Education: Associate Degree in Nursing required.

Must have and maintain current CPR Certification and comply with recertification on a regular basis. Must have a valid driver's license and be insurable by the Sault Tribe Insurance Department. Must comply with annual driver's license review and insurability standards with the Sault Tribe Insurance Department. Will be required to undergo and successfully pass a background check. Must maintain a good moral character standing.
Knowledge, Skills and Abilities: Knowledge of Nursing Theory, Skills, and Best Practice Standards required. Knowledge of prevention, transmission, and treatment of disease required. Position Summary:
The Maternal Health Navigator serves as the primary care manager for expectant mothers and their families. This role will guide patients through their prenatal and postnatal journeys. They will help navigate them through the various complexities of the healthcare system. The Maternal Health Navigator will be responsible for educating patients and their birthing companions about the birthing process and will ensure patients receive the best care available to them.
Duties and Responsibilities:
- Develop rapport and relationships that foster trust to ensure participants feel comfortable and safe.
- Provide upon enrollment, email/text/phone support during business hours (or after hours under certain circumstances)
- Provide emotional, physical, and educational support during pregnancy.
- Provide at least 2-3 prenatal home visits per patient to discuss their pregnancy needs including, but not limited to creating an ideal birth plan, making informed birthing decisions, educating on active labor, creating a pain management techniques plan, review pain management techniques, begin Lamaze training, and create a postpartum plan.
- Educate the support partner to implement comfort measures, movement, positioning, aromatherapy, and other measures that are supportive of the birthing mother.
- Provide at least 3 postnatal visits to discuss postpartum needs/concerns and postpartum plan including, but not limited to feeding, debrief on labor & delivery experience, meal prep/planning, bathing, swaddling techniques, family planning/child spacing, safe sleep practices, stress management, and ASQ - Development Assessment.
- Provide emotional, physical, and educational support during postpartum.
- Educate caregivers on newborn care.
- Participate in and complete an 8-week online Doula Certification Training.
- Complete all required documentation and reports in a timely fashion and in the manner required by program contracts.
- Participate in staff meetings, supervision, agency meetings and staff trainings.
Competencies
- Bachelor's degree with minimum 3 years' experience working in a community clinic or related field OR
- Associate Degree in related fields with 4 years' experience working in a community clinic or related field.

Reporting to the Sexual Health Program Manager, the Health Navigator provides outreach, risk-reduction counseling and HIV/Hepatitis C/STI screening services, check-in, and linkage to care services with a focus gay, bisexual and other men who have sex with men (MSM), transgender and gender nonconforming populations and people who inject drugs (PWID).
Primary Responsibilities:
Client Engagement

Following the workflow, register and check-in patients during walk-in hours
-
Communicate with the navigators, nurses, and providers about patient’s needs after they are checked into the Epic system
-
Answer the Sexual Health Hotline to screen and triage callers
Provide Direct Services in Clinic

Always create a non-judgment, patient centered atmosphere for patients

Discuss basic HIV and STI prevention information and the importance of accessing care with patients (with a focus on our priority populations)

Complete demographic and risk assessment forms in Epic to understand the patient’s risk factors while using motivational interviewing techniques to ascertain needed information

Conduct serum testing for HIV, HCV and syphilis, performing phlebotomy

Create specimen collection forms that are sent to the Massachusetts State Lab and Quest Lab orders

Communicate with providers, nurses, and other navigators about patient’s care throughout clinic

Document results to patients by accessing the Massachusetts State Lab Electronic Laboratory Reporting and Communication, according to established workflows

Collaborates with other members of patient programs department. Participates in approved community outreach and education events. Through outreach and visits support productivity and health outcome targets for department wide disease management activities. Supports with recruitment, coordinates logistics, and when needed supports with facilitation of Diabetes Prevention Program. This position is located at our Humboldt Park location, 2750 W North Ave, Chicago, IL 60647.
Member Duties : Health educators promote healthy lifestyle modification and deliver self-management education to at-risk and high-risk patient populations via in-person and/or telehealth visits. Health Educator delivers 1:1 health education visit, facilitates group sessions, completes disease management assessments, conducts outreach, and schedules appointment. This position requires someone with deep commitment to providing compassionate care that honors and individuality of every patient. An ideal candidate enjoys patient interaction and collaborating with program leaders to coordinate services/programs, have strong communication skills to effective communicate with patients, health care providers, and team members and enjoys working as part of a collaborative team to achieve high levels of productivity and maximize services, flexibility to adapt to change, and ability to independently handle patient care responsibilities and other work as assigned.

This job performs delegated nursing care responsibilities to Adult/Geriatric/Medical
Psychiatric patients within the scope of guideline set forth by the physicians, treatment team, and other clinical Staff in accordance with Unit/Hospital policies and procedures, and local, state, federal, and Joint Commission on Accreditation of Healthcare Organizations (JCAHO) standards and requirements to provide efficient individualized care.
**Education**
Required - Bachelor's degree in psychology or sociology or related field.
**Work Experience**
Preferred - Prior experience in psychiatry.
**Certifications**
Required - Basic Life Support (BLS) from the American Heart Association.
Crisis Prevention Intervention (CPI).
**Knowledge Skills and Abilities (KSAs)**
+ Working knowledge of medical terminology.
+ Ability to file alphabetically and numerically, legible handwriting, spelling/grammar aptitude.
