Dedicated to addressing a variety of behavioral, emotional and learning problems, we provide a continuum of care from outpatient counseling to residential treatment.
Position Summary:
The Clinical Diversion Specialist will ensure the appropriate placement of youth throughout our array of referral and treatment services for the Cuyahoga County Juvenile Court EIDC (Early Intervention and Diversion Center) and CALM (Coordinated Approach to Low level Misdemeanor) Program. He or she will provide clinical assessments based on verbal, written, and face-to-face interviews to determine appropriate level of treatment for youth either referred to the Intervention Center or brought to the program by the police for domestic violence calls. The Clinical Diversion Specialist may explore alternatives such as kinship placement or respite placement for up to three days, in addition to respite if deemed necessary to remove youth from the home. In less acute cases, he or she will connect youth to appropriate mental health and prosocial services. When necessary, the Clinical Diversion Specialist will complete a youth screener, a safety plan, symptomology screeners, and connect the youth and guardian with ongoing services. This program provides services 24/7.
PRN on-call hours are Saturday and/or Sunday from 8am-12a. PRN staff answer these calls remotely and only go into the office if there is a respite placement. All other duties can be completed remotely.
Essential Duties:
- Responsible for completion of behavioral health and juvenile justice screeners.
- Communicate results of assessment and recommended services to referring court/law enforcement staff.
- Administer, track, and submit outcome measures in accordance with program standards.
- Responsible for case management and referral tracking and ensuring proper funding of all cases before the case leaves intake.
- Face-to-face or virtual admission with clients and guardian including, but not limited to, completing all required clinical and administrative paperwork, completing and submitting behavioral health reports and safety plans to referring court personnel, and, when applicable, completing Psychiatric Diagnostic Evaluations and Treatment Plans for new admissions to Applewood Centers' programs.
- Coordinate and link with other community behavioral health and substance use providers to ensure continuity of care for open cases.
- Education: Minimum Bachelor's degree in Social Work or Counseling required.
- Licensure: Valid Ohio LSW, LPC or MFT required. LISW, LPCC or IMFT preferred.
- Three years' experience in clinical diagnosis and assessment preferred.
- Three to five years' experience in mental health field preferred
- Experience working with children, adolescents, and their families.
- Other: A valid driver's license with approved driving record(less than 6 points), personal transportation and insurance, if required to drive on behalf of the agency.

Program Summary:
It has been estimated that nearly 70% of children currently involved in the Juvenile Justice System experience psychological issues or distress. We have been working within this setting since January of 2016 and in 2020, 76% of the youth seen by Applewood mental health providers had improved behavioral and emotional symptomology during their stay within the facility. We are passionate about this work and are currently seeking qualified mental health professionals to help us continue to address this issue within the Cuyahoga County Juvenile Detention Center.
Position Summary: As a Mental Health Professional at Applewood Centers, you will play a vital role in providing essential mental health services to youth within the Cuyahoga County Juvenile Detention Center. You will be instrumental in addressing the significant psychological issues faced by children in the Youth Justice System, contributing to our proven success in improving their behavioral and emotional well-being.
Essential Duties:
- Provide high quality, detailed and in depth bio/psycho/social assessment for youth referred to program.
- Gather information for ongoing treatment needs and goals and prepare detailed recommendations for clients upon discharge from the facility
- Assess and develop treatment goals and interventions, implement interventions, and identify and targeting of barriers to treatment progress.
- Develop comprehensive, diagnostic case conceptualization as well as focused, measurable and achievable service plans that are consistent with the client's identified needs and cultural assessment-cultivating and maintaining professional alignment and engagement with families.
- Appropriately articulate and present clinical findings, therapeutic goals/targets, and recommendations to court officials/staff in a professional manner. Collaborate with ODYS staff to identify clinical interventions that best support youth in the correctional facility (i.e. safety planning, suicide watch protocols, mental health interventions, and linkage to stepdown programming).
- Form and maintain collaborative working relationships with collateral professionals and/or agencies concerning individual cases.
- Develop and maintain the clinical record through timely and legible completion of assigned documentation according to applicable licensing and accreditation regulations and standards.
- Close cases in a timely manner, accurately completing aftercare plans and related paperwork, including treatment summaries, based on goal achievement and/or agency policy/procedure.
- Attend weekly team meetings and supervisions on time and as scheduled. Be open to feedback and suggestion from others, be prepared for meetings, actively participate as a supportive team member, and maintain a positive working relationship with co-workers.
- Actively and consistently participate in trainings.
- Has/Maintains a good on-time and attendance record.
- Set and maintain appropriate boundaries with incarcerated youth, family members and ODYS staff.
Other Duties:

Qualifications :

Position Summary:
We are hiring a Clinical Therapist to join our School Based team and provide assessments, behavioral health counseling, and consultative services. Services to be provided primarily in the school but also in the community and home setting during non-school time. Schools are located on both east and west sides of Cleveland and surrounding suburbs. We have full time, part time and PRN positions available.
The School Based Therapist provides guidance and instruction to foster emotional, physical and mental growth of clients through therapeutic intervention and positive role modeling. The therapist provides assessments, psychotherapy, TBS, consultation, prevention, and referral to individuals, groups and professional partners within an assigned school as well as within the community and/or office as needed. - Form and maintain collaborative working relationships with natural supports, collateral professionals and/or agencies concerning individual cases.
- Assist and advocate on behalf of clients and families to secure needed resources, services and supports.
- Consult with professional staff at the school regarding treatment planning, progress, and ongoing needs in a timely and consistent manner.
- Provide mental health-related training to teachers and other school personnel.
- Contact and meet with parents of clients for purposes of ongoing training and service coordination. Contact may include home and community site visits.
- Therapist must follow the assigned school schedule and maintain flexibility in scheduling to accommodate evenings for appointments as needed.
- Maintain a professionalism with clients and families that reflects courtesy, respect and the highest ethical standards.
- Complete all required documentation completely, accurately, legibly and within specified timeframes and consistent with contractual obligations.
- Secure supervisory signatures per licensing guidelines and agency policy/procedure.
Qualifications:
- A Master's Degree in Social Work, Marriage and Family Therapy, or Counseling, an LSW, LPC or MFT is required
- Bachelor level licensed social workers who have diagnostic and clinical experience will be considered.
- Prior experience in the delivery of mental health services in the school/community to children and families with complex needs is essential.

Job Description:
We are seeking dedicated, patient-focused physicians licensed in at least to join our team. Whether your background is in primary care, internal medicine, addiction medicine, emergency medicine, or urgent care, this role provides an opportunity to practice modern, high-quality medicine without the constraints of a traditional practice setting. While most of our services are fully virtual, in-person hybrid opportunities may be available.
Responsibilities:
- Provide exceptional virtual care, including patient assessments, medical history reviews, and appropriate medical treatment.
- Deliver compassionate, patient-centered care, building therapeutic doctor-patient relationships.
- Order and interpret lab tests as needed to make informed, high-quality treatment decisions.
- Prescribe medications and provide expert guidance on their proper usage (QuickMD does not prescribe controlled substances, except for buprenorphine for opioid addiction treatment).
- Collaborate with physicians and nurse practitioners, experienced nurses, and a dedicated support staff to ensure comprehensive patient care.
- Maintain appropriate medical documentation to support continuity of care.
- Stay up to date with medical advancements, telehealth innovation, and best practices.
Requirements:
- Active medical license in at least three U.S. states, but preferably five, plus willingness to obtain additional state licenses (assistance provided and costs covered).
- Critical to be licensed in at least five states and/or three or emore of the following states: CA, FL, NC, PA, TX.
- Experience treating patients in telehealth or high-volume in-person settings.
- Ability to schedule and work at least 15 core hours weekly (8AM-6PM locally), over four days each week, scheduling at least one week in advance.
- Strong communication skills and a commitment to medical excellence.
- Comfort using telemedicine platforms and EMRs (training provided).
- Passion for expanding access to care and making a meaningful impact.
- At least one DEA certification and applicable CSL (if required).
