  • Educational: Graduate of a practical (vocational) nursing program for LPN/LVN and has a current license as a LPN/LVN in the state(s) in which candidate completed at least 1200 hours of experience as an LPN/LVN or completion of the OCan contact ffice's preceptor program
  • Licensure: Current unrestricted license to practice as Licensed Practical Nurse (LPN) in the state associated with this position   
  • Current CPR/AED/BLS/First Aid certification

Summary of Position:

  4. Supervises the care provided by the CNA's to assure compliance with the interdisciplinary care plan and resident profile.
  5. Participates in educational programs, inservices, meetings and workshops and utilizes current literature to enhance professional skills and knowledge. Attends all annual mandatory inservices.
  6. Participants in quality improvement programs as directed.
  7. Follows standards of the Diligent Minimal Lift Program.
  8. Performs other duties as required.

Minimum Qualifications:

Education: Graduate of an accredited school of nursing for practical nursing

Experience: 1+ years clinical experience in a hospital or similar health care setting preferred.

Certificates, Licenses, Registrations:

Current State of CT license to practice as a licensed practical nurse.

Key Competencies: Ability to work as part of a team and to manage multiple priorities; to speak, read and write English in order to communicate with patients, residents and clinical staff; to effectively interact with residents and staff; organized; ability to utilize a personal computer/CRT to input and retrieve patient information; and familiarity with care plan development.

Provides care and services that demonstrate behaviors and the principles of person-centered care including affording people dignity, respect and compassion, offering coordinated care, support and treatment and care that is personalized.

The Licensed Practical/Vocational Nurse works under the direction of a qualified professional nurse. Participates in the planning, implementation, and evaluation of nursing care, within the objectives, standards, and policies of the department and within the parameters of the licensing board. Communicates and works collaboratively with members of the multidisciplinary team i.e., medical staff, administration, clinicians, dietitians, and milieu personnel. Takes reasonable care and precautions to ensure high client safety standards and a therapeutic environment exist.

Roles and Responsibilities:

  • Provides direct and individualized nursing care to an assigned group of clients under the supervision of a registered professional nurse, while always maintaining the assurance of client safety and following unit and facility policies and procedures. Complies with all company safety policies and practices.
  • Assists in relevant health and psychiatric teaching for client and family. Serves as a role model for clients and peers.
  • Shares responsibility for the planning, implementation, and evaluation of nursing care plans and nursing care.
  • Effectively communicates with the medical staff, receives verbal/written orders, and takes care to minimize abbreviation or other errors. Participates in team conferences, sharing knowledge, information, and ideas among team members.
  • Prepares, administers, controls and accurately records medications in accordance with the policy regarding the administration of medications by a licensed practical nurse, with thorough documentation regarding administration effect and/or adverse effects.
  • Observes, documents, and communicates with/reports to the R.N. in charge any clinical, physical, unit or personnel, or other issues which could alter client care. Reports status changes and concerns to registered nurses and doctors. Communicates with all departments as needed to assure total quality client care is delivered.
  • Consults, follows, and documents with sufficient detail nursing care according to facility policy and procedures. Records client symptoms, responses, and progress concisely, accurately, and completely.
  • Safely and accurately performs vital signs, electrocardiograms, phlebotomy/labs, and diabetic/glucose monitoring. Interprets results and takes appropriate nursing action.
  • Provides coverage in emergency services as needed.

Graduation from an approved Licensed Practical/Vocational Nursing Program required. One or more years' nursing experience within clinical area of service provided by facility preferred. Knowledge of and experience with psychotropic medications and medication administration procedures. Ability to apply nursing standards and practice within a recovery setting. Valid current practical/vocational nursing license from the state or from a nurse compact state, where applicable. CPR and de-escalation certification required (training available upon hire and offered by facility). First aid may be required based on state or facility requirements.

Responsibilities:

  • Set up and operate welding equipment to weld and assemble truck bodies, hoists and fabricated parts (may include robotic welder).
  • Maintain a clean work environment.
  • Assist with continuous process improvement and lean initiatives by following process flow and remaining flexible to complete tasks as part of the team.

Skills/Qualifications:
  • Able to perform basic math to determine ability to measure, assemble and problem-solve.
  • Able to demonstrate welding abi8lity and pass welding certifications.
  • Able to manually move product and materials up to 50 pounds through regular heavy physical exertion, including but not limited to climbing, lifting, carrying, pushing, pulling and bending.
  • Able to stand and work in various positions for several hours at a time. Most of the work is done on concrete, ladders and in dump bodies.
  • Able to determine and make welding equipment adjustments as needed, read complicated instructions, blueprints, measuring devices, and perform basic computer data entry.

Description

Position Summary

Under the direction of an RN, the LPN provides individual and family-centered nursing care and treatment to elderly, acutely ill, or injured residents. Integrates technical skills and theoretical knowledge to meet the individual needs of residents and families. Assists the health care team with the prevention of illness and injury, maintenance of wellness, and provision of comfort.

Essential Functions

  • Provides direct care and/or assists with care as directed and consistent with their scope of practice and competency. Initiates the SBAR process and collaborate with the RN for assessment and recommendations. Makes rounds with physicians and other team members, as needed.
  • Participates in resident evaluations and care planning activities, reviews and revises residents' assessments and care plans as needed to the extent permitted by state nurse practice act. Coordinates residents' care activities and communicates changes to other team members and resident family/significant other. Oversees Medication Aide/Tech in medication/treatment administration. Administers treatments.
  • Reviews, transcribes, communicates and implements physician orders, obtaining RN signature as required by state regulations. Communicates with resident's physician. Assists with emergencies; administers cardiopulmonary resuscitation.
  • Documents care performed and observations of resident status in the clinical record promptly by utilizing standard clinical documentation guidelines as required by the company, and local, state and federal rules and regulations.
  • Participates in medication management to promote optimal safety and effectiveness for residents by
  • administering medications as ordered, including next scheduled dose following admission/readmission.
  • Notifying Charge Nurse of medications not available;
  • Preparing, administering and documenting medications as prescribed;
  • Observing and reporting resident responses to medication;
  • Identifying and promptly communicating adverse drug reactions;
  • Maintaining narcotic records accurately within scope of practice;
  • Completing medication interchange and insurance prior to authorization as assigned;
  • Ordering or arranging for ordering of pharmaceuticals;
  • Notifying physicians of automatic stop orders;
  • Completing recapitulated physician orders review, as assigned; and/or,
  • Cleaning, organizing and replenishing supplies on medication/treatment carts after each use and prior to shift change.
  • Completes clinical admission paperwork for new admissions.
  • Initiates and assists consultants while providing services. Reviews documentation from consultant visit and follows up on recommendation. Assist with follow up on results of qualitative and quantitative medical record audits. Communicates and implements pharmacy review/drug regime review recommendations, as assigned.
  • Assists with clinical discharge process.
  • Ensures punctuality and regular attendance for assigned shifts.
Other Functions
  • Performs other tasks as assigned.
  • Conducts job responsibilities in accordance with the standards set out in the Company's Code of Business Conduct, its policies and procedures, applicable federal and state laws, and applicable professional standards.
Knowledge/Skills/Abilities
  • Ability to react decisively and quickly in emergency situations.
  • Ability to communicate effectively with residents and their family members, and facility staff.


Requirements

Education

Graduate of accredited LPN School of Nursing.

Licenses/Certification

Current valid LPN license in the state employed.

Valid CPR certification.

Experience

Six months experience in a long-term care environment preferred.
