Become the Case Manager you always wanted to be
- Work with interdisciplinary team, guiding treatment plans based on patient needs and preferences.
- Coordinate with interdisciplinary team to establish tentative discharge plan and contingency plans
- Participate in planning for and the execution of patient discharge experience.
- Monitor patient experience: quality/timeliness/service appropriateness/payors/expectations.
- Facilitate team conferences weekly and coordinate all treatment plan modifications.

- Maintain knowledge of regulations/standards, company policies/procedures, and department operations.

- Attend Acute Care Transfer (ACT) meetings to identify trends and collaboratively reduce ACTs.
- Meet with patient/family per Patient Arrival and Initial Visit Standard within 24 hrs. of admission.
- Perform assessment of goals and complete case management addendum within 48 hours of admission.
- Educate patient/family on rehabilitation and Case Manager role; establish communication plan.
- Schedule and facilitate family conferences as needed.
- Assist patient with timely procuring/planning of resources to avoid discharge delays or issues.
- Monitor compliance with regulations for orthotics and prosthetics ordering and payment.
- Make appropriate/timely referrals, including documentation to post discharge providers/physicians.
- Ensure accuracy of discharge and payor-related information in the patient record.
- Participate in utilization review process: data collection, trend review, and resolution actions.
- Participate in case management on-call schedule as needed.
Qualifications
• License or Certification:

- Must be qualified to independently complete an assessment within the scope of practice of his/her discipline (for example, RN, SW, OT, PT, ST, and Rehabilitation Counseling).
- If licensure is required for one’s discipline within the state, individual must hold an active license.
- Must meet eligibility requirements for CCM® or ACM™ certification upon entry into this position OR within two years of entry into the position.
- CCM® or ACM™ certification required OR must be obtained within two years of being placed in the Case Manager II position.
• Minimum Qualifications:

- For Nursing, must possess minimum of an Associate Degree in Nursing, RN licensure with BSN preferred. A diploma is acceptable only in those states whose minimum requirement for licensure or certification is a diploma rather than an associate degree.
- For all other eligible licensed or certified health care professionals, must possess a minimum of a bachelor’s degree and graduate degree is preferred.
- 2 years of rehabilitation experience preferred.

_Weekend Program Guidelines_
* Eligible participants must work 48 weekends per year, 24 hours per weekend (either two 12- hour or three 8-hour shifts per weekend), between the hours of Friday at 3 p.m. and Monday at 7 a.m.
* Eligible participants may use up to a total of 96 hours of a combination scheduled and unscheduled Paid Time Off (PTO) and Ill Time Off (ITO) on the weekends for a maximum of eight weekend shifts off in a fiscal year. Holiday scheduling and work requirements will mirror the department's standard expectations for non-weekend staff.
* Using more than 96 hours of PTO/ITO/UTO in a fiscal year is cause for removal from the program.
* Prorated number of hours applies to new hires and transfers into the program after the beginning of the fiscal year.
* All PTO requests must be approved, in advance, by the participant's director/manager.
* Working and extra shift during the week shall not offset a participant's obligation to work 24 weekend hours per weekend. Weekday hours will be paid at the participant's regular weekday rate.
* Weekend program employees are not eligible to receive paid holiday time off.
* Overtime caused by weekday or extra weekend shifts must be preapproved by the Unit Director/Manager; scheduling or working unapproved overtime is reason for removal from the program.
* Eligible participants must be willing to rotate shifts or work steady off shifts as per unit/department guidelines.
* Eligible participants are not eligible for any bonus pay on the weekends or for a shift differential. Low Census Days will be offered last to weekend staff to support meeting program requirements and are managed as per Full Time staff.
* Recognize relevant years of experience by calculating weekend rate using current hourly rate x differential percentage. o Respiratory Therapists are eligible for a 20% differential percentage. o Retain staff by reducing the number of weekend shifts required for those who currently must work every other weekend.
_Weekend Program Eligibility_
+ * Eligible participants must have a performance rating of Solid/Strong/Good to enter the program, and a Solid/Strong/Good rating to remain in the program. * Minimum of six months of experience required.
+ * If a participant resigns or is removed from the program for not meeting weekend guidelines; he/she is not permitted to return to the program for one calendar year. To be eligible after waiting out a year, the participant must meet all the criteria set forth herein.
+ * If a participant does not meet the minimum weekend program hour requirements, his/her participation will be evaluated for removal from the program.
Program compliance is assessed quarterly. If you do not meet the requirements at any point during or at the end of the quarter, you may be removed from the program.
Responsibilities:
Administers medical gases utilizing the following equipment: cannula, tent, croupette, mask, aerosol mask, HFNC and trach collar.Administers IPPV, IPPB, aerosol, nebulizer, and incentive spirometry treatments. All of the above contain drugs except incentive spirometry.Administers aerosols for the purpose of sputum inductionInitiates and maintains mechanical pulmonary ventilation equipment in the ED, Nursery and ICU settings.
Performs arterial puncture using both the radial and brachial arteries (may use femoral artery with a doctor's order). Performs capillary punctures on newborn infants and small children using either heel or finger sticks.Responsible for operating, maintenance, calibration, and performance of blood gas analyzers.Administers bedside pulmonary function tests.Maintains an adequate level of laboratory supplies for the Blood Gas laboratory.
Prepares and maintains required reports and patient records.Keeps up to date with current trends and developments. Maintains working knowledge of Department procedure manuals. Participates in inservice programs to improve respiratory practice. Employee complies with established occupational exposure policies.Does work rotating shifts as need arises. Assists with planning, coordination, and supervision of the activities of the technical staff during the shift.
Consults with Certified Respiratory Therapists (CRT) and Registered Respiratory Therapists (RRT)regarding proper use of equipment, management of difficult patients, etc.Assists in the evaluation of equipment the Department is considering to purchase.Takes an active role in inservice education of the staff and well as orientation of new Department employees. Assists in the quality assurance program within the Department.

+ Identify clinical, psychosocial, historical, financial, cultural, and spiritual needs that guide the planning process with the patient to attain optimal outcomes. Take patient/family/caregiver level of health literacy into consideration. Evaluate patient/family/caregiver level of understanding and engagement with the progress toward goals and incorporate findings into the plan of care. Balances resources with patient preferences and goals of care. Evaluate the potential impact of social determinants of health that may elevate the risk of a poor transition.
+ Complete detailed assessment on every patient in order to establish understanding of medical and social factors, determine patient's capacity for self-care, identify support systems, outline barriers to discharge, and determine likeliness of requiring post-hospital services and the availability of such services. Continually reassess discharge plan for factors that may affect continuing care needs or the appropriateness of the discharge plan.
+ Facilitate teams to develop and execute safe and efficient discharges. Maintain knowledge about area resources and their capabilities and capacities as well as various types of service providers available. Ensure appropriate arrangements for post-hospital care will be made before discharge and work to avoid unnecessary delays in discharge. Integrate patients' goals, the health care team's assessment, risks and available resources in order to develop and coordinate a successful transition plan.
+ Engage in clear communication with the patient/member/caregivers as well as the interdisciplinary care team in order to develop discharge plans. Serve as a liaison between the patient and the care team. Actively collaborate with the attending practitioner, caregivers, and other members of the multidisciplinary team to coordinate an individualized plan of care. Incorporate discipline-specific recommendations, test results, outstanding orders into discharge plan and monitor/revise and respond to the progression of discharge milestone.
+ Serve as a contact between hospitals and post-hospital care facilities as well as the physicians who provide care in either or both of these settings.
+ Recognize and demonstrate shared accountability in development of a discharge plan with the patient/member/caregiver as well as with team members to ensure optimal outcomes.
+ Align practice with the mission, vision, and values of the organization. Adheres to ethical standards and codes of conduct of applicable professional organization and UPMC. Maintain clinical knowledge of and ensures compliance with regulatory requirements.
+ Advocate on behalf of patient/family/caregivers for services access and for the protection of the patient's health, well-being, safety, and rights.
+ Manage cost of care with the benefits of patient safety, clinical quality, risk and patient satisfaction to provide recommendations and decisions that ensure optimal outcomes.
+ Embrace and incorporate innovation and technology to improve collaboration and patient outcomes. Document care in patient medical chart.
+ Provide staff orientation and mentoring as appropriate.
+ Diploma or associate degree in nursing and active Registered Nurse license.
+ At least one year of experience in discharge planning/care coordination required OR Bachelor's degree in social work or another health or human services field that promotes the physical, psychosocial, and/or vocational well-being of those being served required. Master's degree preferred.
+ At least one year of experience in discharge planning/care coordination required for the Discharge Plan Manager role. No experience required for the Discharge Planning Associate role.
+ 2 years experience in a health care related field strongly preferred.
+ Prior case management/care coordination experience preferred.
+ KNOWLEDGE AND SKILLS: Must possess knowledge in navigating communications with payer sources and programs. Possess knowledge and understanding of regulatory guidelines. Must be skilled in planning/organization, follow up/control, delegation. Problem solving, self-development, organizational behaviors/competencies. Must be able to read, understand, analyze, and interpret medical record documents. Must possess the ability to apply principles of logic and critical thinking to a wide range of problems and to deal with a variety of abstract and concrete variables. Demonstrate ability to function independently, taking initiative to be proactive and drive a discharge plan while working with a multi-disciplinary team. Be able to lead care teams to develop and execute safe and efficient discharge plans. Maintain knowledge about area resources and their capabilities and capacities as well as various types of service providers available. Demonstrate understanding of inpatient care setting operations. Ability to manage multiple priorities in a fast-paced environment.Licensure, Certifications, and Clearances:
+ For Discharge Plan Manager role: Licensed Bachelors Social Work (LBSW) OR Licensed Clinical Social Worker (LCSW) OR Licensed Social Worker (LSW) OR Other Healthcare Professional Licenses for Discharge Planning OR Registered Nurse (RN)
+ Registered Nurses employed in this position are required to maintain active RN license. OR Those without an active RN license, an LBSW or other related healthcare professional license required. CCM or ACM or other nursing or social work certification preferred.
+ No license required for Discharge Planning Associate role (RNs must obtain licensure).

*Current licensure either in the state where the facility is located or, if the facility is in a state covered by the multistate Nursing Licensure Compact (NLC) agreement, a multistate license issued by a participating NLC state. Hires and current employees working on an out-of-state NLC license who later change their residency to the state where the facility is also located will have 60 days upon changing their residency to apply for licensure within that state.

Responsibilities:

Experience Required:

+ Direct patient care experience is preferred but not required.
Qualifications:
+ High school diploma or equivalent.
+ If no previous hospital or pre-hospital training, successful completion of UPMC patient care technician class OR successful completion of the first year of clinical work in a nursing program.
+ Successful completion of basic information system training.
+ Microcomputer experience preferred.
+ Ability to effectively communicate both orally and in writing.
+ Emergency Medical Technician (EMT) or Paramedic or currently enrolled in an EMT or Paramedic program preferred.
+ Access to medications is limited to the distribution of the medication to the nurse.

Responsibilities:
+ Appearance is professional and in accordance with the Department of Laboratory Services dress code and hospital policy.
+ Communicates with patients, explaining procedures prior to venipuncture. Serves patient in a manner conducive to their safety and comfort.
+ Processes outpatients, to include interpreting physician orders, venipuncture and billing.
+ Collects routine, fasting, timed and stat blood samples as indicated on label ordered by a physician and entered into the computer.
+ Properly labels specimens with patient's name, hospital and room number, name of test, date and time collected and phlebotomist's initials.
+ Performs receptionist duties.
+ Prior to collecting blood samples, identifies patient by identification armband, checking it with label using two unique identifiers.
Education and Formal Training Required:
+ High school graduate or equivalent; completion of an acceptable phlebotomy training program.
+ If no previous phlebotomy training program, successful completion of UPMC phlebotomy training program.
Experience Required:
+ Prefer one year experience, however a clinical rotation during phlebotomy training program is acceptable.
Must be able to learn:
+ The practices and procedures required in the work place.
+ The types of tests performed and laboratory terminology.
