The role you’ll contribute:
Actively participates in outstanding customer service and accepts responsibility in maintaining relationships that are equally respectful to all. Provides direct and indirect Occupational Therapy services to inpatients and outpatients in the pediatric (1 year old and older), adolescent, adult, and geriatric populations. Employee is responsible to adhere to the Florida Hospital Fish Memorial Corporate Compliance plan, to the rules and regulations of all local, state and federal agencies and to the standards of all accrediting bodies.

- Responsible for direct and indirect clinical services in the occupational therapy in/outpatient areas.
- Attempts initial evaluation of newly assigned inpatients within 24 hours, or documents the reason for the inability to do so in the medical record. Uses appropriate procedures, documents the data using accepted abbreviations, set measurable goals and formulates an individualized plan of care per policy.
- Completes documentation of inpatient evaluations and treatments on the date of service. (Outpatients documentation within one week).
- Provides patient/family education and HEP's for carryover of treatment into the home environment as observed by documentation.
- Provides age appropriate evaluation and intervention techniques to the population served. Completes annual age specific competencies specific to scope of care

The role you’ll contribute:
The Home Health ST Case Manager is a professional therapist who coordinates and directs the home care patient's care based on individual patient needs. Case Manager is responsible for independent management of the Home Health patient population requiring the use of advanced assessment, teaching and decision-making skills. The therapist is responsible for ensuring that appropriate referrals to other services are made, interdisciplinary conferencing takes place regularly, and appropriate documentation is completed. Relevant knowledge and experience is consistently applied to new patient populations. The Case Manager cares for a caseload of home health patients by evaluating the patient for appropriateness of home health and developing the home care plan in conjunction with the physician. S/he educates patients, families, caregivers and community providers to safely perform care. S/he provides follow up by evaluating effectiveness of the home care plan, and monitoring patient/family's response to the plan to achieve patient/family goals and top decile outcomes. The Case Manager also identifies performance improvement and home health standard of care initiatives and assists to design or implement programs to address needed changes.

- Performs an evaluation and utilizes the findings to determine diagnosis (for SLP services), prognosis and plan of care in consultation with the physician. Determines specific interventions, duration and frequency needed to achieve anticipated goals and outcomes.

- Instructs the patient and caregiver regarding the patient’s speech language improvement program including adaptive equipment and the home program.
- Instructs other health team personnel including, when appropriate, home health aides and caregivers in certain phases of speech therapy with which they may work with the patient.
- Selects, applies or modifies skilled intervention to the patient using various skilled interventions consistent with the state practice act to achieve the best outcome possible for the patient. Assesses patient response to interventions and performs reassessments as required.

The role you’ll contribute:

Note: This is a Float position to the following locations: St Pete, Park Blvd, MacDill, Carrollwood, Brandon, Sun City, Lakewood ranch, Sarasota, Sarasota Springs.
ESSENTIAL FUNCTIONS:
- Operates mammography equipment and performs various mammography related procedures.
- Develops and evaluates the film for technical quality such as density contrast, definition, and distortion.
- Explains procedures and educates women about the role of regular mammography in preventive breast health.
- Responsibilities also include preparing and maintaining records and files, and cleaning and adjusting equipment as needed.
- Familiar with standard concepts, practices and procedures.
- Relies on experience and judgment to plan and accomplish goals.
- Performs a variety of tasks.
- Works under general supervision.
- Duties as assigned.

MINIMUM QUALIFICATIONS:
- Requires registration as a Radiologic and Mammography Technologist by the A.R.R.T.
- Requires current Radiology and Mammography license in the State
- SimonMed Imaging requires valid hands-on CPR certification
PHYSICAL DEMANDS: This position may require duties including lifting and carrying up to 40 pounds, sitting for prolonged periods of time, with frequent standing and walking.
DRESS ATTIRE: Business Casual or scrubs dependent on department
