Job Description

Salary: $1125 / Week
The Special Education Teacher, under the direction of the Special Education Program Administrator provides quality services to special education students, kindergarten through grade 12. The Special Education Teacher has a commitment to team participation in planning and implementation of student programs including special instruction, tutorial assistance and consultation with general education staff.
Minimum Requirements:
+ Bachelor's Degree from an accredited university preferred
+ Valid state teaching certificate as required by state, contract/district regulations
+ Minimum of one year experience in teaching environment preferred
+ State Teacher Certification; Type: Standard Elementary Teaching preferred
+ State Teacher Certification; Type: Standard Secondary Teaching preferred
+ State Teacher Certification; Type: Standard Special Teaching preferred
+ Endorsement: Learning Behavior Specialist (LBS1) preferred
+ Current CPR if applicable
+ TB Questionnaire, PPD or chest x-ray if applicable
+ Current Health certificate (per contract or state regulation)
+ Must meet all federal, state and local requirements

‖ Responsibilities Include:

  • Providing vision and leadership to support student achievement and effective operations by supervising staff, developing, and implementing performance management plans, identifying professional learning needs, and participating in professional learning opportunities.

  • Effectively participating in the employment and selection process to ensure an effective site team is in place.

  • Analyzing the professional development needs of staff and aligning needs with the ChanceLight instructional vision, while also actively recognizing accomplishments and addressing issues to ensure productive site operations.

  • Ensuring the safety of staff and students by maintaining the site and classrooms for operation and ensuring compliance and staff awareness with contract and specific state requirements including staffing ratios and crisis protocols.

  • Performing managerial responsibilities including but not limited to overseeing the day-to-day operation of school schedule and activities, custodial and maintenance, food services, transportation services, administration, financial/budgetary operations, resource allocation, and district/community relationships.

  • Driving the enrollment process from referral to last day of attendance; ensuring timely classroom placement upon receiving referral, conducting effective site tours, managing classroom rosters according to student need and state requirements, and participating in the development of plans for transitioning students to the least restrictive environment.

  • Instructing, monitoring, and evaluating all staff and students in the use of learning materials and resources to ensure fidelity of instruction and student performance/evaluation.

  • Conducting Functional Behavior Assessments (FBAs) to identify the causes and functions of students' challenging behaviors.

  • Creating and implementing BIPs that align with ChanceLight standards and guidelines, addressing the individual needs of each student.

  • Supporting the development of measurable, objective IEP goals and facilitating progress monitoring in collaboration with education personnel.

  • Implementing evidence-based interventions by applying scientifically proven behavioral and educational strategies, promoting positive outcomes in academic, social, emotional, and independent functioning.

  • Providing crisis management intervention and support for students exhibiting aggressive or self-injurious behavior and developing strategies for improvement of behavior.

  • Monitoring and ensuring adherence to all Department of Education regulations and internal standards for educational and behavioral services.

  • Maintaining accurate and detailed records of assessments, interventions, progress, and compliance activities.

  • Participating in the assurance and accreditation processes to ensure that the site passes or meets performance plans.

  • Keeping up to date with research-based practices and developments in subject areas, resources and professional development, including but not limited to organization-wide initiatives such as CPI.

  • Providing, soliciting, and responding constructively to formal and informal feedback.

  • Working collaboratively with site team, field level support and national support for the good of the organization.

‖ Qualifications Required:

  • Master's degree or higher in educational leadership, special education, applied behavior analysis or a closely related field of study.
  • Meet all state and district licensing and/or certification requirements.
  • Licensed currently or in the process of obtaining a board certified behavior analyst (BCBA) credential.
  • Licensed currently or in the process of obtaining an educational leadership, administration, principal or similar credential preferred.
  • Licensed currently or in the process of obtaining a state special education credential preferred.
  • Minimum 5yrs prior experience and/or knowledge in special education services and compliance, particularly for students with developmental disabilities and/or emotional disturbance.
  • Minimum 3yrs prior experience working in an educational leadership or school administrator position.
  • Effective management skills and comprehensive knowledge of administrative and school operations, functions, and staffing requirements.
  • Prior experience and highly knowledgeable in the development, assessment, and management of curriculum, content areas and instruction.
  • Highly skilled in working with individuals with severe learning, emotional behavioral disorders, autism spectrum disorders, and/or other related disabilities.

We are seeking a compassionate, experienced, and motivated Registered Dietitian (RD) to join our team as a Diabetes Care and Education Specialist (DCES). In this role, you will empower individuals living with diabetes through education, personalized care planning, and ongoing support. You will work closely with interdisciplinary teams to deliver culturally competent, evidence-based diabetes care across multiple locations.

Provide individualized and group diabetes self-management education and support (DSMES)
Develop and implement care plans in collaboration with patients and providers

We are in need of a diabetes educator to support our current Endocrinology office. The goal of the Diabetes Center is to encourage patients to receive early specialty care and education - getting them on the right track with their diabetes management before returning them to their primary care physicians for ongoing management. In this role, you will be responsible for:
+ Coordinating the diabetes education program.
+ Providing educational resources.
+ Promoting and implementing services that achieve improvement.

+ Has a strong understanding of oral and injectable medications for diabetes.

**Responsibilities:**
+ Provides direct patient care using medical nutrition therapy or diabetes self-management education for assigned patient populations requiring nutrition and/or diabetes support in which medications and monitoring are an integral component in the design of therapy.
+ Collaborates with Endocrinology providers to design and administer nutrition and diabetes care and education that supports patient safety in the setting of high-risk medication use.
+ Completes comprehensive assessments for each patient including emotional and behavioral health, knowledge level, disease history, desired outcomes, and electronic medical record data.
+ Develops an individualized care plan with safe and effective interventions based on the patient' assessed needs and goals, and promotes successful self-management through education and support.
+ Utilizes the most recent national standards and guidelines for diabetes and obesity care in practice, including the ADCES7 Self-Care BehaviorsTM.
+ Uses diabetes and weight management technology to assess diabetes status, provide patient care, and track outcomes.
+ Delivers specialized interventions and comprehensive counseling and education services to patients with diabetes and obesity who require nutritional management and diabetes care and education.
+ Implements a care plan, communicates plan, establishes a follow-up system (outpatient), and maintains documentation.
+ Provides complete, concise, and timely documentation of each patient encounter in the electronic medical record.
+ Develops and contributes to Cleveland Clinic sponsored education programs for physicians, nurses, allied health professionals, students, peers and the public.
+ Uses evidence-based guidelines to creates and revise education materials related to diabetes and weight management.
+ Participates in program development designed to market and expand departmental services and revenue generation.
+ Markets services to potential and current referral sources; develops and implements services in response to needs and wants of patients, referring providers, and administration.
+ Actively contributes to departmental initiatives, including committees, projects, and community outreach.
+ Completes reports for ongoing productivity and outcomes tracking.
+ Participates in professional growth activities; attends and contributes to conferences and seminars on diabetes, nutrition, and related topics.
+ Represents Cleveland Clinic on diabetes and nutrition related matters.
+ Supports departmental research and quality initiatives, and applies current research and evidence-based care to practice.
+ Adheres to department policies, procedures, quality standards and safety standards, and complies with governmental and accreditation regulations.
+ Other duties as assigned.
**Education:**
+ Bachelor's degree or higher in Nursing, Dietetics, or a closely related field.
+ Dietitians must have completed a dietetic internship through a credentialed program.
**Certifications:**
+ Maintains current license to practice dietetics or nursing in the State of Ohio
+ Dietitians must also have active certification as a Registered Dietitian/Nutritionist (RD/RDN).
+ Must earn and maintain certification as a Certified Diabetes Care and Education Specialist (CDCES) within two years of hire/assignment.
**Complexity of Work:**
+ Requires critical thinking skills, decisive judgment, and the ability to work unsupervised or with minimal supervision.
+ Must be able to work in a stressful environment and take appropriate action.
**Work Experience:**
+ Minimum one year experience as a Registered Dietitian or a Registered Nurse.

+ Ability to navigate extensively to provide services to a large number of patients throughout the hospital and clinic sites. Must have manual dexterity to sufficiently perform electronic data entry, and demonstrate devices and medications to patients.
