution, multitasking, problem solving, and decision-making ability.
- Ability to think and act quickly and calmly in an emergency and make independent decisions.
- Knowledge of relevant technology including experience with instructional technology, Microsoft Office Suite, database entry and basic office equipment.

As a Special Education Teacher , you will lead the instructional process for Primary Education students with Individualized Education Plans (IEPs), designing dynamic and personalized learning experiences within the learning center and/or classroom.
