Job Summary

Inspects and directs contractor work associated with main and service installations restorations or general maintenance. Maintains accurate records and drawings as required.

Job Responsibilities
  • Using construction design specifications, inspects and directs outside underground contractor work of gas mains and services and or project restoration work to ensure that they are installed in accordance with all company and industry standards
  • Prepares all documents of construction such as daily progress report, all work tickets and drawing of projects
  • Requisitions company material for job and coordinates pickup and delivery. Reconciles or balance material drawn out, material used, and material returned
  • Responsible for all safety on the job site and in the public way while working and also at the close of the workday
  • Performs physical work including walking distances, climbing up and down ladders, using tapelines, measuring wheels and basic survey equipment such as a construction level
  • Communicates verbally and in writing to shop personnel, contractors and construction management to ensure operational success. Performs work functionally in Excel, Word, Microsoft Project and other "Expert Systems" used in the construction process
  • Communicates and works with customers, builders / developers and contractors in determining the locations of new gas facilities
  • Evaluates new and existing residential, commercial and industrial service applications for new and/or changing gas loads
Minimum Qualifications
  • High School Diploma, HSED, or GED
  • Valid drivers license
  • 1+ years: Standard computer applications (Word, Excel, Power Point, etc.) to create pertinent documentation and supply information to relevant parties

The Field Coordinator is an essential team member of Centers Plan for Healthy Living’s Care Management department and assists with the care of all members enrolled in CPHL’s managed care plans. The Field Coordinator schedules and completes in-person visits as part of routine follow-up and collaborates with Care Management to ensure the member’s care needs are met.

PRIMARY RESPONSIBILITIES:

  • Schedule in-person visit appointments

  • Perform routine in-person visits at the member’s residence (home, nursing facility, adult care facility, etc.)

  • Review and complete in-person visit documentation with the member and/or their caregiver(s)

  • Maintain up-to-date and accurate documentation for assigned cases

  • Follow care coordination documentation guidelines, which includes, but are not limited to: documentation of problems, interventions and outcomes

  • Reach weekly caseload requirement and provide daily report of completed visits

  • Collaborate with Care Management to coordinate resources and services for members, including working with the member and caregiver(s) to ensure safety in the community, providing education, and obtaining relevant information to coordinate care

  • Participate in team meetings and on-going educational activities as required

  • Ensure that all job responsibilities are carried out in compliance with CPHL policies, NY State, CMS, and Federal regulations

**Help at Home is hiring - we offer weekly pay between $17.50-$0.00/hr in our Chicagoland area offices and 17.00- 19.50 outside of Chicagoland**
We are seeking a **Field Coordinator** to provide clerical and customer service support. The employee will be responsible for making daily visits to our clients' homes to ensure the delivery of quality care services from our staff. They will also provide administrative support to their branch by managing phone calls and completing any other clerical tasks.

**Responsibilities**
+ Conduct daily visits to the homes of our clients
+ Enter and submit documentation records of home visits
+ Answer incoming phone calls and redirect calls, as needed
+ Greet and assist guests
+ Assist the Branch Manager with any other administrative/clerical duties, as assigned
**Minimum Qualifications**
+ High school diploma or GED
+ Valid driver's license
