The Advanced Practice Nurse (APN) is a registered professional nurse currently licensed in the State of Texas, who is prepared for advanced nursing practice by virtue of knowledge and skills obtained through a post-basic or advanced educational program of study acceptable to the Board of Nurse Examiners (BNE) and the Texas State Board of Medical Examiners. The advanced practice nurse is prepared to practice in an expanded role to provide health care to individuals, families, and/or groups in a variety of settings. The advanced practice nurse acts independently and/or in collaboration with other health care professionals in the delivery of health care services. The APN is a primary health care provider who provides nursing and medical services to individuals, families, and groups, across the life span, emphasizing health promotion and disease prevention as well as the diagnosis and management of acute and chronic diseases. The APN's services include but are not limited to ordering, conducting, and interpreting appropriate diagnostic and lab tests, prescription of pharmacological agents and treatments and non-pharmacological therapies.
ESSENTIAL DUTIES AND RESPONSIBILITIES:

- Responds to questions from patients receiving service and/or their families directly and proactively. Provides information (in language appropriate to their level of functioning) to assist in understanding and managing symptoms of mental illness.
- Conducts and documents psychiatric evaluations under physicians' supervision, in accord with Metrocare Services procedures.
- Responds flexibly to handle unscheduled evaluations of individuals who walk in and whose level of functioning appears to indicate an imminent crisis or immediate need.
- Takes an active role in interdisciplinary team meetings to discuss evaluations and diagnostic formulations and to assist the team and the patient to develop an appropriate plan of service which clearly states problems; indicates goals, measurable objectives, and intervention strategies to address each identified problem; and identifies staff responsible for each planned intervention within a specified time frame.
- Participates in the development and updating of the individual service plans.
- Demonstrates good professional judgment in implementing interventions as defined in the individual service plan, in reviewing the case with the team as needed, and in modifying the individual service plan as appropriate, based on changing needs of the individual receiving services.
- Documents that the rationale for all interventions, including any changes, has been explained to the individual and that informed consent has been obtained according to procedures.
- Demonstrates good professional judgment in prescribing and monitoring psychoactive medications for each assigned patient.
- Implements and documents medication-related practices in accord with administrative procedures.
- Documents that laboratory studies and/or physical examinations have been ordered, completed, and reviewed per procedure, including prior to beginning psychoactive medications and within recommended frequencies for each medication during the course of treatment. Also documents that appropriate and timely adjustments in medications are made based on laboratory/examination findings.
- Documents provision of medication education to each individual according to procedures.
- Assures that provided care is coordinated with the care of other community medical providers who may also be treating the individual.
- Assures that other providers in 24-hour residential services and/or home caregivers assisting with medications monitoring are informed on a timely basis regarding medication changes, behavioral signs which need to be observed and reported, and of how and to whom to report.
- Prescribes only medications on the formulary, at doses within acceptable ranges as identified in the PDR, with exceptions to be approved by the Center Medical Director.
- Is responsive to psychiatric and/or medical emergency care needs of individuals receiving services.
- Maintains current knowledge of Texas Mental Illness Code and necessary procedures for involuntary hospitalization.
- Works closely with other staff in determining when admission to a more restrictive setting is indicated.
- Accurately completes necessary documentation for obtaining an Order of Protective Custody, when applicable.
- Documentation in individual service records will be sufficient for filing and justification of insurance/third-party claims for the service provided.
- Documentation of time will be in accord with procedures for every service event provided.
- Adheres to procedures in maintaining security of confidential consumer information.
- Schedules sufficient percentage of time for the provision of face-to-face interactions with consumers to achieve productivity benchmarks assigned by DMS.
- Documents these face-to-face interactions in the medical record in a manner consonant with DMS standards for quality of documentation.
- Provides clinical care in a manner consonant with standards, and review of medical records demonstrates documentation of evaluation, treatment planning and medication prescription consonant with quality of care standards.
- Performs other duties as assigned.

QUALIFICATIONS

Master's Degree from an accredited university. At least 6 months experience as an APN in the clinical area.

Basic math skills required. Ability to work with reports and numbers. Ability to calculate moderately complex figures and amounts to accurately report activities and budgets.

- To operate within the boundaries of the parameters set forth for APNs by the BNE for the State of Texas and the American Academy of Nurse Practitioners.

- To identify appropriate diagnostic tests, pharmacologic agents and non-pharmacologic interventions.

- To properly use protocols, standing delegation orders, standing medical orders, practice guidelines or other physician orders for medical aspects of patient care including prescription drug orders that are mutually agreed upon and signed by the APN and physician.

The Psychiatric Advanced Practice RN oversees and performs highly advanced (senior level) psychiatric care under the clinical supervision of an assigned psychiatrist. Assesses, plans, and provides comprehensive psychiatric patient care independently or in collaboration with other health care professionals. Meets face to face with individuals served as needed. Directs psychiatric activities/treatment, monitors trends and activities, and makes recommendations for improvement. Prescribes medications, to include antipsychotics, orders and interprets laboratory and diagnostic testing, consults, and referrals, in accordance with their scope of practice. Performs all other duties as assigned by the supervisor, including planning, assigning, and/or supervising the work of others; and representing the supervisor and/or other psychiatrists in their absence. Participates in the quality assessment and quality improvement initiatives. Serves as a leader of the individual's interdisciplinary team. Works under the clinical and administrative supervision of the Medical Director and/or other delegating physician or psychiatrist with extensive latitude for the use of initiative and independent judgment. May also work collaboratively with physicians and psychiatrists via telemedicine.
This position may be eligible to earn additional pay for work performed on evenings, nights and/or weekends.
Essential Job Functions (EJFs):
Attends work on a regular basis and may be required to work a specific shift schedule or, at times, even a rotating schedule, extended shift and/or overtime in accordance with agency leave policy and performs other duties as assigned. May serve in an on-call status on a rotating schedule as required to meet operational needs of the facility.
Operates within the scope of their licensure to promote and maintain a therapeutic and safe psychiatric milieu. Oversees and provides acute and long-term psychiatric care to individuals, including daily assessment of the individual's mental health status when warranted. The assessment also includes diagnosis, treatment, and development of a plan of care with documentation of the acute issue as it pertains to psychiatric treatment.
Prescribes medications, including antipsychotics, obtain timely medications consents, orders laboratory and diagnostic testing, consults, makes referrals and interprets the results. Refers all care decisions that fall outside the scope of licensure and expertise to the treatment team or a Medical Doctor (MD).
Provides psychiatric care meeting face to face with individuals, which includes an initial and ongoing assessment for individuals with identified acute and long-term psychiatric diagnosis; conducts a review of appropriate treatment specific to the disease process and updates or changes in psychiatric treatment are made as warranted. Writes prescriptions and performs medication reviews, within the scope of his/her licensing credentials and training expertise. Reports any significant trends or issues which occur to the supervisor and provides suggestions for action based upon their expertise.
Provides comprehensive psychiatric care, which includes conducting at least quarterly psychiatric clinics and rounds, and leading IDT (interdisciplinary team) meetings. Conducts periodic reviews of hospitalized patients when on-call for evaluation the reported change in status or condition of individuals under treatment.
Oversees and provides psychiatric diagnosis and the review of the positive behavior support plan (PBSP) and/or psychiatric support plan (PSP), including review of all the identified risks of the individual, (choking, falls, seizures, aggression, etc.) as well as their health status, and ensures necessary supports (OT, PT, Nursing, etc.) have been identified and appropriate areas of need are being addressed.
Oversees and develops accurate and timely documentation, which includes annual summaries, use of chemical restraints and evaluating the correctness of assessments, and compliance with policies and procedures.
Participates in the SSLC wide quality assessment and performance improvement program by performing audits of medical records, monitoring compliance with standards, developing plans of correction, monitoring implementation of performance improvement activities, and evaluating the outcome of those activities. Upon supervisory request, participates in professional practice peer reviews by monitoring the practice of other APRNs and providing feedback to improve performance.
Performs other duties as assigned by the supervisor, including overseeing, planning, assigning, and/or supervising the work of others as well as participating in meetings both internal and external to the SSLC; and representing the supervisor and/or psychiatrist in their absence
Other duties as assigned include but are not limited to actively participating and/or serving in a supporting role to meet the agency's obligations for disaster response and/or recovery or Continuity of Operations (COOP) activation. Such participation may require an alternate shift pattern assignment and/or location.
Knowledge, Skills and Abilities (KSAs):
Knowledge of methods and procedures in the field of psychiatric/nursing including laws and ordinances governing mental health and hospital operations.
Knowledge of principles, methods and procedures for the delivery of medical/psychiatric evaluations, diagnosis, and treatment.

Registrations, Licensure Requirements or Certifications:
Registered nurse in the State of Texas or in another state that recognizes reciprocity through the Nurse Licensure Compact.
Certified as an advanced practice registered nurse by the Texas Board of Nursing.
Must have, or can obtain, a current registration certificate from the Drug Enforcement Agency (DEA).
Must possess a National Provider Identifier (NPI#) as defined by Medicare Part B and enroll as a Medicaid and Medicare Part B provider.
Initial Screening Criteria:
Graduation from an advanced practice registered nurse educational program accredited by a national nursing education accrediting body that is recognized by the U.S. Department of Education and the Texas Board of Nursing with a master's degree or higher level.
At least 2 years of clinical nursing experience providing medical or psychiatric care.
One (1) year of work as a Psychiatric Advanced Practice Registered Nurse (APRN) is preferred.
Experience in supervising or leading the work of others.

REQUIREMENTS:
Computer literate & EMR Proficiency;
Unrestricted APN license in the State of NJ with at least 1 year clinical experience
Current DEA/CDS
Willingness and ability to work independently
Personable, passionate and energetic
Ability to travel within the region
Previous experience with Medicare eligible patients a plus.
Bi-lingual a plus
