Position Description:
  • Telephonic and field-based outreach to engage clients in our care management program.
  • Establishes close relationships with and serves as a point of contact for clients.
  • Deliver weekly or monthly health education and promote self-management to clients.
  • Communicate with Care Team members (Care Coordinators, Community Health Worker, Primary Care Physicians and other health care providers) to facilitate client care.

Take assignments accurately. Works as a team member with assigned RN and other healthcare workers to provide care. Able to organize time and prioritize assignments. Visit each assigned patient at the beginning of the shift/assignment and report observations to R.N. Make rounds on assigned patients at least every two hours, observing and reporting changes to the R.N. Use the Multidisciplinary Plan of Care as a guide for individualized patient care and teaching as directed by the R.N. Identifies discharge barriers and communicates to R.N. Assist with or provide activities of daily living (ADLs) including baths, linen change of occupied and unoccupied bed, dressing, feeding, providing water, grooming and hygiene, AM and HS care, mouth/foley/peri-care. Collect simple specimens and record specimen collection. Reinforce pulmonary hygiene including use of incentive spirometer, sets up oxygen, and connects patient to pulse oximeter. Provide nourishments; monitor and set up NG feedings, G-tube feeding or J-tube feeding for the nurse to administer. Under the direct supervision of a licensed nurse, this position assists the RN/LVN in the provision of patient care including patient data collection, oral care, bathing, assistance with ambulating and toileting. This position participates in quality patient care delivery, efficient patient flow and effective communication with the health care team. Integration of patient care standards are incorporated into all clinical activities.

Responsibilities

Demonstrates competence in the performance of duties and responsibilities related to patient care.

  • Provides assistance in activities of patients' daily needs.
  • Records and reports observations of patients to immediate supervisor, including, but not limited to, temperatures, pulse, respiration, blood pressure, weight, daily intake and output, and pain.
  • Transports patients, equipment and supplies.
  • Assists in the preparation of patients for treatment and examination.

Provides for the safety and comfort of the patient.

Demonstrates the ability to safely and competently provide care in emergency situations.

Demonstrates the ability to safely, responsibly and competently use equipment, supplies, and perform delegated therapeutic/diagnostic interventions.

Obtains non-narcotic medications from pharmacy as requested by RN/Physicians in urgent/emergent situations.

Performs other duties as assigned.

Qualifications
  • Current BLS (American Heart Association)
  • Certification (CNA) preferred

6 mos. previous Nursing Assistant/Patient Care Assistant experience and/or completion of the first quarter of an RN or LVN nursing education program

Primary Responsibilities:
  • Meet with new and returning patients to understand their goals and guide them toward the appropriate treatment plan, partnering with the surgical team (Doctor) as the next step in their transformation journey.
  • Educate patients on our unique approach, available procedures, process expectations, and financing/payment options.
  • Coordinate timely follow-up with both the in-center clinical team and the patient to ensure a seamless, positive experience before and after the procedure.
  • Build and manage a strong patient pipeline, including proactive follow-up with prospective patients.
  • Serve as a leader within your center, keeping the team informed, engaged, and focused on delivering an exceptional patient experience.

Compensation:

  • Total compensation ranges between 160,000.00 250,000.00 annually
