Location: This position will perform patient Home Health visits in the Hardee County area.




The role you’ll contribute:




The Home Health Registered Nurse (RN) Care Manager is a professional nurse who coordinates and directs the home care patient's care based on individual patient needs. The RN Care Manager is responsible for independent management of the Home Health patient population requiring the use of advanced assessment, teaching and decision-making skills. The nurse is responsible for ensuring that appropriate referrals to other services are made, interdisciplinary conferencing takes place regularly, and appropriate documentation is completed. Relevant knowledge and experience is consistently applied to new patient populations. The Care Manager cares for a caseload of home health patients by evaluating the patient for appropriateness of home health and developing the home care plan in conjunction with the physician. S/he educates patients, families, caregivers and community providers to safely perform care. S/he provides follow up by evaluating effectiveness of the home care plan, and monitoring patient/family's response to the plan to achieve patient/family goals and top decile outcomes. The Care Manager also identifies performance improvement and home health standard of care initiatives and assists to design or implement programs to address needed changes.

Accurately and timely documents these assessments.
  • Utilizing assessment data, formulates a patient specific plan of care along with the patient, family and physician which is feasible within the physical, financial and emotional resources of the family. Establishes individualized, realistic, measurable patient centered goals in consultation with the patient, family and other health care providers including goals to improve or stabilize patient’s medical condition, functional abilities and promote independence. Considers the physical, cultural, psychosocial, spiritual, age specific and educational needs of the patient when developing the plan of care.
  • Implements the plan of care through direct patient care, coordination, delegation and supervision of the activities of the health care team. Provides care based on physician’s orders, in compliance with policies and procedures, standards of care, and regulatory requirements. Delegates appropriately and provides nursing supervision in the provision of care to patients by other licensed nurses and other personnel. Promotes continuity of care by accurately and completely communicating to other caregivers the status of patient for whom care is provided. Provides skilled nursing care, preventative rehabilitative procedures, and prescribed treatments with a variety of patient populations within various potentially complex home situations.




The expertise and experiences you’ll need to succeed:




Minimum qualifications:




  • Graduate of an approved school of professional nursing
  • 1 year of relevant clinical RN experience
  • Registered Nurse – State Licensure and/or Compact State Licensure In Florida
  • Active American Heart Association BLS
  • Valid in state Driver’s License with current car insurance




Preferred qualifications:




  • Bachelor's Degree in Nursing
  • Recent, relevant experience in a Medicare-certified home health agency as a case-manager
  • Home Health Case-Manager Certification
  • Certificate for OASIS Specialist - Clinical

  • Maintains contact with primary care physicians and specialists, and, when directed, attends doctor visits with mentor and/or individual receiving services.
    • Promotes health and wellness for individuals and ensures nursing actions and steps are put in place for prevention
  • Administers medications according to established guidelines 
    • Provides documentation for all medications given and reviews medication administration records for compliance
  • Oversees the management of high-risk medical conditions and private duty nursing services
  • Develops training flow charts specific to each individual receiving services, provides and documents medical skills training to clients, mentors, respite providers, staff, and where appropriate, the biological family
    • Presents medical skills components at Pre-service Orientation
  • Participates as an Individual Health Plan (IHP) or Plan of Care (POC) team member,
    • Evaluates the medical needs of each individual for initial IHP/POC,
    • Ensures IHP/POC goals and objectives are medically indicated, and
    • Generates ongoing health care protocols for the specific medical conditions outlined in the IHP/POC
  • Provides consultation and technical assistance to staff and mentors regarding home care issues along with operation of durable medical equipment.
    • May be required to coordinate, organize and/or assist with household activities such as light housekeeping and meal preparation; transport individuals receiving services to planned and/or necessary activities and appointments 
    • Participates in on call system as needed
  • May actively participate in IDT meetings, Human Rights Committee, Health and Safety Committee and Administration meetings
  • Performs other related duties and activities as required

  • Provide training to staff members including universal precautions, infection control, and medication administration. 
  • Serve as liaison and advocate with other medical and professional service providers as well as work with external agencies to coordinate services and ensure consistent treatment of individuals receiving care. 
  • Provide training for individuals served with topics including self-medication, prescribed treatments, sex education/STD prevention, and smoking cessation. 

Qualifications:

  • Associate’s Degree in Nursing or related field, or state certificate
  • One year of experience working with population served in a nursing capacity
  • Current state LPN or LVN licensure for state of residence
  • Current driver's license, car registration, and auto insurance
  • Other certifications as required by state licensing regulations such as CPR/FA and HIV/AIDS certification, BBP, and ACT training
  • Ability to effectively communicate with staff, individuals served, and families

RESPONSIBILITIES

Key Responsibilities:  

Act as safety representative responsible for safety policies and procedures of both the company and the customer, including hands-on information regarding operations, safety, equipment, emergency response and administrative functions; 

Hands-on operation vacuum equipment and hydro blasting equipment; 

Operation of special equipment, such as Ultra High-Pressure pumps; 

Operate equipment by energizing the pumping equipment and/or any specialized hydro blasting and/or chemical cleaning equipment, vacuum trucks and cutting equipment being used on the job; 

May be required to examine surface cleaned to ensure conformance to company and customer specifications; 

Perform routine maintenance on company equipment being used on the job; 

Perform any and all duties associated with liquid vacs, air machines and jet rodders; 

May direct a crew of vacuum technicians; 

May transport equipment and personnel to and from customer locations; 

Completes pre and post trip inspections, driver’s logs and complies with all other vehicle policy and DOT requirements; 

Loading and unloading hoses; 

Unload vacuum truck at the dump site; 

Provide general maintenance in the field; 

Perform other related duties as assigned.

QUALIFICATIONS

Required Qualifications:  

Minimum 2 years, verifiable service within the industrial cleaning industry; 

High school diploma/GED or equivalent; 

Strong knowledge in the operation of equipment used in Hydro blasting and vacuum truck; 

Strong communications and interpersonal skills; 

Current certification as an authorized HPC-Industrial driver; Valid driver's license. 

Mechanical aptitude and experience troubleshooting HPC-Industrial equipment is preferred. 

Availability to work all shifts. 

25% travel is required.

Must have valid Class A or B driver's license with Tanker endorsement. 

Must be licensed to drive manual transmission trucks.

Practice Highlights

Busy, established, newly remodeled practice with plenty of growing room
Large staff with 22 employees, including mammogram, ultrasound and surgical suites
Approximately 2.5 days in clinic; 1 surgery day; 2 days covering Labor & Delivery (flexible)
Average daily patient census 35-40 total
Average deliveries 40 - 60 per month
Great mentorship opportunity for resident physicians
1:5 weekend Call (Midwives take 1st call/Physicians take 2nd Call from home)
Deliveries are performed at AdventHealth Sebring Hospital/GYN surgeries are performed at HCA Highlands Hospital
