**Responsibilities**
Nursing Assistants work collaboratively with our nursing and therapy teams and assist patients in their rehabilitation process. They are obtaining vital signs, help the patient with daily activities, promptly answering call lights, reporting any issues or progress observed, and reinforcing rehab techniques as assigned by the therapy team. Our Nursing Assistants encourage and support patients through the recovery process and gently guide them to reach their goals.

**Job Description Summary:**
This position provides nursing care to patients and families along the health illness continuum in diverse health care settings while collaborating with the health care team. The LPN is accountable for the practice of nursing as defined by the Ohio Board of Nursing. **Responsibilities And Duties:**
25% Performs initial, ongoing and functional health status assessment as applicable to the population and/or individual. Collects and documents initial and ongoing subjective and objective data from patients, significant others, and communities. Reviews data obtained by other members of health care team and incorporates into assessment. Identifies and communicates assessment findings, both initial and ongoing, to appropriate people via referrals, rounds, huddles, reports and care conference, etc. Uses assessment data to identify pertinent nursing diagnoses and collaborative problems. Collaborates assessment findings with providers and members of healthcare team as appropriate. 25% Based on nursing diagnoses and collaborative problems, documents planned nursing interventions to achieve outcomes appropriate to patient needs including: age, size, and development stage. Initiates and/or coordinates care plans and individualizes critical paths/medical management plans. Promotes cost effective care by minimizing fragmentation, maximizing coordination and facilitating patient/family through the health care system. Accountable for quality improvement and clinical outcomes and any other duties assigned. Carries out and documents interventions to assist patient/community to achieve expected outcomes. Uses scientific knowledge, critical thinking, technical skills, and effective interpersonal skills to provide direct patient care as needed.

**PURPOSE AND SCOPE:**

Functions as part of the hemodialysis health care team in providing safe and effective dialysis therapy for assigned patients under the direct supervision of a registered nurse. Participates in the implementation and evaluation of patient care. Responsible for reporting and documenting all new or unusual incidents, information, complaints, or problems to the supervisor. Promotes and assists in the maintenance of a sage and clean work environment.
**DUTIES / ACTIVITIES** :
**CUSTOMER SERVICE:**

**Patient Related:**
+ Provide direct patient care for assigned patient(s).
+ Perform all technical aspects of dialysis procedures for assigned patients as prescribed. Administer and monitor treatments in accordance with current treatment plan or as directed by the supervisor.
+ Monitor and document dialysis treatment parameters on dialysis flow sheets. Document other information related to the care of the patient in the individual patient record.
+ Responsible for reporting any significant information, change in patient condition, or equipment problems to the supervisor.
+ Assist in developing and following the teaching plan to educate the patient and family regarding end stage renal disease, dialysis therapy, diet and medications.
+ Assist with all emergency operational procedures.
+ Administer intravenous medication in accordance with physician orders and state nurse practice laws.
+ Perform and document pre, interim, and post treatment review of patient condition.
+ Initiate basic CPR measures in the event of cardiac and/or pulmonary arrest, and respond to emergency situations related to dialysis treatment.
**Technical:**
+ Safely operate all dialysis related equipment according to the proper procedures.
+ Provide minor troubleshooting when necessary.
+ Ensure a clean, safe and sanitary environment in the dialysis facility treatment area.
+ Ensure all blood spills are immediately addressed according to FMCNA Bloodborne Pathogen Control Policies.
+ Operate all emergency equipment effectively in accordance to the standard operation procedures.
**Staff Related:**
+ Participate in staff meetings as scheduled.
+ Assist in short term and long term patient care plan meetings.
+ Acquire information and knowledge in current practice related to dialysis principals and technique by participating in scheduled in-service classes.
+ Act as a resource person for other staff members. Assist with the coordination of the care plan with Physician, Dietitian and Social Worker.

**PHYSICAL DEMANDS AND WORKING CONDITIONS** **:**

Day to day work includes desk and personal computer work and interaction with patients, facility, staff and physicians. The position provides direct patient care that regularly involves heavy lifting and moving of patients, and assisting with ambulation. Equipment aids and/or coworkers may provide assistance. This position requires frequent, prolonged periods of standing and the employee must be able to bend over. The employee may occasionally be required to move, with assistance, machines and equipment weighing up to 200lbs., and may lift chemical water solutions of up to 30lbs., up as high as 5 feet. The position requires travel between assigned facilities and various locations within the community. Travel to regional, Business Unit and Corporate meetings may be required.
The work environment is characteristic of a health care facility with air temperature control and moderate noises levels. May be exposed to infectious and contagious diseases/materials.
**EDUCATION:**
+ Graduate of an accredited School of Practical Nursing (LPN, LVN)
+ Current appropriate State licensure.
**EXPERIENCE AND REQUIRED SKILLS:**

We're looking for Progressive Care Unit RNs for an immediate travel nurse opening in Columbus, OH. The right RN should have 1-2 years recent acute care experience. Read below for more requirements.

As a PCU Travel Nurse, you will care for patients who require close monitoring and frequent assessment, but aren't unstable enough to need ICU care. Progressive Care Unit RNs will utilize computer equipment to monitor cardiac and other vital information, detecting any changes and thereby enabling intervention of life-threatening or emergency situations. Close observation is necessary as many PCU patients receive a significant amount of complex medications, sometimes through an intravenous line that may need to be adjusted or titrated. The PCU RN is sometimes also called a step-down nurse, and the PCU is also known as cardiac stepdown, medical stepdown, neuro stepdown, surgical stepdown and ER holding.

As a PCU Travel Nurse, you should be prepared to perform the following tasks:

• Provide basic bedside care; stabilize critical cardiac patients.
• Change dressings, insert catheters and start IVs.
• Prepare, administer and record prescribed medication, reporting any adverse reactions to treatment.
• Monitor and adjust specialized equipment used on patients, and interpret and record electronic displays.
• Initiate corrective action whenever information from monitoring equipment shows adverse symptomatology.

PCU Travel Nurses should be able to stand and walk for long periods of time, as well as bend, lean and stoop without difficulty. RNs should be able to easily lift 20 pounds. Moving or lifting of patients may require lifting of up to 50 pounds at times. Because of the fast-paced environment, Progressive Care Unit RNs must possess good skills for coping with stress and be able to relate to people of all ages and backgrounds.

Provides general nursing care to patients and families throughout the continuum of care in diverse, acute health care settings. (Department has scheduled staff to operate 24/7/365).
The RN accountable for the practice of nursing as defined by the Ohio Board of Nursing.
The RN delegates nursing tasks to licensed practical nurses (LPNs) and unlicensed assistive personnel (UAPs) using the criteria written in the Ohio Nurse Practice Act.
According to department policies and nursing care standards, provides professional nursing care services to patients.
Responsible for assessing, planning, implementing, and evaluating nursing care provided to assigned patients.
**Responsibilities And Duties:**
Assessment/Diagnosis - Performs initial, ongoing, and functional health status assessment as applicable to the population and or individual (30%).
Outcomes Identification/Planning - Based on nursing diagnoses and collaborative problems, documents planned nursing interventions to achieve outcomes appropriate to patient needs (30%).
Implementation/Evaluation - Evaluates and documents response to nursing interventions and achievement of outcomes at appropriately determined intervals; as part of a multidisciplinary team, revises plan of care based on evaluative data (20%).
Leadership - Actively participates in process improvement activities to achieve targeted measures of clinical quality, customer satisfaction, and financial performance (10%).
Operations (10%).

**Minimum Qualifications:**
BLS - Basic Life Support - American Heart Association, RN - Registered Nurse - Ohio Board of Nursing

+ Current Registered Nurse license from the State of Ohio and/or current Registered Nurse license in good standing with a multi-state compact state.
+ BLS certification
