**Location: Virtual** - This role enables associates to work virtually full-time, with the exception of required in-person training sessions (when indicated), providing maximum flexibility and autonomy. **Work Shift: 11am - 11pm CST; The NP will work six (6) 12-hour shifts and one (1) 8-hour shift in a two-week period, which includes Saturday and Sunday every other weekend.**
The **Advanced Practice Provider** is responsible for collaborating with company physicians, the patient's other physicians and providers, and their family members to develop complex plans of care in accordance with the patient's health status and overall goals and values. Provides clinical and non-clinical support to patients.
Primary duties may include but are not limited to:
+ Provides primary and urgent health care via telephone and tele video modalities to patients who receive home and community-based services through state Medicaid programs, dual eligible members and other membership as assigned by our MCO partners.
+ Develops and implements clinical plans of care for adult patients facing chronic and complex conditions (e.g., co-morbid medical and mental health diagnoses, limited personal resources, chronic medical conditions.).
+ Gathers history and physical exam and diagnostics as needed and then develops and implements treatment plans given the patient's goals of care and current conditions. Identifies and closes gaps in care.
+ Meets the patient's and family's physical and psychosocial needs with support and input from the company's inter-disciplinary team.
+ Educates patients and families about medication usage, side effects, illness progression, diet and nutrition, medical adherence and crisis anticipation and prevention.
+ Maintains contact with other clinical team members, patients' other physicians and patients' other medical providers to coordinate optimal care and resources for the patient and his or her family in a timely basis and consistent with state regulations and company health standards and policy.
+ Maintains patient medical records and medical documentation consistent with state regulations and company standards and policy.
+ Participates in continuing education as required by state and certifying body.
+ Prescribes medication as permitted by state prescribing authority.
Position Requirements:
+ Requires an MS in Nursing.
+ Requires an active national NP certification.
+ Requires valid, current, active and unrestricted Family or Adult Nurse Practitioner (NP) license in applicable states.
+ Experience working with Electronic Medical Records (EMR) required.
+ Requires 2+ years of experience in managing complex care cases.
Preferred qualifications, skills and experiences:
+ Emergency Room and/or Urgent Care experience is strongly preferred.
+ Possession of DEA registration or eligibility preferred.
+ Experience in managing complex care cases for developmental disabilities and chronically ill patients is strongly preferred.

Purpose:

Support the Medical Specialties Clinic in delivering high-quality clinical care to patients with rheumatologic and autoimmune conditions. This role includes a combination of direct patient care and administrative responsibilities, with a focus on collaborative, evidence-based management of chronic and complex diseases.

Responsibilities:
  • Provide outpatient care to patients seeking diagnosis and treatment for rheumatologic conditions such as rheumatoid arthritis, lupus, vasculitis, and other autoimmune diseases. Work closely with rheumatologists and other specialists to ensure accurate diagnosis, effective treatment, and continuity of care.

  • Collaborate with a multidisciplinary team—including physicians, pharmacists, social workers, and care coordinators—to develop and implement comprehensive, patient-centered treatment plans. Educate patients on disease processes, medication management (including biologics and immunosuppressants), and lifestyle modifications.

  • Monitor disease activity and treatment response using clinical assessments, laboratory data, and patient-reported outcomes. Adjust treatment plans as needed in consultation with supervising physicians.

  • Participate in quality improvement initiatives and contribute to the development of clinical protocols and patient education materials specific to rheumatologic care. Track patient progress and coordinate care across the continuum, including referrals to physical therapy, infusion services, or specialty care as needed.

  • Support the education of medical students, residents, and fellows through clinical teaching, case discussions, and participation in academic activities. Stay current with advances in rheumatology through continuing education and professional development.

Required Qualifications:

The individual will work collaboratively with a multidisciplinary team to serve the growing needs of the pediatric neurosciences program and to deliver excellent pediatric neurosurgical care.

Purpose:

Support pediatric neurosciences in providing clinical care to patients receiving operative and non-operative care for neurological conditions as a member of the Pediatric Neurosurgery service. This position will include a mix of clinical (inpatient and outpatient) care and administrative responsibilities and includes appointment to the allied health staff at Ascension Seton Family of Hospitals.

Responsibilities:

Clinical:

  • Provide care to patients with acute and chronic illnesses encountered in the neurosurgical service (inpatient, emergency department, and clinics).

  • Oversee, collect and prepare patient clinical records, labs and imaging to optimize outpatient consultation in pediatric neurosurgery clinics.

  • Conduct comprehensive or episodic health history and physical assessment of pediatric patients/families with acute, chronic and/or potential health problems.

  • Conduct and coordinate preoperative evaluations for patients proceeding to surgery.

  • Provide care to patients on the inpatient Pediatric Neurosurgery service under the supervision of neurosurgery faculty. This includes participation in daily rounds and collaborating with the neurosurgeons, fellow APPs, and other health care team members in the clinical evaluation and management of patients, as well as communicating the clinical management plan with members of the primary team or with consulting teams.

  • Evaluate patients in follow-up for routine post op care and manage complications related to surgery. Work in collaboration with faculty, residents, fellows, surgeons, and nursing staff to provide on-going inpatient and outpatient care and management.

  • Manage and direct communications between health care team and pediatric patients, their families and referring physicians.

  • Use available tools to track patient progress and coordinate care across the care continuum.

  • Provide after-hours evening, weekend and holiday call coverage as part of the Pediatric Neurosurgery APP team on an agreed-upon schedule.

  • Possible involvement in assisting surgeries and procedures if properly qualified and certified for hospital privileges.

Clinical Quality, Safety and Trials work:

  • Participate in clinical trials investigating new neurological treatments as well as research projects for studying clinical quality, safety and outcomes.

  • Engage in analysis of clinical practice and identify opportunities to increase effectiveness and efficiency to improve value-based, high quality health care paradigms.

Community/Outreach:

  • Participate in building collaborative partnership models across disciplines with community physicians to ensure the provision of comprehensive, multi-disciplinary care.

Administrative:
